[The influence of partial androgen deficiency of aging men (PADAM) on the impulse regime of incretion of several hormones and mitotic activity].

This research work is devoted to an important subject--study of the impulse regime of the incretion of a series of hormones among men with partial androgen deficience of aging men (PADAM). The results of this study suggest that PADAM leads to a breakdown of the impulse regime of incretion of a series of hormones, including luteinizing hormone (LH), follicle stimulating hormone (FSH), and somatotropic hormones (STH), as well as cortisol and insulin among men of older age groups. These changes accompany the development of metabolic syndrome (X-syndrome); their development can be inversed through androgen-replacement therapy.

[Infective endocarditis: the features of its clinical course and the prognosis].

The paper covers an investigation of 150 patients with infective endocarditis (IE), including 100 patients (aged 18 to 30 years old) with intravenous drug abuse as the main risk factor. This subgroup is characterized by an acute clinical course of IE, with tricuspid valve disorder in most cases and septic pulmonary embolism relapse in 72% of cases. Heart failure, multiple cardiac valvular disorder and focal lung destruction were found to be the main factors of unfavorable outcome. A relation between the size of vegetation on the heart valves and the mortality rate was established. At the same time, secondary immunodeficiency due to HIV-infection had no significant effect on the mortality rate in the group of drug addicts. More frequent cases of heart failure with systemic circulation embolism lead to higher hospital mortality in the group of patients with a subacute clinical course of IE. In elderly patients other concomitant pathology resulted in late IE detection and a high mortality rate.

[Immunological interactions in different forms of coronary heart disease].

The study included measurement of serum levels of IL-1beta, IL-2, IL-6, IL-8, and TNF-alpha, as well as the expression of mRNA IL-1beta, IL-2, IL-6, and TGfb1 in the vascular wall of patients with coronary atherosclerosis (angina, myocardial infarction). IL-1beta, IL-2, IL-6, IL-8, and TNF-alpha serum levels in patients with coronary atherosclerosis were found to significantly higher than those in healthy individuals (control group). Detection of IL-1beta, IL-2, IL-6, and TGFb1 in tissues revealed mRNA of the cytokines under study in radial artery wall. The main aortic cytokine was found to be IL-2 (7 samples out of 8); the main cytokines in peripheral arteries were IL-1beta and IL-6 (5 samples out of 8). The results show that elevation of IL-1beta, IL-2, and IL-8 in patients with coronary atherosclerosis demonstrates an immunoinflammatory nature of the disease; the detection of dissimilar cytokines in tissue samples reflects not only different degree of vessel involvement, but also a phase character of the process.

[Immunological mechanisms in pathogenesis of coronary atherosclerosis].

AIM: To evaluate functional parameters of immune system in patients with different variants of ischemic heart disease (IHD): stable angina pectoris and myocardial infarction; the role of immunological disorders in pathogenesis of IHD. MATERIAL AND METHODS: Immunological investigation of 136 IHD patients (102 with stable angina and 34 with myocardial infarction) included the study of the phenotype of lymphocytes: CD3, CD4, CD8, CD16, CD19, CD11/18, CD25, CD95 and HLA-DR. Measurements were made of serum concentrations of IL-1beta, IL-2, IL-6, IL-8, TNF-alpha, rIL-2 and expression of mRNA IL-1beta, IL-2, IL-6, TGFb1 in vascular wall of patients with coronary atherosclerosis (angina pectoris, myocardial infarction). RESULTS: One of the basic mechanisms taking part in development of atherosclerosis is immune-mediated inflammation of the vascular wall. Inflammatory cytokines have a significant role in this process. The serum levels of IL-1beta, IL-2, IL-6, IL-8, TNF-alpha in patients with coronary atherosclerosis were found to be significantly higher than in healthy controls. The study of IL-1beta, IL-2, IL-6, TGFb1 in tissue revealed that radial artery wall contains mRNA of the cytokines. The main cytokine of the aorta appeared to be IL-2, and the main peripheral artery cytokines were IL-1beta, IL-6. CONCLUSION: Increased serum concentrations of IL-1beta, IL-2, IL-8 in patients with coronary atherosclerosis reflect immune-inflammatory nature of the disease. Detection of dissimilar cytokines in tissue sampling reflects not only different degree of vascular involvement in the process but also phase evolution of the disease. Coronary atherosclerosis may result in adaptive immune response.

