[Intratracheal lymphotropic ozone therapy in erosive-ulcerous tracheitis].

Under observation there were 452 patients with chronic stenosis of hollow organs of the neck having cannulas during the period from 3 months to several years. During the tracheobronchoscopic examination it was found that 35 patients had trachea ulcers, 46 patients had erosive tracheobronchitis. The ulcers were localized on the anterior wall of the thoracic part of the trachea. Their diameter was from 1 to 2.5 cm. The ulcers were accompanied by diffuse bilateral bronchitis of the II-III degree of the inflammation intensity. 2-3 ml of ozonated sodium chloride solution with the concentration of ozone in it 5 mg/l were introduced into the ulcer edges, i.e. lymphotropically, into the submucous membrane. The same solution (40-60 ml) was used for daily sanitation of the tracheobronchial tree. Complete epithelization of the ulcers and cleansing of the bronchial tree took 3-4 curative bronchoscopies.

[Serotonin level in endocrine cells in bronchial mucosa and stomach in chronic obstructive lung diseases].

85 patients aged 26-60 years with pulmonary chronic obstructive diseases (PCOD) and gastric and duodenal erosive-ulcerous lesions were investigated. In PCOD the increased bioamines level in endocrine cells of bronchial ciliated epithelium and gastric glandular epithelium was detected before the treatment. The complex treatment in the form of bronchoscopy with lymphotropic administration of antibiotic and immunomodulator against the background of intragastral ozonotherapy contributes to the decrease in tissue edema, restoration of their trophicity and inflammatory process resolution. Against the background of therapy the serotonin content in endocrine cells of bronchial ciliated epithelium and gastric mucosa decreased to the norm.

[Prophylaxis of purulent complications in operations on large joints by endolymphatic infusion of drugs].

2 groups of patients operated on knee joints for traumatic injuries were examined. In the study group endolymphatic drug therapy was carried out in postoperative period, in control group of patients conventional way of intramuscular administration of preparations was used. During the treatment, plasmatic cells, macrophages and tissue basophiles in synovial liquor were studied as well as synthesis of nucleic acids (DNA and RNA) in plasmatic cells. It was shown that immune homeostasis of synovial liquor in postoperative period normalized faster in endolymphatic administration. The studies of tissue basophiles have shown that inflammatory process in the knee joint is completed faster when endolymphatic route of drug administration was used. Clinical results evidence of higher effectiveness of endolymphatic therapy in comparison with conventional method of treatment in postoperative period. In the former case the inflammatory process was completed by the 5th day, in the latter--by the 7th-9th day. The endolymphatic therapy in all the patients resulted in healing of operation wounds by primary intention. In the group of patients treated by standard methods, suppuration of the wound was detected in 6 from 27 patients and postoperative synovitis in 7-9 days was observed in 6 patients.

[The vascular microrelief of the blood and lymph microcirculatory beds of the heart].

Peculiarities of topography and stereometry of the heart hemomicrocirculatory bed were studied by method of scanning electron microscopy of corrosive preparations. Different links of the heart hemomicrocirculatory bed were found to have expressed morphological signs, revealed distinctively by the methods named. This allows to differentiate them and to carry out an analysis of their functional peculiarities.

[The valvular apparatus and tissue organization of the endothelium of the thoracic duct].

The structure of ostial valves and valves located along the thoracic duct and of its branches ostial valves and right lymphatic duct ostial valves were studied in 30 experimental outbred dogs and 46 cats. Cryodestruction of thoracic duct was performed in 28 outbred cats. 1, 3, 7 and 14 days later perfusive fixation with intercellular borders impregnation was carried out with simultaneous examination of intact regions of intravalvular segments, cisterna chyli and area of thoracic duct trunks connection with valvular surfaces. Tissue organization in ageing was studied using the intervalvular segment of old animals. Specimens were studied by means of scanning electron microscopy and film preparations of endothelium. Valves, located along the thoracic duct length are bicuspid formations, while ostial ones are falciform and cuneiform respectively in 80 and 20%. Endotheliocytes of cuspids are characterized with high content of microfilaments bundles in the cytoplasm and low content of microvesicles. Cells of the valvular free margin cross the cuspid edge and have adaptive changes preventing their desquamation: fusiform shape, long basal processes and bundles of microfilaments in the cytoplasm. Peculiar "pericyte-like" cells alike with myofibroblasts lie deep in the cuspid thickness close to the sinusal venous side. Fascicles of the duct smooth myocytes reach the base of the valve. Besides, in the ostial valve stroma there is elastic membrane, better displayed along the cuspid venous side. Increased polymorphism and changes of the endotheliocytes metric characteristics were demonstrated in the zones of turbulent lymph flow. Analysis of the newly formed endothelium tissue mosaics allows to reveal mechanisms of monolayer repair: spreading and migration of endotheliocytes on the first day, their proliferation within three days, desquamation of newly formed endotheliocytes and spreading of adjacent cells on later stages.

