[The loss of volume in modern respirators during change of compliance and resistance (author's transl)].

Respirators with constant volume proved to have a distinct loss of originally set tidal volume, when mechanical respiratory disturbances are present. In cases with decreased compliance the loss of volume is mainly caused by compression of the basic volume of the apparatus. In cases with increased bronchial flow resistance - unfavourable inspiration - expiration ratio, too short or even lacking endinspiratory pressure plateau result in the distinct decrease of the set respiratory minute volume, which then requires an appropriate correction. The degree of volume loss was measured and analysed in 10 respirators which are in current use.

Airway Resistance

[Report on an emergency of alveolar leakage following lung decortication (author's transl)].

A case of respiratory distress is reported following lung decortication. Of a 331/min applied respiratory volume 90% was lost due to the multiple alveolar leakage. The lung could be inflated only when the chest was closed and several Bülau drains with powerful suckers were inserted. When the lung surface came in contact with the chest wall, the lossof tidal volume could immediately be reduced and respiration was possible again.

Calcinosis

[Controlled hyperventilation with dead space (author's transl)].

We report on a new kind of ventilation-hyperventilation, whereby dead space is added between the patient and the respirator. This kind of ventilation is compared with positive end expiratory pressure ventilation together with intermittent sighs on 10 dogs. Over a period of 80 hours no respiratory or circulatory differences were found in the two groups. There also was no difference in clinical or anatomic histologic findings. It was demonstrated that dogs can be artificially ventilated for a much longer period than estimated. The findings of "respirator lungs" were only shown in an emphysema of the lungs.
