[Reconstruction of the oral cavity with myocutaneous flaps and the laterocervical hinge approach].

The AA. tell their experience about the myocutaneous flaps purposely carved for the reconstructive stage of surgical intervents forecasting big body-stuff loss, as the oral floor exeresis, of the tongue, of larynx, of mandible contemporaneously with latero-cervical dissection. The method we purpose is very simple and valid as it has never given some problem about the flourishing and vitality of the flaps. We tell about very large flaps we carve by the all thickness simple vertical and median section of the inferior lip and of inferior part of neck. These flaps are holded in right and left latero cervical side for all the length going from clavicle to mastoid. The vascularization has ever been excellent for the large surface of the hinge and of course for the large nourishing hematic bed. Consequently we purpose the myocutaneous flaps holded in latero-cervical side as useful and no-replaceable in the chirurgichal reconstruction of "Supercommando".

[Malignant retroperitoneal teratoma].

The writers point out a case of malignant teratoma of retro-peritoneal origin and lateral-cervical metastasis. Preliminary tests had shown a mass of a big orange, size of cystic appearance on the scan, clearly defined by TAC, as an oval shape, of cystic appearance, with semiliquid contents, strictly connected with the left-hand side of the thyroid gland and the big vessels of the neck and the fossa over the collar bone plane which pressed on the trachea and put it out of its original position. The needle suction cytologic test, had diagnosed a carcinoma slightly differentiated. After the difficult surgical dissection because of the proximity of the mass to the thyroid gland the trachea and the large vessels, and especially because of the sizable dislocation in mediastinum up to the level of the pleural cupola, the mass was diagnosed after surgery and histological tests as a branchial carcinoma. After a short while, the onset of an abdominal mass, followed soon after by the patient's death, allowed us to look at the case again and to conclude that it was an extra-peritoneal malignant teratoma with a lateral-cervical metastasis.

[Unknown foreign body of the cranial base].

The authors, after reviewing the literature concerning the foreign bodies of the cranio-facial district, and stating the multiform symptomatology sdown by themselves, describe the case they had the opportunity to observe and resolved through a delicate and investigative operation. The question was of a steel syringe needle, missing the connecting tract, which started from Spix' spine and reached the cranial base after 2 years' migration. As it was possible to reconstruct, it stopped there, with its point between the fibres of the medial pterygoid muscle, its intermediate tract located between the velum palatinum elevating muscle and the velum tensor, while its cranial portion, after transfixing tuba, ran into the petro-squamous slit.

[Surgical treatment of neoplasms of the vestibule of the mouth].

In this work the Authors show their own operative technique, attained in the surgical treatment of the neoplasms of the vestibule of the mouth. This operative technique also shows the possibility of reducing two surgical treatments to one. In other words it is possible to remove the primitive lesion and carry out, at the same time, the emptiness of those lymphonodulos chains the lymphtic vessels of the region are tributary to. In such a way the time elapsed between the operation and the following nuclear therapy is considerably reduced. Besides, though the exeresis implies an aesthetical injury, not considerable anyway, no functional deficiencies are shown since all the mouth functions, mastication, phonation, language, deglutition etc. are promptly recovered.