[Comparative characteristics of specific autoantibodies in rheumatoid arthritis].

AIM: To assess diagnostic informative value of specific autoantibodies (AAB)--antikeratin antibodies (AKA), antiperinuclear factor (APF) and antibodies to cyclic peptide containing citrullin (CCP) from the family of antifilaggrine autoantibodies (AFA)--in rheumatoid arthritis (RA). MATERIAL AND METHODS: A total of 121 patients with RA and 45 patients with seronegative spondylarthropathies. Rheumatoid factor was detected with latex agglutination. AKA and APF were estimated with indirect immunofluorescence the substrate of which was series frozen sections of rat esophagus in the middle third of 4 mcm and cells of the epithelium of the internal surface of healthy donor's cheek. For detection of antibodies to cyclic peptide, the test DIASTAT Anti-CCP ELISA (Axis Shield, Great Britain) was made. RESULTS: The RF was detected in 67.8% patients with RA; AKA, APF and antibodies to CCP--in 43.6, 52.9 and 68.0% patients, respectively. AKA were most specific for RA (100%) while the RF was least specific among the AAB studied (87%). Simultaneous use of RF and AFA allows AAB detection in 84% RA patients at early stages of the disease. AFA were detected in patients with high clinico-laboratory activity of the process, high indices of functional joint insufficiency. 95% of all cases of destructive arthritis were detected in patients with RF, AKA, APF and antibodies to CCP. CONCLUSION: For RA patients it is necessary to determine both RF and AFA. Detection of AFA allows early diagnosis of RA as well as to single out patients with more aggressive course of the disease and unfavourable prognosis.

[Effect of testosterone concentration on the intensity of 5alpha-dihydrotestosterone and 17beta-oestradiol formation].

The culture of human male foreskin fibroblasts was incubated with various concentrations of testosterone in the medium. It has been shown that the minimum formation of 17beta-oestradiol and 5alpha-dihydrotestosterone occurs at physiological testosterone concentrations. Any deviation of testosterone concentration, both up and down, was accompanied by an increase in the formation of 17beta-oestradiol and 5alpha-dihydrotestosterone.

[The use of nebivolol for the treatment of hypertension in patients with osteoarthrosis].

Nebivolol (5 mg/day) was given for 3 months to 20 patients with essential hypertension and osteoarthrosis treated with diclofenac and to 20 other patients with essential hypertension. After 3 months lowering of blood pressure, decreases of components of 24-hour blood pressure profile, improvements of signs of left ventricular hypertrophy occurred in both groups. There were no significant differences between changes of parameters studied between 2 groups. Thus diclofenac (nonselective cyclooxygenase inhibitor) did not attenuate antihypertensive effect of nebivolol and its action on left ventricular hypertrophy.

[Structum in combined treatment of low back pain syndrome].

AIM: To assess duration of a clinical response and tolerance of structum in patients with low back pain (LBP) and comorbid cardiovascular disease. MATERIAL AND METHODS: 25 patients with primary LBP and coronary heart disease (n = 13) and/or essential arterial hypertension (n = 18) were examined and treated for 6 months with structum. RESULTS: To the end of the first treatment months structum significantly relieved pain intensity, spinal motility, increased exercise tolerance. Excellent and good response to structum were observed in 71% patients, no response was in 29%. Tolerance of the drug was good in 23 (92%) patients. The effect persisted for 3 months. CHD characteristics did not change while arterial pressure went down noticiably. CONCLUSION: Structum is highly effective in the treatment of LBP. Its long-term intake had no effect on CHD.

[Use of meloxicam (movalis) in patients with rheumatic diseases with concomitant coronary heart disease].

