[Indications for hypoxitherapy].

Hypoxytherapy is the non-medicinal therapeutic method using gaseous hypoxic mixture (GHM) with decreased oxygen contents. The method is based on the activation of body protective mechanisms, phagocytosis stimulation, microcirculation improvement, sedative effect. GHM therapy is indicated in neurosis, CHD, hypertension, chronic pulmonary obstructive disease, to prevent from side effect of ionizing radiation, to increase the resistance during complex therapy of oncologic patients. The method is contraindicated in acute diseases, decompensation of chronic diseases. The authors noted that it is reasonable to use GHM low doses in rehabilitation period after acute pneumonia and in geriatrics. Quite satisfactory effect was obtained in therapy of lower extremity atherosclerosis. For the first time the fact of concrement passage under GHM influence was registered and hypoxytherapy was included into the complex therapy of urolithiasis.

[Toxic complications of high-dose polychemotherapy in the transplantation of bone marrow and of peripheral blood stem cells].

The authors propose their own system of assessment of high-dose polychemotherapy toxicity. The system was applied to toxic complications of high-dose polychemotherapy in 31 patients with hematological malignancies subjected to allogenic, autologous bone marrow transplantation and transplantation of stem cells from peripheral blood within the scope of different protocols of high-dose polychemotherapy in conditioning regimen. A special scale developed in the Belarus Center for Bone Marrow Transplantation basing on the above system provides prediction of survival in early post-transplantation period.

Use of specific anti-HLA antibodies in clinical nephrology.

In order to optimize the regimen of pathogenetic therapy of glomerulonephritis with nephrotic syndrome we used transfusions of plasma containing specific anti-HLA antibodies against the patient's phenotype as a part of the treatment in the acute stages of disease. The method was theoretically based on the conception of autoimmune genesis of glomerular lesion as well as on the assumption that the highest expression of HLA-antigens are on the surface of activated immunocompetent cells. In previous experiments in vitro we demonstrated immunosuppressive activity of cytotoxic serum on lymphocytes in the absence of complement. Stable remission that was associated with the recovery of serum IgG/IgM balance and normalization of cellular immunity indices was achieved in 72% of 50 treated patients after 2-4 transfusions. Immunotherapeutic properties of anti-HLA antibodies were also confirmed with preliminary results of plasma transfusions in 5 kidney graft recipients. Posttransfusion reactions of a non-haemolytic type were slight and transient. The data presented allow us to conclude that our method may be useful in clinical cases associated with undesirable immune activation induced by both auto- and alloantigens.

[The activity of superoxide dismutase and its isoenzymes in the immunocompetent cells of the peripheral blood in systemic lupus erythematosus and rheumatoid arthritis].

General activity of superoxide dismutase (SOD) and its isoenzymic spectrum in lymphocytes and neutrophils were studied in the peripheral blood from 63 SLP and 85 RA patients. A significant decline in SOD activity and qualitative shifts in SOD isoenzymic spectrum were found in both cell groups. The differences were specific for lymphocytes and neutrophils. SOD isoenzymic aberrations proved significant in minimal activity of SLP and RA and go in the same direction in both the diseases. General SOD activity and its isoenzymic composition can serve sensitive indications of immunocompetent cell function both in SLP and RA and of the need in antioxidants inclusion into combined treatment of both the diseases.

[The possibilities for the early diagnosis of ankylosing spondylarthritis].

Soluble antigen HLA-B27 was assayed in blood serum of 30 patients afflicted with ankylosing spondyloarthritis (AS) and 25 healthy subjects having HLA-B27 (B27+) antigen in the phenotype. As compared to B27+ healthy subjects, B27+ patients with AS demonstrated a significantly higher level of soluble HLA-B27 antigen. A substance whose functional activity was found to be identical to that of soluble HLA-B27 antigen in blood serum of B27- patients with AS was discovered for the first time. It was absent from B27- healthy subjects. In addition to the identification of the fixed forms of soluble HLA-B27 antigen, the assay of this antigen in blood serum enhances the potentialities of laboratory diagnosis of AS, which plays an important role at the early disease stages.

[The creation of an all-Union registry of typed donors--the first stage towards bone marrow transplantation from an unrelated donor].

The changes in tactic approaches may be recorded in clinical transplantation of the bone marrow (TBM). The countries of Europe and America realize TBM from the HLA-compatible unrelated donor. The first stage in such TBM is the establishment of the All-Union Register of typified donors which includes about 8,000 persons and is the member of the World Donor International Association. The authors describe the scientific and organizational principles of the work in the international regime; provide the ethnic and genogeographic characteristics of the All-Union Register. Give brief data on the activity of the All-Union Register during April 15 -- December 1, 1990.

[The distribution of HLA-system antigens in lung cancer patients].

