[Mucoepidermoid carcinoma of the epibulbar connective tissue with diffuse intraocular epithelial invasion].

PATIENT: A 55-year-old man was treated twice with local excision and crycoagulation for a recurrent limbal mass of the left eye. The original histologic diagnosis was squamous cell carcinoma. Three months after the last recurrence the globe was enucleated because of a spontaneous perforation at the corneoscleral limbus with iris prolapse. Histologic examination, including PAS and mucicarmine stains, revealed a mucoepidermoid carcinoma of the epibulbar conjunctiva with infiltration of the cornea, sclera, iris and ciliary body. CONCLUSIONS: Examination of specially stained sections (e.g. with mucicarmine) should be routinely performed for those conjuctival neoplasms that contain a squamous component. Aggressive surgical management, such as early enucleation including normal appearing tissue next to the globe, should be considered for treatment of primary mucoepidermoid carcinoma to avoid later exenteration or metastasis.

Cell density of human lens epithelium in women higher than in men.

Flat preparations of the central lens epithelium in 181 human donor eyes, 120 male, 61 female, were used to determine epithelial cell density. Donor age ranged from 13 to 91 years. A similar linear age-dependent decrease of cell density was found in both sexes. In addition, the epithelial cell density was significantly higher in females. The age-adjusted mean cell density in males was 5008.6 cells/mm2, in females 5780.6 cells/mm2. A similar sex difference had been found before in lens epithelium of human cataracts. The cause and significance of this observation are unknown.

["Biocytology" of diffuse malignant melanomas of the anterior uvea (preliminary report)].

Between 1983 and 1986, six eyes with diffuse malignant melanomas of the anterior uvea were enucleated at the Eye Hospital of the University of Erlangen-Nürnberg. The characteristic clinical findings were a monolateral open-angle glaucoma in the same eye and a diffuse spreading of malignant melanocytes on the inner walls of the anterior chamber. These cells can be differentiated biomicroscopically (40 x magnification, Zeiss slit-lamp) from melanin granula and benign melanocytes. Findings in two patients show that individual cells can be biocytologically classified in this way. Malignant cells are characterized by large, prominent nuclei and long cytoplasmic processes. Biocytological evaluation of individual melanocytic cells can be helpful in the differential diagnosis of melanocytic processes of the anterior uvea.

[Incidence of the pseudo-exfoliation syndrome in enucleated eyes].

A total of 455 enucleated globes were examined for a pseudoexfoliation syndrome. The typical alterations could be diagnosed most clearly from the ciliary processes. The diagnosis is frequently missed in first examinations of routine sections. Among 323 nonselected eyes with secondary glaucoma, which had to be enucleated following painful amaurosis, pseudoexfoliation syndrome was found in 3.4%. On the other hand, in 132 eyes with intraocular tumors it was found only once. Our examinations provide indications that pseudoexfoliation syndrome possibly causes an unusually high incidence of deleterious secondary changes which render enucleation necessary.

[Rhinoepithelial, cystic choristoma of the orbit].

Over a period of 9 months a 12-year-old girl spontaneously developed a palpable cystic tumor in the upper eye lid which led to an indentation and downward displacement of the globe. The cyst was not joined to the intact orbital wall and it proved possible to remove it from the orbit by blunt dissection. Histopathologic examination revealed an epithelium typical of the upper respiratory tract. The process was clearly distinguishable from a mucocele of the paranasal sinuses.

[Ocular involvement in ectodermal dysplasia].

The authors describe the ophthalmological findings and clinical course in two patients with EEC syndrome and one patient with anhydrotic ectodermal dysplasia. A retinal complication was seen in a 43-year-old patient which seems to be associated with the EEC syndrome and has not yet been described. A review of the literature indicates the frequency of ocular findings. So far, primary ocular changes in anhydrotic ectodermal dysplasia have never been observed. The older the patient, the more severe the secondary complications affecting the lids and cornea can be; the eye may even be lost.

[More than 30 years' successful follow-up of implantation of a Schreck anterior chamber lens].

The authors report on two patients in whom Schreck anterior chamber lenses were successfully tolerated for more than 30 years. In 1955 a lens of this type was implanted in the left eye of an 18-year-old female patient, for unilateral congenital cataract, and in 1956 a 28-year-old man was treated in the same manner one year after a perforating injury. The implants were found to be sheathed in proliferative transparent to whitish tissue from the chamber angle region; both the cornea and intraocular lens were clear and there was no sign of irritation in the anterior chambers. The long courses document the fact that methylacrylate is evidently sufficiently inert over a long period of time to be tolerated by the intraocular structures. The observations support the view that methylacrylate remains unchanged in the eye even for periods of time exceeding 30 years.

[Intraocular ossification in clinically unsuspected malignant melanoma of the uvea and in phthisis bulbi].

Phthisical eyes of 2 patients revealed clinically unsuspected, partially necrotic and partially vital malignant melanomas of the uvea and extensive intraocular ossification. Both eyes had had recurrent intraocular inflammatory episodes for years before they were enucleated because of pain. Intraocular ossification does not rule out the presence of clinically unsuspected malignant melanoma of the uvea.
