[Familial forms of hypertrophic cardiomyopathy].

Consanguineous relatives of a patient with hypertrophic cardiomyopathy (HCMP) were examined. Asymmetric hypertrophy of the interventricular septum with signs of obstruction of the left ventricular outflow tract was detected in the patient's brother. Hypertrophy of the left ventricular posterior wall and Wolff-Parkinson-White syndrome (WPW, type A) was detected in the patient's daughter. This case demonstrated a diversity of HCMP clinical, electrocardiographic and echocardiographic signs among the members of the same family.

[Effect of interlok, remantadine and biotop on normal monocyte functional activity and in acute viral hepatitis].

The influence of interlok, biotop and remantadine on the functional activity of monocytes was determined with the use of the NBT test permitting the quantitative evaluation of this characteristic. In this study interlok was found to give promising results in the treatment of acute viral hepatitis, due both to its antiviral action and to the stimulation of the functional activity of monocytes. biotop proved to render a sharply pronounced stimulating effect on the functional activity of monocytes, thus enhancing antiviral resistance. Remantadine suppressed the functional activity of monocytes and increased their lesion by the virus, thus creating favorable conditions for virus persistence in monocytes.

[Comparative clinico-instrumental characteristics of different morphological variants of hypertrophic cardiomyopathy].

Six morphologic variants of hypertrophic cardiomyopathy (HCMP) have been identified on the basis of the echocardiographically-documented extent and localization of left-ventricular hypertrophy in 34 patients with HCMP. More than half of the patients had no complaints. The more characteristic echocardiographic features of the disease included a considerable thickening of one or more departments within the left ventricle, the size of its cavity being diminished. In most cases, HCMP was associated with considerably enlarged R waves on the electrocardiograms from 35 precordial leads that were usually combined with ST segment depression and T wave inversion. The latter two features often provided grounds for a hyperdiagnosis of coronary heart disease (CHD). What distinguishes HCMP from CHD is the discrepancy between marked and persistent pathologic electrocardiographic changes and a relatively uneventful clinical picture of the disease. The comparison of echocardiographic and ECG charting data allows a more detailed assessment of the kind of left-ventricular hypertrophy in HCMP patients, and can be used in the differential diagnosis of HCMP vs. CHD.

[Characteristics of monocyte functional activity in acute viral hepatitis].

The functional activity of monocytes in acute viral hepatitis has been studied. As revealed by the NBT test, circulating monocytes are damaged by viruses and/or virus-containing complexes. Proceeding from this fact, the percentage of monocytes containing virus inclusions should be considered as the percentage of monocytes damaged by the virus, and the coefficient of the inclusion of viral particles should be considered as the quantitative characteristic of the viral damage of monocytes. The decrease of the percentage of functionally active monocytes creates suitable conditions for the persistence of the virus in monocytes and for the disease to become chronic.

[The cerebrocardiac syndrome].

The authors describe case histories of patients with brain involvement--extensive brain strokes with blood breakthrough to the ventricles. The development of myocardial lesions (from extensive hemorrhages to the heart muscle till the development of large myocardial infarction) attests to the possibility of centrally caused disorders of coronary circulation. It is necessary to note that the development of myocardial lesion is masked, as a rule, by the clinical picture of a grave brain stroke and is not adequately detectable at ECG studies. It is believed that the development of hemorrhages in the myocardium is consequent on the reperfusion syndrome, whereas formation of myocardial infarction is the result of the spasm of the coronary arteries.

[Comparative evaluation of the effect of nitroglycerin, sodium nitroprusside and phentolamine on hemodynamics and myocardial contraction during coronary occlusion in the dog].

Acute experiments on 27 dogs were conducted to examine the effect of intravenous nitroglycerin, sodium nitroprusside and phentolamine on the hemodynamics and regional contractility of the left ventricle under conditions of experimental coronary occlusion. Nitroglycerin was the most effective agent versus the other drugs in decreasing the elevated diastolic pressure in the left ventricle whereas sodium nitroprusside showed the most pronounced hypotensive effect in relation to systemic arterial pressure. All these drugs produced indirect favourable chrono- and inotropic effects especially marked with phentolamine. The effect of nitroglycerin and sodium nitroprusside on the cardiac output may vary to a great extent: a decrease in the stroke volume occurred in the presence of the remaining high degree of peripheral vascular resistance and a fall in the elevated and diastolic pressure in the left ventricle, an increase in the stroke volume was observed in the case of a considerable drop in the overall peripheral vascular resistance and the persisting endodiastolic overburden of the left ventricle.

[Prognosis of complex ventricular arrhythmias at different periods in myocardial infarct].

The incidence and pattern of ventricular arrhythmias in acute, subacute, late hospital and posthospital periods of myocardial infarction were investigated, using Holter monitoring, in 121 patients with large-focal and transmural myocardial infarction. A high incidence of complex ventricular arrhythmias was noted after the transfer from intensive care unit (38.8%). The relationship of complex ventricular arrhythmias to ventricular arrhythmias at previous stages was reviewed. Factors contributing to arrhythmias at different stages of myocardial infarction were analysed; the informative value of clinical signs and electric myocardial instability parameters was assessed. Signs having the greatest predictive power were identified, and risk indices for the development of arrhythmias during the subacute and late hospital stages of myocardial infarction were established using linear discriminant analysis, to enable identification of patients at risk of developing ventricular arrhythmias.

[Effect of combination of inderal with nitrites on the contractile function of the heart in experimental coronary occlusion].

In acute experiments on 29 dogs with coronary occlusion changes in cardiohaemodynamics, epicardial electrogram and the regional contractility of the myocardium were studied with the aid of echocardiography after combined administration of inderal with nitroglycerin or sodium nitroprusside. It was established that under these conditions nitrites retain their ability to decrease the load on the heart: they decrease the end diastolic pressure and the volume of the cavity of the left ventricle, enhance the productivity of the heart Nitrites given with inderal do not change the contractility of the ischemized area and also do not affect the magnitude of the rise of ST segment of the epicardium electrogram of the ischaemic zone. It is concluded that the use of nitrites in acute myocardial ischaemia combined with inderal is justified to decrease congestive insufficiency of the heart, excepting cases with marked hypotension.
