[Recognition of actual periods of nocturnal rest from 24-hour heart rate profile].

UNLABELLED: Inability to delineate exactly periods of nocturnal sleep and diurnal wakefulness during 24-hour blood pressure (BP) monitoring causes reporting of erroneous BP values for these periods. We suggested a simple mathematical algorithm for detection of periods of nocturnal rest determined as period of low values of cardiac rhythm using data of BP monitoring itself. AIM: To compare novel method of recognition of periods of sleep with 2 standard techniques: use of fixed time interval between 23 and 7 hours (1), or period of sleep according to patient's diaries (2). Reproducibility of nocturnal BP lowering between two 24-hour intervals during 48-hour blood pressure monitoring was used as a measure of precision of determination of diurnal/nocturnal BP. METHODS: Ambulatory 48-hour BP monitoring was carried out in 33 patients with uncomplicated stage II hypertensive disease. Automatic analysis of BP monitoring data was performed with the use of specially designed computer application. Standard deviation (SD) of differences (SDD) between pairs of nocturnal BP lowering during 48 hours was used as a measure of reproducibility. RESULTS: Reproducibility of values obtained with novel algorithm (SDD for systolic/diastolic BP 6.7/8.2 mm Hg) was substantially better than those obtained with standard methods (1) and (2) (SDD 13.0/14.8 and 13.5/18.3 mm Hg, respectively). CONCLUSION: The proposed method of recognition of the period of nocturnal rest substantially improved precision of automatic analysis of 24-hour BP monitoring.

[Function of the brain venous system and a 24-h profile of arterial pressure in hypertensive patients].

AIM: To study blood flow along the internal jugular veins (IJV) in the initial state and under the action of nitroglycerin in patients with essential arterial hypertension (EAH) with reference to characteristics of a 24-h profile of blood pressure. MATERIAL AND METHODS: A 24-h monitoring of blood pressure (MBP), duplex scanning of IJV, venous IJV outflow were studied in 26 patients with EAH stage 2 (mean age 39 +/- 5.0) before and after sublingual intake of nitroglycerin (0.5 mg). RESULTS: By 24-h MBP findings, two groups of patients were identified: with unaffected blood pressure profile (group 1) and affected profile (group 2). Nitroglycerin produced a unidirectional reaction of IJV (increased outflow) in group 1 and multidirectional changes of venous outflow along the IJV in patients of group 2. CONCLUSION: Nitroglycerin test showed different venous cerebral hemodynamics in EAH patients with different variants of 24-h MBP.

[Relationship between changes of endothelial vasodilator function, vascular and cardiac structure and derangements of blood pressure 24-hour profile].

AIM: To assess relationship between 24-hour blood pressure (BP) rhythm and structural and functional state of the heart and blood vessels. MATERIAL AND METHODS: Echocardiography, sonography of carotid artery intima-media, and assessment of endothelium dependent (flow mediated) and endothelium independent (nitroglycerine) brachial artery vasodilatation were used in the study of 83 patients with stage I-II essential hypertension (mean age 49.2+/-6.2 years). Seventy four patients received indapamide and enalapril for 24 weeks. RESULTS: According to results of 24-hour BP monitoring the patients were divided into 2 groups with normal (dippers) and abnormal (non-dippers) BP rhythm. Non-dippers compared with dippers had more pronounced lowering of endothelium-dependent autoregulation of arterial tone, bigger left ventricular mass thicker carotid artery wall. Linear correlation was found between changes of 24-hour BP rhythm and structural and functional parameters of the heart and vessels. More pronounced regression of their derangements occurred during antihypertensive therapy in those patients in whom normalization of 24-hour rhythm was achieved.

[A decrease in left ventricular hypertrophy and the dynamics of the parameters of circadian BP monitoring under the influence of ramipril in patients with essential arterial hypertension].

To evaluate factors of action of ramipril, an inhibitor of angiotensin-converting enzyme, which may induce a decrease in left ventricular hypertrophy (LVH), 45 LVH patients aged 21-53 years with mild and moderate essential hypertension have underwent echo-CG determination of left ventricular mass and 24-h monitoring of arterial pressure (AP). Multiple regression was used to examine prognostic significance of such parameters as age, sex, height, weight, duration of the disease, mean 24-h AP, its variability and the degree of nocturnal fall. It was found that ramipril reduced LVH irrespective of the hypotensive effect.

[The use of a new method for determining the ratio of the glomerular filtration rate to circulating plasma volume in patients with arterial hypertension].

Renal scintigrams with 99Tc-DTPA were analysed retrospectively in 19 patients with essential hypertension. Glomerular filtration rate (GFR) was measured by Gates technique. Ratio GFR to plasma volume (PV) was quantified by A. M. Peters' technique (1994). PV was calculated as the relation of these two indices. Total GFR in hypertensive patients widely ranged (from 76.8 ml/min to 165.7 ml/min). Three groups were selected taking into account hypertension duration: patients with decreased, normal and increased values of basal GFR. The severity of hypertension did not affect the GFR, but showed positive and strong correlation with PV (r = 0.91; p < 0.05). GFR/PV was decreased in hypertensive patients. Factors affecting the GFR in hypertensive subjects are discussed.

