Action of (2-->1)Fructo-oligopolysaccharide fraction of Chlorophytum borivilianum against Streptozotocin-Induced oxidative stress.

A fructo-oligosaccharide was isolated from Chlorophytum borivilianum and identified as O-beta-D-fructofuranosyl-(2-->1)-(beta-D-fructofuranosyl) (n)-(2-->1)-alpha-D-glucopyranoside (n = 5 - 30) using high-pressure anion exchange chromatography, MALDI-MS, NMR, GC, HPTLC and chemical analysis. The extract and the fructo-oligosaccharide were found to have significant antidiabetic activity with the blood sugar levels being 118.32 +/- 3.56 and 110.21 +/- 4.22, respectively, as compared to the control value of 231.25 +/- 3.03 along with moderate antioxidant activity in streptozotocin-induced diabetic animals.

Hypolipidemic activity of seeds of Cassia tora Linn.

Ethanolic extract of seeds of Cassia tora L. and its fractions were investigated for hypolipidemic activity on triton induced hyperlipidemic profile. Ethanolic extract and its ether soluble and water soluble fraction decreased serum level of total cholesterol by 42.07, 40.77 and 71.25%, respectively. On the other hand ethanolic extract, ether soluble fraction and water soluble fraction increased the serum HDL-cholesterol level by 6.72, 17.20 and 19.18%, respectively. Ethanolic extract, ether fraction and water fraction decreased triglyceride level by 26.84, 35.74 and 38.46%, respectively. The reduction in LDL-cholesterol level by ethanolic extract, ether soluble fraction and water soluble fraction were 69.25, 72.06 and 76.12%, respectively.

Effect of Calotropis procera latex on isoproterenol induced myocardial infarction in albino rats.

The alcoholic extract of the latex obtained from Calotropis procera (Asclepidaceae) was evaluated for protection against isoproterenol (20 mg/100 g body wt., s.c.)-induced myocardial infarction in albino rats. The heart damage induced by isoproterenol was indicated by elevated levels of the marker enzymes such as Creatine Kinase-isoenzyme (CK-MB), Lactate dehydrogenase (LDH), Serum Glutamate Oxaloacetic Transaminase (SGOT) and Serum Glutamate Pyruvate Transaminase (SGPT) in serum with increased lipid peroxide and reduced glutathione content in heart homogenates. Microscopical examination (histopathology) was also performed on the myocardial tissue. Pretreatment with an ethanolic latex extract of Calotropis procera at a dose of 300 mg/kg body wt., administered orally thrice a day for 30 days, reduced significantly (p < 0.01) the elevated marker enzyme levels in serum and heart homogenates in isoproterenol-induced myocardial infarction. Histopathological observation revealed a marked protection by the extract in myocardial necrotic damage.

Hepatoprotective activity of leaves of Kalanchoe pinnata Pers.

Kalanchoe pinnata Pers. is naturalized throughout the hot and moist parts of India. Juice of the fresh leaves is used very effectively for the treatment of jaundice in folk medicines of Bundelkhand region of India. The juice of the leaves and the ethanolic extract of the marc left after expressing the juice were studied in rats against CCl(4)-induced hepatotoxicity. The test material was found effective as hepatoprotective as evidenced by in vitro, in vivo and histopathological studies. The juice was found more effective than ethanolic extract.

Helicobacter pylori infection and recurrent abdominal pain in children.

The association of Helicobacter pylori infection with recurrent abdominal pain (RAP) was studied in 65 children aged between 3 and 12 years. Cases with a specific cause for abdominal pain were excluded from the study. All the patients were subjected to upper gastrointestinal endoscopy and antral biopsy. The biopsy specimens were subjected to rapid urease test, histopathological examination, and culture to document the presence of H. pylori in the gastric mucosa. All the patients were put on H. pylori eradication drugs for 10 days. About 69 per cent of the children had histopathological evidence of antral inflammation. Helicobacter pylori detection rate was 77 per cent. About 83 per cent of patients had complete symptomatic relief when followed over a period of 6 months. The study concludes a possible association of H. pylori with recurrent abdominal pain in children and all these patients should be considered for H. pylori eradication.

Internet--implications for the future of phytopharmacological research.

