Assisted circulation in cardiac and respiratory insufficiency.

Intra-aortic counterpulsation with a balloon pump (IABP) was used in 63 patients with cardiogenic shock, heart failure after surgery with artificial circulation, internal lacerations of the myocardium, or unstable angina. It was found that the IABP was most efficient in conditions related to myocardial ischemia. Use of arteriovenous perfusion (in 11 patients) or artificial ventricles is indicated in severe perturbations of the pumping function of the heart. As experience has shown, cardiologic and heart surgery patients usually have multiorgan insufficiency. For this reason, it is necessary to use other artificial organs in complex treatment: artificial lungs, kidneys, livers, etc.

[Heart function in patients with mitral defect immediately after the insertion of a mitral valve prosthesis].

Changes in left and right ventricular function were examined in 42 patients before and early after the application of mitral valve prosthesis (MVP). Hemodynamic suppression observed early after MVP application was found to be due to reduced preload, and critical inhibition of contractility in 1/3 of the patients. Baseline left-ventricular contractility and pumping function parameters were decreased to a greater degree in patients constituting Group 2. It is suggested that early postoperative circulatory disorders are rooted in preoperative myocardial lesions, inadequate heart muscle protection during extracorporeal circulation with general hypothermy, and violation of conditions of left-ventricular loading immediately upon the recovery of the mitral valve blocking function.

[Surgical treatment of different variants of tachycardia].

The paper presents a generalized experience in the surgical treatment of patients with frequent attacks of paroxysmal tachycardia associated with additional cardiac conduction pathways, that are not affected by the drug treatment. The results are described of 27 operations of destruction of anomalous cardiac conduction pathways. The use of a special ultrasound destructor permits the surgeon to perform the operation on the functioning heart, thus providing maximal radical surgery with minimal cardiac trauma and best effects.

[Experimental and clinical use of artificial heart ventricles].

The paper sums up experience with the development of a domestic system for auxiliary circulation and a module-type artificial ventricle, and presents the results of their experimental trial and first-ever clinical use in patients with severe heart failure following open-heart surgery. Good operational characteristics and high efficiency of this equipment in clinical as well as experimental terms are demonstrated.

[Intraoperative control of the state of the arrested heart].

Monitoring the function of the operated on heart over time based on the principle of recording the effort of the myocardium at rest was tried under the clinical conditions after preliminary experiments on the hearts of rats and dogs. The study included 11 patients operated on for aortal, mitral and multi-valvular prosthetics under pharmacological cardioplegia. Such monitoring enables an objective appraisal of the heart status, Besides, it makes it possible to decide on whether repeated administration of cardioplegic solutions is required, to specify the times of reoxygenation and to prognose the pattern of the heart function recovery after operation is over. The data obtained helped detect some disadvantages of the generally accepted method of pharmacological cardioplegia.

[Topical diagnosis of disorders of the heart conduction system and surgical treatment of patients with supraventricular tachycardia].

Questions of preoperative diagnosis of the place of the disturbance of the normal heart excitation, spread of this excitation and distribution of places of the most frequent disposition of these foci in the heart depending on the pathology of the process are discussed. Sixty six patients with different kinds of tachicardia were examined, 40 patients were operated upon under conditions of artificial blood circulation. The examination of patients was performed by both routine and special methods (such as electrophysiological examination with the registration of 9 points of different parts of the heart simultaneously).

In vitro evaluation of U.S. and U.S.S.R. artificial hearts.

This report summarizes the results of the in vitro evaluation of Soviet and American artificial hearts. The devices were tested at the All-Union Institute of Transplantation and Artificial Organs, Moscow, U.S.S.R., and Baylor College of Medicine, Houston, Texas, U.S.A. These studies were designed to standardize procedures to allow comparison of artificial ventricles of different designs. Also, these studies might provide a means for evaluation of other characteristics such as hemolysis, durability, and reliability. Static and dynamic tests were performed, varying preload, afterload, rate, and systolic and diastolic time intervals. All designs demonstrated comparable function curves with capability of taking over the pressure and volume work of the natural heart.

[Hemosorption as a treatment method in digitalis poisoning of heart failure patients with myocardial infarct].

Results of haemosorption in treating digitalis intoxication are analyzed. In 2 patients haemosorption was used for 90 min at the volume speed of 100 ml/min. Digoxin clearance at the start of haemosorption was 98.4 and 83.8 ml/min, at the end 85.7 and 70.3 ml/min. Digoxin concentration decreased by 44.8 and 13.0%. Hemosorption enabled performing electric defibrillation with restoration of the sinus rhythm.

Effects of various countermeasures against the adverse effects of weightlessness on central circulation in the healthy man.

The effect of lower body negative pressure, local negative pressure below the knee joint, and both pneumatic and mechanical occlusion cuffs at the upper third of the femur on central circulation, oxidative metabolism, and acid-base equilibrium of blood was studied on eight healthy male volunteers with doublelumen Swan-Ganz flow-directed thermodilution catheters implanted for 7 d into the pulmonary artery. The counter-measures against the adverse effects of weightlessness were used in two regimens: regimen I at -30, -50, +40 torr and regimen II at -60, -100, and +60 torr. The parameters were recorded in recumbency, during the head up tilt at 70 degrees for 15 min, and then during the head-down tilt at -20 degrees for 60 min. After the first 20 min of anti-orthostasis, the effects of the above countermeasures in the two regimens were investigated. The tile-induced changes in central circulation, those observed during application of the countermeasures, and the effect of different countermeasures were studied on a comparative basis. It was demonstrated that the highly informative method of implantation of catheters into the pulmonary artery can be successfully used in biomedical investigations of normal men.
