[Cardiovascular adjustments in hypertensive patients. An echocardiographic study].

Echocardiography was used to asses parameters of cardiovascular function in order to identify the main cardiac adjustment mechanisms to arterial hypertension. In addition to morphological parameters of septal and parietal thickness and diameter, telesystolic (Ses) and telediastolic (Spk) stress, EF, Vcfm, the index of left ventricular mass (ILVM), hypertrophy ratio (h/r), contractility index (Do), peripheral resistances (RPT) and Tarazi's index (SAC) were evaluated. In comparison to control subjects, higher levels of PwTs, h/r, SAC, RPT and ILVM (at the limit of significance) were found in hypertensive patients, which were reflected by higher Spk and Do values. From the further analysis of data to identify patients with signs of left ventricular hypertrophy (h/r greater than 0.40 and/or ILVM greater than 140) it was concluded that the hypertrophy ratio (h/r) is the functional parameter which most closely reveals the type of myocardial adjustment. Irrespective of absolute values of ILVM, the adequacy of myocardial hypertrophy to the dimensions of the cavity is accompanied by the normalisation of stress and the return of Do values close to normal levels. Hearts with low h/r those with the highest stress levels, as well as preesting the lowest EF and the highest Do values.

Positive patch test reactions to nickel, cobalt, and potassium dichromate in a series of 576 patients.

In 1988, 633 patients attending our Allergology Center underwent patch testing with the standard series recommended by the Gruppo Italiano Ricerca Dermatiti da Contatto e Ambientali. Of these, 576 patients completed the test correctly and their results were evaluated statistically. Nickel, cobalt, and potassium dichromate were the three most common sensitizers; concomitant positive reactions were present at significant levels. The authors discuss environmental and occupational exposure to contact allergens and correlate the major sites of dermatitis with the sensitivity to each metal in patients of both sexes.

[Immunohistochemistry and in situ hybridization techniques in the study of 75 patients with HPV-induced lesions of the genitalia].

Seventy-five patients with anogenital warts were biopsied in the Center for Sexually Transmitted Disease of Galliera Hospital (Genoa). Bioptic specimens were processed for histologic examination, immunohistochemistry using polyclonal antibodies against HPV capsid common antigens and in situ hybridization using HPV 6/11, 16/18, 31/35/51 DNA probes. The aim of the study was to detect patients positive for HPV 16/18, 31/35/51 DNA probes. These HPV types have a higher oncogenic potential and patients with lesions harbouring them are considered to be at risk for progression to malignancy. These patients should be followed up carefully and periodically for early detection of new lesions and neoplastic transformation.

[HPV-induced subclinical lesions of the male genitalia. Use of a colposcope].

Thirty-nine HIV-negative men, without apparent genital warts, were evaluated for evidence of subclinical genital condylomata by visual examination of the genitalia with colposcopy after the application of 5% acetic acid. 24 patients (group I) had a history of recurrent genital condylomata; 8 (group II) were sexual partners of women with HPV-related lesions and 7 (group III) presented another sexually transmitted disease. Under colposcopic magnification, acetowhite areas were biopsied for conventional histology and in situ hybridization using 6/11, 16/18 and 31/35/51 HPV-DNA probes. Colposcopy and histologic features were positive for condylomata acuminata in 16 patients of group III (14%). HPV 16/18 - whose oncogenic potentiality is well known - was detected in 3 patients: of these, one patient of group I presented a histologic picture suggestive of bowenoid papulosis.

Adolescent

[Hyperkeratosis lenticularis perstans. A clinical case and review of the literature].

A case of a woman of 55-years-old with verrucous lesions localized on the dorsa of her feet and hands is reported. The light microecopy reveals marked hyperkeratosis with some degree of parakeratosis, epidermal atrophy and light lympho-histiocytic infiltrate in the papillary dermis. A diagnosis of hyperkeratosis lenticularis perstans is made on the basis of the physical examination and histologic findings. A brief review of the literature on the genetic, ultrastructural characteristics and pathogenesis of the disease is made.

Female

Seborrheic dermatitis in otherwise healthy patients and in patients with lymphadenopathy syndrome/AIDS-related complex: treatment with 1% bifonazole cream.

Recent studies reported in the literature have repeatedly pointed out the utility of imidazole derivatives, one of which is bifonazole, in the therapy of seborrheic dermatitis (SD). The cause of this pathological situation is still under discussion but the therapeutic success obtained with antimycotics speaks in favor of the pathogenetic importance of Malassezia ovale in this context. There is a high frequency of SD type complaints in patients with HIV infections in whom immune defenses against various pathogens, including yeasts, are deficient. We therefore verified the clinical efficacy of 1% bifonazole cream on 15 subjects with SD of the face, 9 of whom were affected with lymphadenopathy syndrome/AIDS-related complex (LAS/ARC). The treatment was administered over a four-week period and gave good results in 12 patients. The best results, however, were observed in those suffering from LAS/ARC.