Modern information technologies and world wide communications through the Internet play a significant role in medicinal plant research across the globe. The phenomenal growth in Internet usage is largely due to the success of World Wide Web. Various useful websites and databases on phytopharmacology are already in the "Net" and many more are being added constantly. The future of phytopharmacological research is handling the existing information in proper way. In this review of the Internet, compilation of important websites is expected to stimulate, instruct and update academicians and researchers involved in phytopharmacological research.

Clinical and biochemical profiles of young diabetics in North-Eastern India.

OBJECTIVE: We compared the clinical and biochemical profiles of young diabetics in North Eastern India. METHODS: Seventy diabetics who were detected at less than 30 years of age were studied. Patients with ketoacidosis or ketonuria on insulin withdrawal were grouped as insulin dependent diabetes mellitus (IDDM), patients with history of chronic abdominal pain with or without exocrine pancreatic dysfunction who either on ultrasonography (USG) or endoscopic retrograde cholangiopancreaticoduodenography (ERCP) revealed pancreatic calcification and/or ductal dilatation were grouped as fibrocalculous pancreatic diabetes (FCPD), those having BMI < 19 kg/m2 with history or stigmata of childhood malnutrition and who were ketosis resistant were taken to be protein deficient diabetes mellitus (PDDM)/malnutrition modulated diabetes mellitus (MMDM) and those who neither had ketonuria nor history of chronic abdominal pain, malabsorption or stigmata of malnutrition were grouped as NIDDM of young (NIDDY). RESULTS: Amongst the young diabetics studied FCPD constituted 32.9%, IDDM 28.6%, MMDM 21.4% and NIDDY 17.11%. USG abnormalities were observed in 21 of the 23 patients of FCPD. Seven out of these showed pancreatic head calcification on X-ray while 14 showed pancreatic duct dilatation and calcification or calculi on USG. In the two remaining patients, ERCP revealed tortuousity of main pancreatic duct and calcification which were not detected on USG. Majority of FCPD and MMDM patients revealed insulin resistance on insulin tolerance test (ITT). HDL was significantly lower in NIDDY, while VLDL and triglycerides were significantly higher in FCPD and MMDM as compared to controls. Microvascular complications of diabetes were seen in all these groups, with peripheral neuropathy being more common in FCPD (43.5%) and background diabetic retinopathy in NIDDY (41%). CONCLUSIONS: We conclude that FCPD and MMDM together form majority (54.29%) of young diabetics at our center and a sizeable proportion of them may have microvascular complications, even at the time of diagnosis.

Choledochal cyst: a changing pattern of presentation.

BACKGROUND: The aim of the present paper was to study the spectrum of choledochal cysts and analyse the results of various surgical procedures. METHODS: A prospective study was undertaken, at University Hospital, Varanasi, India, of 10 patients with choledochal cyst who presented between January 1996 and June 2000. The patients had a median age at presentation of 16.0 years (interquartile range (IQR): 13.75 years). All patients underwent ultrasonography and endoscopic retrograde cholangiopancreaticography for confirmation of diagnosis. Cyst excision was performed in eight patients followed by reconstruction by hepaticoduodenostomy and Roux-en-Y hepaticojejunostomy in four patients each. Two patients underwent cystoduodenostomy. RESULTS: One patient who underwent hepaticoduodenostomy had a minor leak that responded to conservative management. All patients were asymptomatic at a median follow up of 36.5 months (IQR: 31 months). CONCLUSIONS: There is a changing trend in the commonest mode and age of presentation; fewer patients are presenting with the classical triad and the mean age of presentation is higher. Reconstruction by hepaticoduodenostomy is equally effective, more physiological and less time consuming as compared to hepaticojejunostomy, if the anastomosis can be achieved without tension.

Endoscopic and histopathological evaluation of preschool children with chronic diarrhoea.

Fifty-seven children with chronic diarrhoea, aged 1-5 years, were studied. Protein-energy malnutrition was present in 49 (85.9 per cent) children. Anaemia (89.5 per cent), presence of mucus and blood in stool (66.6 per cent), abdominal distension (52.6 per cent), and abdominal pain (28.1 per cent) were the common clinical findings at admission. The enteropathogens isolated from the stool of 68.4 per cent of patients were Escherichia coli (19.3 per cent), Candida albicans (12.3 per cent), E. histolytica (8.8 per cent), and Giardia lamblia (7 per cent). Secondary lactose intolerance was present in 21 per cent of children, endoscopic appearance was abnormal in 23.3 per cent of children and the commonest finding was chronic duodenitis (16.7 per cent). Abnormal histopathology was observed in 73.3 per cent of cases and villous atrophy with mononuclear cell infiltration (56.7 per cent) was the most common abnormality detected. The mean duration of diarrhoea had no impact on endoscopic appearance but it significantly affected the histopathological changes. However, no correlation was found in endoscopic and histopathological lesions in relation to malnutrition and aetiological agents.

