[Eye protection for servicemen under battle conditions by the use of special glasses].

The paper substantiates a possibility of eye protection for military men in combat with special glasses and corrective spectacles. It shows that explosive shrapnel wounds dominate over the gunshot ones of visual organs. In the overwhelming majority of cases they are produced by small splinters with kinetic energy less than 18 J. It is found that the high-impact resistant polymeric optical material can be used for effective eye protection, which should result in a considerable reduction of eye wounds.

[The delivery of ophthalmological care in World War II].

The basic questions of organization of the military field ophthalmology were worked out by B. L. Poliak and tested on practice before the Great Patriotic war during Japanese conflict on the Khalka [correction of Khalkhin-Gol] River and in Soviet-Finnish war. Ophthalmological care during the Great Patriotic war had a number of innovations, such as stitching of fibrotic membrane, electromagnetic removal of foreign bodies, and application of complex treatment made it possible to lower the cases of preventive enucleation and reduce the time of plastic operations. In the result of these methods the blindness came to 3.9% (in US Army--5.1%).

[The prospects for improving medical care for minor casualties and patients in ophthalmology].

As for modern war the casualty rate of ophthalmological minor wounded will be approximately 1.7% of all sanitary losses. The improvement of medical care to this category of patients will be possible if some organizational principles are observed: selection of these patients into a separate group in order to promote their rapid evacuation to the hospital for minor wounded (HMW); existence of physician-ophthalmologist at the stage of secondary care for qualified triage, immediate and holding care, and pre-evacuation procedures; application of modern ophthalmological facilities in field conditions; optic correction of sight in patients at HMW.

Eye Diseases↗

[Experience in rendering ophthalmological care to the wounded].

Medical losses from ophthalmological injuries inflicted by small arms were from 2.2 to 4.5%, and taking into account concomitant injuries--up to 5.6%. Frequently there were binocular lesions, perforating wounds and destructions of eyeballs. Complicated ophthalmological operations were performed on patients who simultaneously had severe injuries of extremities, chest, abdomen, etc. Vitrectomy was a basic stage in the treatment of eye-penetrating wounds. Application of active measures for management of ophthalmological wounds at the stages of medical care made it possible to improve the outcome of treatment, diminish the frequency of purulent complications, for example endophthalmitis by 2 times.

[Immobilized thrombolytic enzymes and their use in ophthalmology].

Soluble highly active conjugates were obtained after complex formation or covalent binding of trypsin and urokinase with polymers of various chemical structure as well as by means of condensation of the enzyme molecules as a result of which oligo- and heterooligoproteins were produced. Pharmacological activity of the conjugates obtained was studied in the course of treatment of rabbits with experimental hyphemia. All the preparations produced exhibited high therapeutic efficiency: the time of hyphemia resorption was decreased, the effect of immobilized enzymes was prolonged as compared with controls. Dependence on the molecular mass and the dose of enzymatic preparations of the period of hyphemia resorption was studied using heteromers of urokinase and hemoglobin. Maximal activity was noted with the preparations of molecular mass 4.10(5) daltons; they maintained the optimal level of pharmacological activity even after 4-fold decrease in the dose used as compared with other immobilized preparations of trypsin and urokinase. The immobilized preparations were 20-fold more effective as compared with native unmodified enzymes.

[Differentiated approach to the treatment of autoimmune ophthalmopathy].

The results of comprehensive examinations and therapy of 67 patients with autoimmune ophthalmopathy are presented. In 80% of the patients the disease coursed along with thyrotoxicosis, in 10% thyroid function was reduced because of autoimmune thyroiditis, and in the rest autoimmune ophthalmopathy was an individual entity. A staged therapeutic complex for the treatment of this condition is suggested, aimed at correction of thyroid function (stage 1), followed by pathogenetic therapy including antiinflammatory measures, glucocorticoids and nonsteroid antiinflammatory drugs to arrest aseptic inflammation in the orbit, and immunosuppressive therapy to correct the immunity shifts. After clinical and immunologic remission was achieved, courses of maintenance therapy (nonsteroid antiinflammatory drugs) were carried out for 3-5 months monitored by blood analyses and ophthalmologic examinations, immunity status tests, CT clinical data. Choice of the treatment strategy was governed by the morphologic variant of ophthalmopathy, immunologic activity of the process, disease severity and pattern. Such an approach helped achieve positive results and clinico-immunologic remission, as well as timely prevent the progress of the disease.

[Soluble heteroprotein conjugates based on recombinant pro-urokinase for ophthalmology].

A procedure is developed for precise synthesis of soluble highly active heteroprotein conjugates enriched in the enzyme with prearranged molecular mass and composition, containing blood serum albumin and recombinant prourokinase. Correlation between the pharmacological activity of the heroconjugates produced and their molecular mass from 2.2 x 10(5) to 5 x 10(5) was studied in experimental vitreous body hemorrhage (hemophthalmia). Heteroconjugates with molecular mass 5 x 10(5) and more exhibited distinctly higher activity due to prolonged fibrinolytic effect. The best medicinal efficiency of the immobilized prourokinase with molecular mass 9 x 10(5) proved to be also in 50-fold decrease of its dose administered into the vitreous body as compared with native enzyme effect.
