[Influence of surgical intervention on reactivity of organism of patients with perforative gastroduodenal ulcer].

Influence of operative intervention of various kinds on state of cellular and humoral immunity was studied in 49 patients with perforative gastroduodenal ulcer. The immunity indexes in patients constitute the prognostic test for estimation of the disease course severity and postoperative therapy efficacy. Significant lowering of postoperative complications frequency was promoted by application of preparations, stimulating the immune system (metrogyl, tactivin, gamma-globulins) in patients after the operation.

[The effect of serotonin on the healing of an experimental stomach ulcer following vagotomy].

On the model of gastric ulcer induced by stress with subsequent trunk vagotomy in rats it is shown that exogenic serotonin induces increased concentrations of serotonin in the gastric tissues, among them in the region of ulcer's defect, decreased amounts of histamine and increased serotonin-histamine indexes, and also promotes calming down of inflammatory phenomena and optimization of reparative processes in the region of ulcer.

Microcirculatory disturbances in gastric mucosa during ulcer disease and effects of serotonin on their dynamics.

Experiments on rat model of stress-induced gastric ulcers revealed marked circulatory disturbances in ulcerated gastric mucosa associated with arteriolar spasm and venous congestion and leading to perivascular edema, diapedesis of erythrocytes, and microhemorrhages. These changes were most pronounced on day 1 after ulcer modeling and progressively decreased during healing (day 14-21). On day 3, arterial hyperemia developed against the background of venous congestion, which was probably related to progressive repair. Serotonin adipinate administered after ulcer modeling accelerated the development of arterial hyperemia (day 1) and reduced venous congestion and extravascular changes. Pretreatment with serotonin was even more effective in attenuating venous congestion and extravascular changes.

[Radical nephrectomy for the kidney tumor with the thrombus invading vena cava inferior].

The results of treatment were analyzed in 92 patients with the kidney tumor in whom the thrombus invasion into vena cava inferior was revealed. Ultrasonographic scanning and magnetic resonance tomography were most informative methods in the diagnosis. The staging of the tumoral thrombus invasion was elaborated depending on which the surgical tactics was choosen. The procedure technique was depicted and the operations schemes were adduced. The vena cava thrombectomy performance is absolutely indicated in patients without metastases in lymph nodes and distant organs. The five-year survival index for this patients was 55-60%.

[Experience of serotonin application in the treatment complex of the lower extremities chronic atherosclerotic ischemia].

In 35 patients with chronic atherosclerotic ischemia of the lower extremities serotonin was applied in complex of treatment. Significant enhancement of volumetric blood flow, of the oxygen partial pressure and of rheovasographic index as well were noted. Clinically positive effect of the treatment was manifested by enhancement of the distance passed without pain, the extremity getting warmer and the pain cessation in rest.

[The administration of sandostatin in the combined treatment of acute pancreatitis and its complications].

Experience of application of the somatostatin synthetic analog sandostatin ("Novartis" firm) in treatment of 212 patients, including 84--with an acute pancreatitis (AP), 41--with postoperative AP (APP), 15--with external pancreatic fistula and 72--with high risk of the APP occurrence for prophylaxis was summarized. The significant inhibiting action of preparation on endocrine pancreatic function was noted. High efficacy of sandostatin in patients with AP and its complications was determined.

[The mechanisms for the decrease in gastric motor-evacuatory function after performing a vagotomy].

In experiment on white rats in was established that after the truncal vagotomy performance in the stress gastric ulcer the lowering of the serotonin and histamine blood content was noted as well as the disorder of ratio of their concentration in gastric wall. Insufficiency of serotonin causes the disorder of gastric motor-evacuation function after the operation. Administration in postoperative period of serotonin adipinate had promoted the raising of serotonin concentration in blood and gastric tissues, histamine level lowering, enhancement of serotonin/histamine ratio.

[Cryoprocedures in the treatment of suppurative wounds].

An analysis of results of treatment of 135 patients with purulent wounds and acute pyonecrotic injuries of soft tissues has shown the application of cryoprocedures especially when combined with the laser irradiation to promote earlier cleaning of the wound from necrotic tissues, early appearance of granulations, quicker healing the wounds and medical rehabilitation of the patients.

[Ultrastructural changes in gastroduodenal mucosal cells in patients with perforating ulcer and effect of serotonin adipinate on their postoperative rehabilitation].

Consideration is given in the paper to the state of the mucosal cell ultrastructure in the gastroduodenal zone in patients with perforating ulcer. Apparent inflammatory-degenerative type ultrastructural alterations have been found to be precent in the gastric and duodenal mucous coat in the zone of perforating ulcer. The above alterations are noted to persist by day 10 in these postsurgical patients having been administered conventional treatments. The use of serotonin adipinate promotes earlier restoration of the ultrastructural picture in the mucous coat of the gastroduodenal zone.
