[Orbital and intracranial complications of paranasal sinus inflammation].

The examination of 2251 patients with paranasal sinusitis has detected various rhinosinusogenic orbital and intracranial complications in 75 of them (3.3% of overall number of sinusitis patients). The complications were caused by acute and chronic paranasal sinusitis in 49 and 26 patients, respectively. 55 patients had orbital, 20--different intracranial complications. The treatment in most cases consists of emergency operation on the paranasal sinuses, in intracranial complications the surgeon opens dura mater of the anterior cranial fossa and prescribes massive antibacterial, dehydration, desintoxication therapy.

[Tumor necrosis factor as indicator of endogenic intoxication severity in acute neurosensory hypoacusis].

Pathogenesis of acute neurosensory hypoacusis is considered in terms of endogenic intoxication syndrome as impaired microcirculation and development of hypoxia in the periphery of the acoustic analyser are likely consequences of this syndrome. Measurement of tumor necrosis factor alpha level in the supernatant of cultivated mononuclears from patients with acute neurosensory hypoacusis indicated more active production of this mediator compared to donors. This fact justifies further investigations of this disease pathogenesis in respect to disorders in regulation of nonspecific adaptive-compensatory mechanisms.

[Problems of diagnosis and treatment of otogenic abscesses of the brain in children].

In 1947-89, 49 children with otogenic abscesses of the brain and cerebellum were treated in the ENT department of the Kuban Medical Institute. During the first two decades the number of these patients was 34 and during the second two decades it was 15, i.e., the number decreased two-fold. In 25 cases, abscesses were located in the temporal lobe, in 14 cases--in the cerebellum, and in 10 cases, multiple abscesses of different localization were diagnosed. For diagnostic purposes electroencephalography, echoencephalography, angiography, pneumography and ventriculography were used although they were sometimes inefficient, especially in the cases of multiple abscesses. Computer tomography made it possible to determine not only the number but also specific localization of abscesses. The method was applied in 8 cases. After proper examination and treatment all the patients recovered. Out of 49 patients 28 recovered and 21 died. Out of 34 patients treated during 1947-67, 17 recovered and 17 died while out of 15 patients treated during 1968-1989, 11 recovered and 4 died. It can be concluded that a significant reduction of the number of cases with brain abscesses and a substantial increase of the success rate can be attributerd to the advanced methods of their detection (echoencephalography and computer tomography) and more efficient methods of therapy of middle ear suppuration.

[The use of computed tomography of the temporal bone for the early diagnosis and prognosis of otogenic intracranial complications].

The authors propose three-projection procedure of computed tomography (CT) of the temporal bone aimed at early diagnosis and prognosis of intracranial otogenic complications. CT was conducted in 62 patients with chronic purulent epitympanitis. Destruction of the tympanic bone walls and the antrum, inflammatory involvement of the meninges were seen on CT in 15 patients. Carious changes with the wall thinning occurring at the same sites, but without intracranial invasion were noted in 25 patients. The picture was confirmed at surgery. The proposed CT technique provides not only the image of early intracranial inflammation, but also helps predict intracranial complications.
