Nonlocal magnetic field-tuned quantum criticality in cubic CeIn(3-x)Sn(x) (x = 0.25).

We show that antiferromagnetism in lightly (approximately 8%) Sn-doped CeIn3 terminates at a critical field mu0H(c) = 42 +/- 2 T. Electrical transport and thermodynamic measurements reveal the effective mass m* not to diverge, suggesting that cubic CeIn3 is representative of a critical spin-density wave (SDW) scenario, unlike the local quantum critical points reported in anisotropic systems such as CeCu(6-x)Au(x) and YbRh2Si(2-x)Ge(x). The existence of a maximum in m* at a lower field mu0H(x) = 30 +/- 1 T may be interpreted as a field-induced crossover from local moment to SDW behavior as the Néel temperature falls below the Fermi temperature.

Measurement of electrical skin impedance of dermal-visceral zones as a diagnostic tool for disorders of the immune system.

Among complementary medicine approaches, diagnostic screening tools based on neuroreflexology have been recently developed. Such techniques are based on the rationale that measurement of electrical impedance of specific dermal zones might reflect the occurrence of pathological states in the corresponding internal organs or systems. Our objective was to evaluate the reliability of a neuroreflexology-based diagnostic test in diagnosing immune-mediated diseases in a blinded single centre study. Seventy-eight patients with immune-mediated diseases (38 patients with autoimmune diseases (AD), and 40 allergic patients) were included in the study. Thirty age and sex matched healthy subjects were also evaluated as a control group. All the patients and subjects underwent conventional medical history and physical examination. We evaluated a device manufactured by Medex Screen Ltd (Arad, Israel). The Medex Test analysis was carried out by a second physician who was blinded to the previous diagnosis. A high correlation between the formal clinical diagnosis and the results of the measurement of electrical skin impedance was reported, with a specificity of 93.3% and a sensitivity of 81.2%. Both sensitivity and specificity dropped when analysing the autoimmune and the allergic group separately, but remained significant for the autoimmune diseases. Degree of activity of the allergic disorders, or specific treatment, did not affect the diagnostic properties of the described device. The Medex Test neurophysiology based technique has the potential to serve as a diagnostic tool for immune based pathologies. Future studies will define this tool place in routine evaluation and potential screening ability.

Expectation of impaired response to recombinant hepatitis B vaccination.

A controlled trial of hepatitis B vaccination with recombinant antigen was undertaken in 18 hemodialysed end-stage renal failure (ESRD) patients (P) and 16 members of our staff (controls, C). In order to identify expected nonresponders (NR), we monitored T lymphocyte (Tc) subsets, peripheral blood mononuclear cells (PBMC), total leukocytes (WBC) and IgG. After 3 vaccine doses of 40 and 20 IU, respectively, for P and C, at 0, 1 and 6 months, 11 P(61.1%) and all C (100%) responded, as in previous reports [1-3]. The 7 NR presented lower CD4+ Tc fraction, compared to responders (R) and C (p less than 0.05), and higher monocyte (p less than 0.00001) and WBC (p less than 0.002) counts, with lower lymphocyte fraction (p less than 0.001), compared to R. We suggest that NR could represent a selected group of ESRD patients, screenable by these easily detectable features, probably markers of a specific immune dysfunction.

[Clinical and therapeutic considerations on hepatic encephalopathy].

Impairment of consciousness in liver encephalopathy ranges from slight anomalies to clinically manifest coma. Since the triggering causes are extremely varied and often include dietary errors and/or medication, it is extremely important for the physician to be able to identify the initial signs so as the to start the appropriate treatment. After reviewing the etiopathogenesis and pathology of liver encephalopathy, the authors list the possibilities for medical management which often lead to satisfactory results.
