[Selection of the method of surgical approach in plate osteosynthesis of mandibular fractures].

BACKGROUND: Plateosteosynthesis by intraoral approach of mandibular fractures gives the best cosmetical result but presents a rather difficult procedure. AIMS: Comparison of intra- versus extraoral approach in plateosteosynthesis of mandibular fractures. METHODS: Plate type, experience of the surgeon, age and sex of the patient, as well as hospital stay and rate of the septic complications were analysed retrospectively in 248 patients of the OMF Department (University Freiburg) with mandibular fractures with respect in view of the operative approach used. RESULTS: The method of osteosynthesis was found to be the strongest determinant. Concerning the surgical approach significant correlation was found between age and sex, and surgical experience. Operative approach was found to have no effect on hospitalization and septic complications. CONCLUSION: Operative approach does not have either medical nor economical consequences in the surgical management of mandibula fractures.

[The role of dalacin C in the management of odontogenic inflammations].

The penicillin-metronidazole combination is well tried antibiotic therapy for the treatment of inflammations of dental origin. However, because of the rising number of penicillin-resistant bacterium strains and penicillin allergies, other antibiotics too are being applied ever more frequently. The literature data and the authors' own experience indicate that clindamycin (Dalacin C) may be used effectively against mixed infections of the oral cavity. A detailed account of the patients and the methods will be reported in the following article.

[Experience with Otten's maxillomandibular fixation method].

The authors are reporting on a method of mandibulomaxillary fixation worked out in their institute through a modification of those defined in literature. A wire hook is fixed by a screw to the mandibula and maxilla and, by the help of it an intermaxillary fixation is formed. The procedure is simple, quickly accomplishable and cheap. Its application is recommended to be introduced for other institutes, as well.

[Malignant tumor developing in transplanted tissue in the oral cavity (case report)].

Primary and metastatic malignant tumors appears frequently in the head and neck region. The occurrence of malignancies in tissues--transplanted to this area--are extraordinary rare. Authors report about a case, where the tumor developed in a cylindroid jump flap, used to the closure of the cleft palate. They discuss theoretical aspects, as well as the course of the disease.

[Long-term studies after intra-arterial cytostatic therapy of head and neck tumors].

This study on selected tumour patients demonstrates relationship between the effectiveness of preoperative, postoperative and palliative intra-arterial cytostatic treatment of tumours of the head and neck and the long-term survival. During the period ranging between 3.5 and 9 years after preoperative treatment of 11 cases, palliative treatment of 6 cases, resulting in 100% tumour regression, and postoperative-adjuvant treatment of 23 cases (selected from 254 cases receiving intra-arterial treatment), local recurrence was observed in 10 cases. The question is raised of the necessity of radical surgery corresponding to the original tumour after preoperative treatment resulting in a completely tumour-free state.
