[Varicella pneumonia in adults. Study of 13 cases].

Clinical and therapeutic features of 13 adult patients, diagnosed of Pneumonia Varicellosa are retrospectively reviewed, for the last ten years period, at Valle de Hebron General Hospital, Barcelona. Most of them had a cigarette consumption of over 20 per day, three of them showed simple chronic bronchitis criteria, and two had antibodies against HIV. Respiratory symptomatology, and dry cough, was present in 9 (75%), dyspnea in 7 (53%) and pleuritic pain in 6 (46%). Thorax radiology showed a bilateral interstitial pattern. IV aciclovir treatment was begun in patients with respiratory symptoms and hypoxemia (53%) with good therapeutic response.

Adolescent

Legionella pneumophila. A cause of severe community-acquired pneumonia.

In a prospective study of community-acquired pneumonias, 30 patients were diagnosed with Legionnaires' disease in 15 months. Clinical, laboratory and radiologic features of these patients are reviewed and compared with those who have pneumococcal pneumonia. Alcoholism, history of smoking, previous antimicrobial therapy, gastrointestinal and neurologic manifestations, elevations of serum transaminases, alkaline phosphatase and creatinine levels were more frequent in pneumonia due to Legionella pneumophila than in pneumococcal pneumonia. The presence of respiratory failure and radiologic progression were common findings that suggested L pneumophila as the etiologic agent of a community-acquired pneumonia. Development of respiratory failure was associated with involvement of several lobes and isolation of L pneumophila in any specimen. In 21 of 30 patients with Legionnaires' disease, L pneumophila was isolated from respiratory specimens. Overall mortality was 10 percent, but it increased to 27 percent in patients not treated with erythromycin initially.

Three cases of idiopathic bronchiolitis obliterans with organizing pneumonia.

Three patients with bronchiolitis obliterans organizing pneumonia are described. Chest X-ray films showed peripheral densities and pulmonary function tests a restrictive pattern. In all three cases transbronchial biopsy was not useful for diagnosis and we point out the importance of an open lung biopsy in order to make the differential diagnosis with other infiltrative lung disease with a different prognosis and therapy.

Aged

Ofloxacin in miliary tuberculosis.

We report one case of severe miliary tuberculosis with liver failure and respiratory insufficiency in a twenty-seven year old patient. We emphasize the presence of hepatic, occular and vestibular toxicities secondary to the treatment and the usefulness of ofloxacin with cycloserine given for nine months.