The purpose of the study was to examine the clinical features of the course of coronary heart disease (CHD) in patients with rheumatoid arthritis (RA) and to evaluate the effects of the selective cyclooxygenase-2 (COG-2) inhibitor movalis (meloxicam) in two dosage forms (as injections and tablets) on the course of CHD in this group of patients. The developed questionnaire was used to make a comparative screening survey of 304 patients with RA and 152 patients with osteoarthrosis (OA), who were aged above 40 years. A comprehensive examination of the cardiovascular system was performed in 52 patients with RA concurrent with CHD and 25 patients with OA concurrent with CHD. Angina pectoris on exertion was detected in 37% of the patients with PA and in 35% of the patients with OA. Most (52%) patients with RA and CHD reported rare anginal attacks (once a month) whereas 24-day ECG monitoring revealed myocardial ischemia in 71% of the patients with RA concurrent with CHD; only silent ischemic episodes were recorded in 70% of them. The incidence of silent myocardial ischemia was higher in patients with RA concurrent with CHD than that in those with OA concurrent with CHD (p < 0.03). Examination of the incidence of anginal attacks showed that there was a tendency of the incidence of pains to decrease in patients with RA concurrent with CHD who took selective COG-2 inhibitors. Patients with RA concurrent with CHD who received movalis had atendency of arrhythmia to occur more rarely and of the duration of myocardial ischemia to reduce as compared with those who took nonselective COG-2 inhibitors.

[Features of silent myocardial ischemia in male patients with coronary disease].

The results of a study dealing with the specific features of silent myocardial ischemia (SMI) in male patients aged over 70 years are presented. Prolonged episodes of silent myocardial ischemia have been ascertained to be more frequently encountered in patients with ischemic heart disease (IHD) as high functional class angina pectoris on exertion, postinfarct cardiosclerosis, arterial hypertension, atherogenic blood lipid disturbances and they increase a total risk for SMI provided that they are combined. All this leads to destabilization of coronary blood flow and becomes a trigger for the development of prognostically significant cardiac arrhythmias by complicating the course of IHD and increasing the cost of drugs.

[Long-term effects of structum administration (according to data from multicenter trial)].

AIM: To evaluate duration of a clinical response to the drug structum in patients with osteoarthrosis (OA) of the knee and hip joints as well as structum effects on OA course and that of concomitant diseases. MATERIAL AND METHODS: The duration of a clinical response to structum after the end of the treatment and its effect on OA and concominant diseases course were studied for 12 months in 9 centers participating in the study of the drug efficacy and tolerance in patients with gon- and coxarthrosis in an open multicenter randomized controlled 6-month trial. Out of 555 patients with OA of the knee and hip joints enrolled in the first study, the examination covered 373 patients: 159 patients of the test group treated for 6 months with structum and 214 controls. By basic clinical parameters the groups were similar. Clinical examination was made after structum treatment and 12 months later and included assessment of the number of exacerbations, hospitalizations, outpatient consultations, days of temporary disability for OA, pain in the joints while walking and at rest by the visual scale, Leken's functional index, x-ray pictures of the joints, administration of nonsteroidal anti-inflammatory drugs (NSAID), exacerbations of concomitant diseases (gastrointestinal diseases, arterial hypertension, ischemic heart disease). RESULTS: An overall functional Leken's index in patients with gon- and coxarthrosis given structum 12 months after the treatment did not reach the initial values as well as pain and daily need in NSAID. Structum effect in patients with knee joint OA persisted for 4.6 months, in hip joint OA--4.1 months; in patients with stage I-II the effect lasted longer than in stage III (5.2 and 4.6 months vs 4.17 and 3.24 months, respectively. Even short-term therapy with structum reduced the number of further exacerbations, hospitalizations and visits to their doctor. 12 months after structum therapy the effect persisted in 40% patients. Frequency of exacerbations of the concomitant diseases was less than in patients on continuous NSAID. CONCLUSION: Structum is a highly effective drug against OA as it acts long, reduces frequency of exacerbations, hospitalizations, visits to the doctor, duration of disability, NSAID requirement and improves the course of some concomitant diseases.

[Innovative technologies in the complex treatment of patients with rheumatoid arthritis].