Hepatic duct stones masquerading as Klatskin tumour.

We report woman who presented with painless progressive jaundice following cholecystectomy. Radiological investigations indicated proximal bile duct obstruction suggestive of hilar malignancy. Exploratory laparatomy revealed bilateral hepatic duct stone causing obstruction.

Xanthogranulomatous cholecystitis.

Xanthogranulomatous cholecystitis exists in a small but significant proportion of routine cholecystectomy specimens. A few recent reports have shown a possible association of this disease with carcinoma of the gallbladder. All cholecystectomized specimens were prospectively evaluated over a period of two and half years in a single surgical unit to examine the incidence of xanthogranulomatous cholecystitis and its association, if any, with carcinoma of the gallbladder in an area that is prone to gallbladder diseases. A total of 460 cholecystectomies were performed for various gallbladder diseases. Histological confirmation revealed chronic cholecystitis in 311 (67.6%) cases, carcinoma of the gallbladder in 62 (13.5%), acute cholecystitis in 29 (6.3%), xanthogranulomatous cholecystitis in 41 (8.9%), and xanthogranuloma and carcinoma of the gallbladder in one case (0.2%) only. Almost all cases were suspected to have chronic cholecystitis on clinical and ultrasonographic features. Two specimens on gross examination showed mass lesions, and hence were suspected to be carcinoma of the gallbladder. Subsequent frozen section and histopathology demonstrated xanthogranulomatous cholecystitis. Only one case of xanthogranuloma was found to be associated with carcinoma of the gallbladder but no firm association could be established between xanthogranulomatous cholecystitis and carcinoma of the gallbladder.

Metallothionein expression in carcinoma of the gallbladder.

AIMS: Metallothioneins (MT) are sulphur-rich, low molecular-weight, intracellular metal-binding proteins with a possible role in carcinogenesis of some human tumours. Metallothionein expression in gallbladder cancer has not been studied previously. METHODS AND RESULTS: Immunohistochemical expression of metallothionein was studied in 42 gallbladders (27 cases of carcinoma of the gallbladder, eight chronic cholecystitis and seven cases of normal gallbladder). Metallothionein expression was significantly higher in cases with carcinoma of the gallbladder (70.37%) as compared to chronic cholecystitis (25%) and normal gallbladders (0%). There was a trend suggestive of increasing MT expression with increasing histological dedifferentiation of carcinoma of the gallbladder. CONCLUSIONS: The increased expression of MT in cases of carcinoma of the gallbladder may represent an increased exposure to heavy metals, which are known carcinogens, and may have a role in gallbladder carcinogenesis. Metallothionein over-expression in carcinoma of the gallbladder may be relevant to the poor prognosis and chemoresistance seen in these cases.

Effect of various lipid-bile salt mixed micelles on the intestinal absorption of amphotericin-B in rat.

Amphotericin-B (AmB), a lipophilic polyene antibiotic, is the drug of choice in the treatment of many serious mycotic diseases. The solubility and gastrointestinal membrane permeability (Papp) of AmB in mixed micellar systems were examined. Membrane permeability was determined using a rat gut perfusion method. The mixed micellar systems studied contained the bile salt in association with fatty acid. All mixed micellar systems enhanced the absorption of AmB relative to the simple micelle. These results have shown that mixed micelles can enhance the absorption of AmB to a greater extent relative to nonmicellar and simple micellar systems. Maximum enhancement (> 20-fold) in the rate of AmB absorption was obtained with the sodium desoxycholate/soya lecithin 40:40 mM system. These results offer a possible explanation for the reported enhancement in gastrointestinal absorption of AmB when coadministered with lipid-bile salt mixed micelles, and these systems can be used as a vehicle for designing novel drug delivery systems for poorly absorbable drug(s).
