[Acute experimental enzymatic cholecystitis (clinico-morphological comparisons)].

In the experiment on 30 mongrel dogs, acute enzymatic cholecystitis (AEC) was modelled by means of intraduodenal connection with a fluoroplastic catheter of the common bile duct and pancreatic duct. The animals died at day 3-5 from transudative biliary peritonitis. A correlation between the severity of the clinical course, pronouncement of enzymologic shifts (increase in activity of amylase and phospholipase A2, content of biliary malonic dialdehyde) and morphological changes in the gallbladder and common bile duct in the form of necrosis and round-cell infiltration of the mucosa was revealed. Only decompression of the bile duct within the first 6 h from the moment of AEC development contributes to regression of clinical and enzymologic disorders and recovery of the animals.

[Emergency laparoscopy in the diagnosis of closed abdominal trauma].

Analysed were the 242 case records of the sufferers treated for closed abdominal trauma. In 80, the laparocentesis was performed with the use of the "seeking catheter", in 78--combined laparoscopy. The indications for the performance of these diagnostic manipulations are substantiated, their diagnostic informative value is assessed.

[Acute enzymatic cholecystitis: difficulties in diagnosis and surgical treatment].

In analysis of the results of clinical, laparoscopic and intraoperative investigations in 70 patients with acute obstructive, and 68--with acute enzymatic cholecystitis, it was established that for differential diagnosis of acute enzymatic cholecystitis, of most importance were the intractable pain in the right hypochondrium, early appearance of peritoneal symptoms, hyaloid edema of the gallbladder wall and hepatoduodenal ligament, exudate in the subhepatic space with a dash of bile, and detection of the phospholipase activity and high content of malonic dialdehyde in the cystic bile.

[Diagnosis and treatment of pylephlebitis of appendicular origin].

Under observation there were 11 patients with destructive appendicitis complicated by pylephlebitis. Early antibiotic treatment using antibiotics of reserve under control of antibioticogram, introduction of metronidazole++, heparin and adequate desintoxicating therapy allows to anticipate successful results. The best results in shorter terms can be achieved by the intraportal administration of antibiotics, heparin and other drugs after recanalization and catheterization of the umbilical vein.

[A new method of laparoscopic cholecystostomy].

A new technique of laparoscopic cholecystostomy is based on a puncture of a polychlorvinyl catheter with trocar and a pneumatic cuff on the catheter end into the gall bladder under laparoscopic control. The technique was used in 13 patients. This technique of laparoscopic cholecystostomy is simple and safe, gives a reliable decompression of bile ducts and has no anesthesiological and surgical risk.