The treatment of rheumatoid arthritis (RA) remains a difficult task since the current therapy does not always result in the disease remission. The search for and development of new methods and principles for the complex therapy of RA have been underway. 144 RA patients aged 22 to 65, disease duration--72.4 +/- 7.3 months, have been investigated. A majority of them (62.3%) had stage 2 RA, and 77.4% had RA stages 2-3. The patients who were taking methotrixate for 5 years were found to respond better and longer to the traditional-therapy drugs versus drugs of other basic groups. An intensification of treatment (by methilprednizolone combined with cyclophosphamide) in a group of stage 1 RA patients) improperly responding to the therapy (resulted in a disease remission for as long as 2.5 months. An intensification of therapy in stage 2 RA (by cyclophosphamide, vincristin and prednizolone) entailed a 13.2-month remission in 23 patients. A big number of contraindications restrict the use of such intense therapy which should be replaced, in such cases, by anticytokine drugs. Remission and improvements were ensured after remicade in 4 and 2 of 7 RA patients respectively. DMARS provided a significantly lower RA intensity that lasted, however, only for 2 weeks, which necessitates further research related with higher drug's doses and with extending its effect. Stem-cells transplantations were used in RA, after which remission and improvements were observed in 80% of patients.

[Principles of treatment and military medical examination of the patients with indolent Non-Hodgkin's lymphomas].

Non-Hodgkin's lymphomas (NHL) are the heterogeneous group of lymphoproliferative malignancies with distinct morphological, immuno-histochemical and genetic criteria of diagnosis, different character of clinical course, sensitivity to different treatment methods and disease prognosis. The WHO classification (1997) encompasses all the lymphoproliferative neoplasms. The NHL can be divided into 2 prognostic groups: the indolent lymphomas and the aggressive non-Hodgkin's lymphomas. The indolent NHL types have a relatively favorable clinical prognosis, with median survival as long as 10 years, but they usually are not curable in advanced clinical stages. Early-stage (I and II) indolent NHL can be effectively treated with radiation therapy alone. In advanced stages of indolent NHL the chemotherapy is conducted.

Androgen administration in middle-aged and ageing men: effects of oral testosterone undecanoate on dihydrotestosterone, oestradiol and prostate volume.

The gradual reduction of plasma testosterone in middle-aged and older men from mid-life onwards coincides paradoxically with the time when there is progressive growth of the prostate, a highly androgen-dependent organ. The growing interest in androgen therapy for older men makes it essential to understand the effects of exogenous testosterone on the non-diseased prostate, yet few studies are available. The present study examined prostate volume, prostate-specific antigen (PSA) and lower urinary tract symptom (IPSS) score in 207 men, aged 40-83 years, presenting with clinical features of age-related androgen deficiency [sexual and/or urinary dysfunction, elevated lutenizing hormone (LH)] who were treated for 6 months with oral testosterone undecanoate (TU). Men were divided into two groups, group 1 (n=92, plasma testosterone levels > 13 nmol/L) were treated with 80 mg daily; group 2 (n=115, plasma testosterone levels < 13 nmol/L) were treated with given 120 mg daily. Before treatment and after 1, 3 and 6 months of treatment, prostate volume was measured by ultrasound and hormones [testosterone, dihydrotestosterone, oestradiol, LH, follicle-stimulating hormone (FSH)] and PSA were measured. Within 1 month of treatment, the elevated blood LH levels were markedly decreased in all men in group 1, as well as most men in group 2. Group 2 was subdivided into men whose LH levels were suppressed (n=95, group 2a) and those whose LH levels did not suppress (n=20, group 2b). Men in group 1 and 2a had marked decreases in prostate volume, PSA and lower urinary tract symptom (IPSS) scores whereas no significant changes were observed in group 2b. Groups 1 and 2a also had more striking suppression of LH, FSH, dihydrotestosterone and oestradiol whereas group 2b had no significant increases in blood testosterone concentrations. These findings suggest that exogenous testosterone in middle-aged and older men with some clinical features of age-related androgen deficiency can retard or reverse prostate growth and that elevated plasma LH may be a useful index of severity of age-related androgen deficiency.

[The course of infectious endocarditis in IV drug abusers and in subjects predisposed to heart diseases].