Data on 152 cases of lung cancer were studied in an attempt to identify relationship between immunogenetic markers of the HLA system, on the one hand, and histologic pattern of the disease and effectiveness of radiation treatment, on the other. Control group included 200 healthy blood donors. Eight A locus antigens and 16 B locus antigens were studied. In the entire group of patients, HLA-Aw19 antigen was observed less frequently as compared to controls. Similarly infrequent were haplotypes containing the antigen. The occurrence of HLA-B16 antigen was higher in the adenocarcinoma group. Resistance to treatment, particularly, in patients with small-cell histology was associated with the presence of HLA-B8 antigen. Possibility of tissue typing of antigens for diagnosis and prediction of response in lung cancer patients is discussed.

[Effect of anti-HLA antibodies on the electrophoretic mobility of erythrocytes].

The authors studied the influence of anti-HLA antibodies on electrophoretic mobility of erythrocytes. A total of 25 samples of erythrocytes and 30 samples of anti-HLA sera of varying specificity were used. It has been established that electrophoretic mobility of erythrocytes, sensitized by anti-HLA antibodies, is lowered that permitted validation of the possibility of using the method of analytical cellular electrophoresis for identification of antigens and antibodies of the HLA system. It has been proved that practically all I class HLA antigens identified in the present work are expressed on erythrocytes .

[The informativeness of determining circulating immune complexes and immunoglobulins during immunization].

The authors suggest using measurements of the concentrations of circulating immune complexes (CIC) by the PEG precipitation method and of immunoglobulins by radial immunodiffusion technique for monitoring the patients after immunization with rhesus antigen (n = 20), with staphylococcal antitoxin (n = 59), and after renal allograft transplantation (n = 18). The results evidence that CIC level measurements by the PEG precipitation method are valuable in the monitoring of rhesus-immunized patients. The authors suggest including such measurements in the complex of tests for examination of rhesus-immunized donors.

[Oxygen activation by lymphocytes during interaction of HLA antibodies with HLA antigens].

The method of luminol-dependent chemiluminescence was used to detect oxygen activation during interaction of HLA antibodies with HLA antigens expressed on lymphocyte surface. Introduction of specific anti-HLA serum into the lymphocyte suspension leads to a rapid decrease of intensity in chemiluminescence intensity follows. The maximum yield of induced chemiluminescence in case of using a specific antiserum is significantly lower than in the control. The method could be used for detection of HLA antigens on lymphocyte surface.

[Expression of A, B, and H antigens on erythrocytes of patients with leukemia].

The level of ABO(H) antigen expression was estimated on erythrocytes isolated from the peripheral blood of 94 leukemia patients. The control group included 1014 blood donors. It was found that the level of H antigen expression was lowered in all forms of leukemia, while the levels of A and B antigen expression were unchanged. It has been suggested that the lowering of H antigen expression level is a pathogenetic sign of the leukemic process, and its value could be used for the control of the disease course character.

[The function of lymphocyte populations based on aberrations in the lactate dehydrogenase isoenzyme spectra in systemic lupus erythematosus and rheumatoid arthritis].

The data were obtained on the presence of considerable aberrations of the isoenzymic spectrum of lactate dehydrogenase in different lymphocyte populations and subpopulations. The isoenzymic shifts reflect the changes occurring in the metabolic status of the cells and are sensitive tests detecting functional deficiency of lymphocytes.

[Study of isoenzymes of lymphocyte populations during controlled immunization and in autoimmune pathology].

It has been shown that during immune system activation pronounced alterations are observed in the metabolic process course in lymphoid cells that, probably, reflect the changes in T- and B-lymphocyte functional activity, and the kinetics of intercellular relationships. The ratio of LDH1 and LDH5 fractions most differing in electrophoretic mobility has been recommended as a sign of B-system immunity activation.

[The level of beta-lysines in serum as an indicator of activity of the inflammatory process].

A follow-up of patients with various diseases has revealed a close correlation between the 2-3-fold increase of the blood serum beta-lysins vs. the norm and the development of an acute inflammatory reaction, no matter what its etiology or localization. A high diagnostic informativeness of this parameter and the simplicity of its measurement recommend the determination of the blood serum beta-lysin levels for clinical practice to detect acute inflammations.

[Mechanisms of the interaction of the leukemic cell with endogenous regulators of the proliferation of hematopoietic tissue].

A substance inhibiting DNA synthesis in mouse leukemic cells was isolated from the regenerating calf spleen. When added to a suspension of leukaemic cells, this substance is adsorbed on their surface. The following changes in cell features being noticed: 1) a minute decrease in electrophoretic cell motility, 2) a decrease in esterase activity of the cells, 3) an increase in microviscosity of membrane lipids, 4) an increase in the intracellular pH values. With a longer contact with this substance, changes in nuclear chromatin structure were noticed, with special reference to weakened bonds between DNA and proteins. The data obtained are of significance for revealing molecular mechanisms of hematopoietic mediator action on target cells.