[Study of myocardial perfusion in patients with hypertension].

Exercise 199Tl myocardial scintigraphic data were studied during dipyridamole test in 39 patients with Class II essential hypertension. Transient myocardial perfusion defects were revealed in 60% of patients with negative exercise test. Intraventricular administration of albumin microspheres in 8 females with normal coronary artery allowed the authors to detect areas of their accumulation at the same sites as in exercise 199Tl myocardial scintigraphy. Thus, the changes in the small vessels which are revealed by intraventricular albumin microspheres are one of the causes of impaired myocardial perfusion in patients with essential hypertension.

[Diagnostic usefulness of ECG changes in response to exercise in women with various forms of ischemic heart disease].

The results of bicycle ergometry and pharmacological tests with isoproterenol and dipyridamol, 24-hour monitoring and blood levels of endogeneous opioids were studied in 99 females with chest pain who had undergone angiography. Coronary microcirculation was examined in 29 patients by introducing albumin microspheres into the left ventricle. The angiography revealed coronary atherosclerosis in 30 patients, whereas its signs were not found in 8 females with documented coronary heart disease (CHD). The predictive value of positive exercise tests in females with angina pectoris was higher for the diagnosis of CHD, including its types without coronary atherosclerosis. In patients with cardialgias, the predictive values of exercise tests were equally low for the diagnosis of coronary atherosclerosis, vasospastic and microvascular CHD types. The patients with cardialgias caused by autonomic dyshormonal myocardiodystrophy showed low blood beta-endorphin and leu-enkephalin levels.

[Bicycle ergometry in the diagnosis of cardiac lesions in chronic tonsillitis].

Bicycle exercise test, load myocardial 99mTc pyrophosphate (99mTc-PYP) scintigraphy and echocardiography were performed in 42 women with chronic tonsillitis between the ages of 18 and 35 years. Of the 21 patients with chronic decompensated tonsillitis combined with ventricular extrasystole and horizontal ST segment depression during the exercise test 18 exhibited diffuse myocardial 99mTc-PYP uptake. Patients with positive scintigraphic findings had reduced index of left-atrium emptying compared with controls and patients with a negative 99mTc-PYP test finding.

[Pathogenesis and diagnosis of dysovarian cardiomyopathy].

Altogether 78 patients with dysovarian cardiomyopathy were examined, of them in 19 patients the FSH, LH, prolactin, estradiol and testosterone levels were studied. Function of the epiphysis was evaluated by melatonin excretion with urine in 46 patients. Correlation of the metabolism of sex hormones and function of the epiphysis was shown. It was assumed that the effect of disturbance of the metabolism of sex hormones on the myocardium was expressed in change of electrolyte metabolism. A new method was proposed for the diagnosis of dysovarian cardiomyopathy with the help of a pharmacological drug epithalamin whose administration returned the level of the gonadotropic hormones to normal and was accompanied by positive time course of a clinical picture of disease.

[Comparative characterisitic of changes in the adrenals against a background of immunization with viral and bacterial antigens].

The effect of alcoholic typhoid vaccine and of the cultural vaccine against the tickborne encephalitis on the adrenocortical secretion in guinea pigs was studied. Functional condition of the adrenal glands was assessed histochemically. Immunization was accompanied by increase in the activity of the adrenal cortex, the most pronounced the first 3 days. Comparative analysis showed that typhoid vaccine produced a more pronounced stree on the adrenal gland function.

[The role of the beta-adrenergic system in the function of calcium-regulating glands].

The examination of 27 patients with coronary heart disease and experiments on 42 rabbits showed that intravenous administration of beta-adrenostimulant isoprenaline enhanced the blood level of parathormone and depressed that of calcitonin whereas administration of any beta-adrenoblocker showed contrary results. The patterns of adrenergic system effects on function of the calcium-regulating glands were discussed.

[Effect of the epiphysial preparation epithalamin on the gonadotropic function of the hypophysis].

The influence of epithalamine on the blood content of immunoreactive follicle-stimulating and luteinizing hormones and prolactin was studied in 49 persons of both sexes and albino rats. It was shown that epithalamine promotes an increase in the concentration of the blood hormones if their initial level is lowered and vice versa. The normal hormone content is not changed under one epithalamine injection. The epiphysis-hypophysis relationships are discussed.

[The antianginal action of calcium antagonists: the results of instrumental examination and their clinical assessment].

In 88 patients with coronary heart disease and stable exercise-induced angina pectoris, the efficiency of calcium antagonists was examined by using myocardial Tl-199 scintigraphy and comparing its data with the parameters of angiography and their intracoronary injection. Three types of therapy-induced changes in myocardial perfusion were identified, which were as follows: 1) significant decreases in hypoperfusion area dimensions; 2) no changes in scintigraphic parameters, and 3) clinically asymptomatic increases in the number of hypoperfused sectors. There was a relationship between the therapy-induced changes and the number of sclerotic arteries though the ability of large coronary vessels to dilate was substantially decreased, as shown by angiopharmacological studies.