40 patients with infective endocarditis (IE) abusing intravenous drugs (heroin, opium surrogates) and 9 IE patients predisposed to heart diseases were examined by Duke diagnostic criteria. IE in drug abusers is characterized by acute course of the disease with affection of the intact valves of the right heart (97.5%) and septicemia provoked by high-virulent microflora (Staph. aureus in 65%). Drug abusers showed the following principal clinical syndromes of IE: thromboembolic (65%); septic with formation of acute DIC syndrome (75%), development of pyodestructive foci in the organs and polyorganic insufficiency (23.3%); acute circulatory insufficiency (37.5%); secondary nephropathy (100%). In IE abusers with predisposition to heart diseases IE ran subacutely in the presence of bacteriemia caused by low-virulent microflora (Strept. viridans in 11%) or in the absence of microbial growth in blood seeding (78%). High IE lethality in drug abusers (40%) is explained both by severe complications and concomitant diseases (viral hepatitis B and C, HIV infection, etc.).

[Results of the multicenter clinical trial of structum preparation in Russia].

AIM: To study clinical effectiveness and tolerance of structum in patients with osteoarthrosis (OA) of the knee joints (KJ) and hip joints (HJ) in a multicenter open randomised trial. MATERIAL AND METHODS: A 6-month trial of effectiveness and tolerance of structum has been performed in 9 medical centers and included outpatients (males and females) with KJ OA or HJ OA satisfying the OA diagnostic criteria of the American Rheumatology College, having x-ray stage I-III according to Kellgren-Lawrence with manifest pain, a total functional Leken index from 4 to 11, regular intake of non-steroid antiinflammatory drugs (NAID) for 30 days in the last 3 months. Consent was obtained from each patient. 192 patients received structum, 363 matched patients served control. Structum was given per os for 3 weeks in a dose of 1.5 g/day then in a dose 1.0 g/day up to 6 months. The patients continued on NAID. The patients' examination was performed in the beginning of the study, at its months 3 and 6. RESULTS: Leken index in HJ and KJ OA significantly fell after 3 months of structum treatment. Up to month 6 it fell still further (p < 0.05). After 6 months of treatment pain syndrome relieved both at rest and movement, pain at rest disappeared fully in 57% of NJ OA patients and 46% of HJ OA patients, for movement pain it was 17 and 13%, respectively. During the treatment NAID intake was less required in both groups (p < 0.05) while 55% of patients in both groups could discontinue NAID after 6 months of the treatment. Tolerance of the drug was rather good, side effects were mild. CONCLUSION: Structum (chondroitin sulphate) is an effective drug for treatment of KJ and HJ OA: it relieves pain, preserves and improves articular function, allows to reduce or discontinue NAID, is well tolerated.

[Levels of anti-inflammatory cytokines interleukin-2, interleukin-8, and soluble interleukin-2 receptor in blood of patients with various forms of ischemic heart disease].

AIM: To quantify interleukin-8 (IL-8), interleukin-2 (IL-2) and soluble receptor of IL-2 (sIL-2r) in blood serum of patients with various forms of ischemic heart disease (IHD). MATERIAL AND METHODS: Levels of IL-8, IL-2 and sIL-2r were measured with enzyme immunoassay (EIA) in the serum of 75 patients with IHD: angina of effort (group 1), progressive angina (group 2) and acute myocardial infarction (group 3). The EIAs were performed at admission and 2 weeks later. RESULTS: Baseline levels of IL-2 in group 1 and 2 patients were close (9.1 +/- 1.6 and 10.1 +/- 3.8 pg/ml) being significantly lower in group 3 (0.81 +/- 0.57 pg/ml, p < 0.01). 14 days of therapy did not change the values noticeably. IL-8 level was the highest in group 1 (94.2 +/- 27.6, 20.03 +/- 7.4, 22.47 +/- 4.8 pg/ml, respectively). sIL-2r in the three groups did not vary greatly (73.95 +/- 12.23, 89.46 +/- 18.17, 89.2 +/- 14.17 pg/ml, respectively). SIL-2r levels rose in 2 weeks in group 3 (to 147.67 +/- 18.17 pg/ml). CONCLUSION: It is confirmed that IL-2, IL-8 and sIL-2r take part in pathogenesis of IHD. IL-2 and IL-8 levels are persistently high in anginal patients while in patients with acute myocardial infarction they are low. Low concentrations of IL-2 in the latter may be attributed to high levels of its soluble receptor.
