Tunable continuous-wave operation of a Cr3+,Li+:Mg2SiO4 laser.

Room-temperature tunable cw laser oscillation of Cr3+ ions in forsterite crystals (Mg2SiO4) codoped with chromium and lithium has been demonstrated, for the first time to the authors' knowledge, in pumping a crystal with a Ti:sapphire laser into the first excited level of Cr3+ ions (4A2-4T2 transition). The laser is smoothly tunable in the region of 1095-1170 nm. A slope efficiency as great as 4.4% has been achieved.

[Immunopathogenesis of severe wounds and traumas: possibilities of immune correction].

The authors describe the present-day views on the nature of immune dysfunctions in severe traumas. Based on personal clinical experiences and literature data the authors discuss the role of immune dysfunctions in pathogenesis of the traumatic disease. Special attention is given to the role of the immune system in the development of the life-threatening condition: polyorganic insufficiency whose formation mainly results from disorganization and functional failure of the system of immune reactivity. Clinical investigations have shown high effectiveness of early administration for severe wounds and traumas of a new means of immunocorrection--yeast recombinant interleukin-2 of man (preparation Roncoleukin). The administration of this immunocorrector in complex schemes of intensive therapy of the victims was shown to prevent the development of severe pyo-septic pathology and perfectly change the course of the traumatic disease.

[Rendering anesthesiological and resuscitation care to wounded by a medical reinforcement group in an armed conflict].

The authors describe the aims and content of anesthesiologic and reanimatologic care of the medical reinforcement group in the medical institution of the 1st echelon of specialized care. Basing on the experience of treatment of 825 casualties with gunshot injuries the rational methods of anesthesia and intensive care are shown including the prolonged controlled ventilation, infusion-transfusion therapy, early enteral nutrition. The main causes of lethal outcomes are analyzed. The conclusion was made that during counter-terrorist operations it is reasonable to include anesthesiologists and reanimatologists into the medical reinforcement group.

[Infectious complications of severe trauma: achievements and problems].

On the base of treatment results obtained in 2000 casualties the pathogenically substantiated classification of infectious complications of the injuries and traumas was proposed. The principles of classification are in conformity with modern ideas connected with the conception of systemic inflammatory response, the theory of general adaptation and the concept of traumatic disease. The classification determines the purposeful diagnostic tactics and provides the preventative therapy of traumas' infectious complications. Selection of special direction in the surgery of injuries in order to solve the problem of traumas' infectious complications in scientific, methodical and practical aspects is substantiated. First of all it is creation of specialized centers that provide the introduction into medical practice of the modern conception of traumas' infectious complications.

[Acute insufficiency of the gastrointestinal tract in patients with severe gunshot injury].

Acute insufficiency of the gastrointestinal tract (GIT) in patients with severe gunshot injuries is an important link of pathogenesis of the polyorganic insufficiency syndrome. The character of the wound, the numerical score objective assessment of the injury severity and severity of the patient's state are considered to be criteria of early diagnosis of a risk of the development of acute insufficiency of GIT. The specific feature of "general" intensive therapy of acute insufficiency of GIT in severe gunshot traumas is the necessary application of regional anesthesia, sympatholytics, anticholinesterase agents and H2-blockers. Intensive "enteral" therapy of acute insufficiency of GIT in severe gunshot wounds includes the measures resulting in improvement of microcirculation, tissue respiration in organs of GIT, decompression of the stomach, local defense of mucosa, detoxication and early enteral balanced nutrition. The described method of treatment of wounded to the stomach used at specialized medical institutions resulted in 6.2 less lethality among this category of patients.

[6 principles of the therapeutic and diagnostic process in severe combined mechanical injury and the characteristics of their realization during the staged treatment of victims].

Great experience with treatment and a comprehensive investigation of the severe combined trauma allowed the authors to choose six basic principles to be followed in diagnosing and treatment of this surgical pathology whose peculiarity is the phenomenon of mutual aggravation of the injuries. Following these principles, especially at surgical hospitals, promotes complete diagnosing, choice of the rational treatment policy in order to avoid medical errors and severe posttraumatic and postshock complications and to improve the outcomes. The authors prove expediency of organization of the multifield specialized centers for treatment of patients with the severe combined traumas.

DNASUN: a package of computer programs for the biotechnology laboratory.

The paper describes a new software package DNASUN developed for supporting gene engineering laboratories. The package provides a user-friendly interface for experimental researches and supports the traditional nucleotide/protein sequence analysis as well as physical mapping, sequencing, plasmid manipulations, optimal oligonucleotide probe selection and other common molecular biology procedures.

A kinetic model of RNA folding.

The RNA folding process is represented as a Markov process with states corresponding to RNA secondary structures and transition probabilities corresponding to transformations of a secondary structure caused by formation or disintegration of a helix. Transition probabilities (kinetic constants) are determined. A notion of a group of structures is introduced, and it allows to reduce the state space. Energetic and kinetic parameters of pseudoknots are estimated. Algorithms for computation of a kinetic ensemble for structures and groups of structures are presented, as well as their modifications that take into account pseudoknots. The described algorithms are implemented as a procedure for prediction of RNA secondary structure that is included in the package DNA-SUN.

[Endotoxemia as a problem of clinical surgery].

The authors have formulated a modern concept of endotoxicosis. The ideas of its essence were formed on the clinical model of the severe associated mechanical trauma. Principles of the complex treatment of endotoxicosis are described.

[Approaches to decreasing polypragmasy in intensive care of patients with severe injuries].

A total of 277 patients with grave combined injuries were treated. Polypragmasy involved all the trends of intensive care and was most often observed in infusion-transfusion and drug therapy. Three main approaches to decreasing polypragmasy in intensive care of victims with grave combined injuries consist in decreasing the volume of infusion-transfusion therapy, use of a limited number of antihypoxants, and early short courses of antibiotics.

[Anesthesiological aspects in determining indications and contraindications for surgical interventions in patients with severe wounds and injuries in the post shock period].

Approaches to validation of indications and contraindications for interventions in wounded patients and victims were developed on the basis of many-year experience. The desirability of intervention is determined primarily by strategic tasks which it solves. The operation is to be performed despite severe clinical condition if it removes the cause of critical state or creates conditions for preventing life-threatening complications of the traumatic disease. All the rest interventions can be postponed till complete or partial compensation of vital systems.

[The modern problems of intensive care in patients with wounds and traumas].

The results of rendering the intensive care to 2244 patients with severe wounds and traumas were analyzed, which made it possible to define the most topical problems needed to be resolved, i.e. treatment schemes for patients with vegetative state (similar to coma) and with asthenic syndrome, perfection of intensive care for the elderly and absolutization of some intensive care techniques. Perspective solutions are suggested to cope with the above issues, which comprise a bigger quantity of rehabilitation centers, an elaboration of pathogenetic tactics of intensive care for the elderly patients and revising the feasibility of a long-term use of the intensive care methods with respect to the regulation and integration suppressed by such methods.

[Posttraumatic liver failure].

An examination of 166 patients was performed at the early postshock period of trauma disease. Results of the investigation of the functional and morphological state of the liver, factors of pathogenesis of posttraumatic hepatic insufficiency are analyzed.
