[The use of echo- and scintigraphic study of the biliary system in chronic cholecystitis].

Data are reported of a study of the functional state of the biliary in 50 patients with chronic cholecystitis by means of complex employment of dynamic echo- and cholecystography and dynamic computerized gamma-hepatobilioscintigraphy. It was found that dynamic echo- and cholecystography proved highly efficient indicating that ultrasound tomography is of importance not only the diagnosis of structural but also functional disorders in cholecystitis, for the choice of optimal tactics and evaluation of the efficacy of therapeutic measures excluding radiation treatment.

[Determination of the sensitivity to glucocorticoids in vitro].

A modified method for the determination of glucocorticoid receptors in human lymphocytes is suggested. The principal distinction of the method is standardization by the lymphocyte count in a sample (1 mln) and the labeled hormone concentration. The modification saves time and money, limits the range of the data variations, and makes use of a lesser volume of blood. Examinations of 70 children aged 4 to 15 suffering from the nephrotic form of glomerulonephritis have made it possible to distinguish two groups of patients: with relatively high values of specific binding X = 6820.1 +/- 530.0 (n = 30, p = 0.95, t = 2.04), this corresponding to a clinical form of hormone-sensitive glomerulonephritis, and with relatively low values of specific binding X = 1815.2 +/- 302.8 (n = 40, p = 0.95, t = 1.96), that corresponds to hormone-resistant glomerulonephritis. Dynamic studies have not shown any statistically significant changes in the specific binding values. These results permit regarding the specific binding value as a prognostic criterion in the assessment of corticosteroid therapy; this allows a wide employment of the described method in practical nephrology.

[A gamma scintigraphic assessment of blood supply of the hip joint in Perthes disease].

In order to specify the peculiarities of the blood supply of the hip joint in Perthes' disease the authors carried out gamma scintigraphy by 99m Tc-pyrophosphate in 34 children. Patients with roentgenologically confirmed diagnosis of Perthes' disease at various stages as well as those in whom Perthes' disease was suspected, but convincing roentgenologic changes were absent, were examined. The data of scintigraphy and roentgenography were compared. The following phases of disturbances in blood supply were distinguished: ischemia, instable blood supply and stable revascularization. It is supposed that recurrences of ischemia may take place at the stage of resorption. Radionuclide investigation allows to make early diagnosis even before the appearance of roentgenologic changes with differentiation of doubtful cases.

[Diagnostic possibilities of static gamma-scintigraphy using 99mTc pyrophosphate and Tc 99m phosphate in children with glomerulo nephritis].

Children with nephrotic glomerulonephritis were examined for incorporation of 99mTc-pyrophosphate and 99mTc-phosphone. The relationship was discovered between the accumulation of the radiopharmaceutical drug and the sensitivity to glucocorticoids used in the pathogenetic treatment. Accumulation of the drug in the course of the treatment documents the presence of complete, partial remission or the disease activity. It is recommended that static gammascintigraphy should be carried out for early assessment of the efficacy of the treatment and its correction.

[Clinico-radiological assessment of the effectiveness of pathogenetic therapy in glomerulonephritis in children].

Liver scintigraphy and hepatobiliary scintigraphy were employed for the assessment of kidney and liver function in children with glomerulonephritis at the stages of pathogenetic therapy with chlorambucil and prednisolone. Noticeable disturbance of nephron and hepatocytic function at the climax of clinical manifestations of the nephrotic syndrome was established. The use of maximum and maintenance doses of prednisolone and chlorambucil resulted in significant improvement of clinical and radionuclide indices. However the effective renal plasma flow and the rate of passage of the radionuclide from the plasma into the proximal canalicular cell did not return to normal even in the catamnesis 1 year after the discontinuation of treatment in 35% of the patients with chronic glomerulonephritis. The use of maximum doses of cytostatics in 98.5% of the children produced no damaging effect on the hepatocyte, in 1.5% of the children the authors observed the development of drug hepatitis. Hypokinetic dyskinesia of the gall bladder was detected in the overwhelming majority of the patients therefore drugs improving bile secretion should be recommended as part of treatment.

[Radionuclide diagnosis of acute pyelonephritis].

Nephroscintigraphy using a 67Ga-citrate complex and 99mTc-pyrophosphate was performed in 88 patients with acute pyelonephritis. Nuclide hyperfixation was revealed in 97.8% of the cases. Three groups of patients were singled out on the basis of the intensity of incorporation and nature of the distribution of the radiopharmaceuticals (RP) in the kidneys. In the 1st group the RP incorporation was insignificant but higher than normal values; the RP distribution in the affected kidney was diffuse-inhomogenous. These changes were considered to be typical of acute serous pyelonephritis. In the 2nd and 3rd groups a sharp rise of the RP accumulation was noted, being typical of acute purulent pyelonephritis. One could distinguish between diffuse and focal lesions by the picture of the RP distribution in the renal parenchyma. Diuresis stimulation made it possible to differentiate an actual nuclide fixation during inflammation from nuclide mechanical retention as a result of urine outflow disorder. According to the authors, both radiopharmaceuticals could be applied for the diagnosis of acute pyelonephritis as well as for differential diagnosis of various forms of the disease.

[Radionuclide evaluation of the function of the operated kidneys].

Altogether 68 patients were examined in early and late periods after operations on the kidneys. Function of the operated kidney was checked up by the results of two-indicator renal scintigraphy with 99mTc-DTPA and 131I-hippuran. When the kidney preserved its secretory capability and the suppression of organ function was caused by inhibited urine passage via the upper urinary tract, therapy aimed at the restoration of the kidney integrity and the elimination of the urine discharge obstruction, resulted in a rapid and complete recovery of all renal functions. Operative treatment of prolonged urological diseases complicated by chronic pyelonephritis, helped to accelerate secretory-excretory processes and slightly improved function of unchanged nephrons. A progressive decrease in the actively functioning parenchyma was recorded on scintigrams in 28% of the cases of the formation of the arteriosclerotic kidney. The authors found radionuclide investigations appropriate in the postoperative period not only in the development of complications and recurrences of urological diseases. Two-indicator renal scintigraphy with glomerulotropic and tubulotropic radiopharmaceuticals made it possible to give an objective assessment of the efficacy of surgical intervention.
