[On validity of intralacunar exposure to surgical laser beam in chronic tonsillitis (experimental study)].

An experimental study was made on the removed tonsil to validate clinical efficiency of intralacunar impact of surgical laser beam. The experiment was based on computed tomography of the removed tonsil with intralacunar introduction of the contrast medium before and after intralacunar impact of surgical laser beam. The study demonstrates changes in the lacunar apparatus of the palatine tonsils in response to laser beam radiation. Dilation of the lacunae, disappearance of lacunar narrowings and branches promoted improvement of the drainage function of the lacunar complex of the palatine tonsils.

[Possibilities of computerized tomography in destructive bone changes in patients with polypous rhinosinusopathy].

The authors made a CT investigation of 15 patients (11 men and 4 women) suffering from polypous rhinosinusopathy. Thirteen patients prior to it underwent polypectomy of the nose. Ophthalmocele was observed in 7 patients. The results of CT investigation made it possible to define the spreading of the destructive process in the accessory nasal sinuses in its polypous lesion and reject a tumor process.

[Total sequestration of the labyrinth].

At present total sequestration of the labyrinth in the case of purulent epitympanitis and cholesteatoma is a rare pathology. This paper describes data from the literature and two observations by the present authors. They offer an efficient method for arresting labyrinthectomy-induced liquorrhea by means of a flap of the lip mucous membrane. They maintain that computer tomography combined with traditional tomography is an informative method for diagnosing labyrinth destruction. They assert that continuous destruction of the labyrinth during prolonged pyorrhea and progressive development of cholesteatoma can be stimulated by functional changes of the body, e.g. pregnancy. They also suggest that the vestibular apparatus has high compensatory potentials under the conditions of a complete destruction of the vestibule and semicircular canals as a result of extended inflammation of the middle and internal ear.

[Role of computerized tomography in the diagnosis of osteomas of the sphenoid sinus with intracranial extension].

The paper reports two rare cases of sphenoidal osteoma which occur statistically in 1.9% of sphenoidal osteomas. Two women underwent computed tomography because of complaints of progressive headaches. The patients had recurrent unilateral exudative otitis media. CT localized the tumor spread into the cerebral cranium and promoted further treatment design. The authors state that indications to CT diagnosis in patients with headaches should be increased. It is also suggested to make CT-diagnosis more frequently in cases of recurrent unilateral exudative otitis.

[Computerized tomography assisted determination of the scope of surgical intervention in intracranial involvement in diseases of the middle ear and paranasal sinuses].

The details are provided on the role of computed tomography (CT) in determination of indications for and scope of surgical interventions in polypous rhinosinusopathy, osteomas of the sphenoid sinus, total sequestration of the labyrinth, chronic otitis media purulenta invading the orbit and the brain. CT permitted optimal surgical intervention and prevention of serious postoperative complications.

[Asymptomatic osteoma of the frontal sinus with intracranial invasion as a cause of post-traumatic pneumocephalus].

A rare case of frontal sinus osteoma growing intracranially in a female of 38 is reported. The disease ran without any otorhinolaryngological or neurological symptoms until the sinus blunt injury followed by headaches resistant to analgetics and other neurological signs. Computed tomography discovered osteoma involving the cerebral cranium and pneumocephalus in the right cranial fossa. Management of patients with paranasal sinuses should account for possible intracranial complications and CT control of the tumor growth is very much desired.