[The lymphatic system of the lower extremities in patients with obliterating diseases of the terminal portion of the aorta and its branches].

The paper deals with the results of X-ray lymphographic and morphological studies of the peripheral lymphatic system in 82 patients with various stages of chronic lower extremity arterial insufficiency. Destructive and sclerotic changes in lymphatic vessels, inguinal lymph nodes, and impairments in lymph flow whose severity was in proportion to the stage of chronic ischemia were detected. The associated immune, barrier-filtration and transport dysfunctions called for their adequate correction. In patients with Stages II and III ischemia, direct controlled endolymphatic therapy promotes the improvement of tissue lymphatic drainage and hemocirculation in the diseased extremity and recovery of immune homeostasis in the region.

[Primary controlled endolymphatic antibiotic therapy in traumatology and orthopedics].

The work analyses 55 cases of traumatic osteomyelitis of the leg bones, open fractures of the leg bones, and medial fractures of the femoral neck. Patients with medial fractures of the neck of the femur were treated by endoprosthesis after Mur-Kaplan. Endolymphatic antibiotic therapy was included in the complex of therapeutic measures in each group of patients. This method allowed suppuration of the postoperative wounds to be avoided and promoted in this manner rehabilitation of the patients.

[Study of the lympho- and hemomicrocirculatory bed of lymph nodes by scanning electron microscopy].

The spatial interrelation of the lymphatic and blood beds has been studied in the lymph node together with its stromal and lymphoid elements, using scanning electron microscopy of corrosive casts and native preparations. There is a great variety of pathways for lymph transport and blood vessels in dependence of their localization in the lymph node. Of a special interest are the pathways of lymphocytes migration across the walls in the cortical and medullary sinuses and in the postcapillary venules, as well as participation of the reticular tissue in the lymph node immune reactions.

[Morphofunctional changes in the lymph nodes and microcirculatory bed of small-intestinal mesentery of dogs in septic peritonitis after endolymphatic administration of ampicillin].

With the aim to study effectiveness+ of endolymphatic (EL) administration of ampicillin (AC), using the model of an acute diffuse septic peritonitis in dogs, the morphological and morphometrical investigation has been performed concerning the state of the lymph nodes (LN), which are regional as regards the pathological focus (pelvic) and remote (tracheobronchial, mesenteric) and hemomicrocirculatory bed (HMCB) of the small intestine mesentery. All LN groups studied are involved in the pathological process, that produces certain increasing disturbances in the structure and cell composition in LN. In 6 h the changes are especially manifested in the pelvic LN, and in 18 h--in the animals without application of AC, or at its intramuscular injection LN lose their typical structure. Their dimensions and number of lymphoid nodules++ and medullary cords decrease, a sharp impoverishment of lymphocytes in LN is observed. By this time critical disturbances in the HMCB structure develop; they are characterized as presence of great amount of avascular areas in the mesentery, extended capillary loops, plasmatic saturation of interstitium. When AC is injected endolymphatically, simultaneously with peritonitis modelling T- and B-dependent zones in LN are preserved, a high volumetric part of lymphocytes is kept in all groups of LN, structure and function of HMCB are normalized. The pronounced delay in development and decreasing manifestation of infective-toxic disorder in LN and HMCB depend on effective concentrations of the antibiotic, produces in the lymphatic system.

[Microrelief of the internal surface of the thoracic duct in dogs (based on scanning electron microscopy data)].

The investigation has been performed on 17 mature dogs. With the aim to investigate the internal surface of the thoracic duct, the methods of scanning electron microscopy of native, impregnated and corrosive preparations have been used. The analysis of the scannograms obtained demonstrates that the valves of the thoracic ducts are mainly bicuspid, have a typical infundibular form, their cuspides are fused, forming a mesentery-like fold on the duct wall. It plays the main role in ensuring unilateral transport of lymph. The microrelief of the thoracic duct endothelium possesses a rather complex organization. The following relief formations are distinguished in endotheliocytes: 1. nuclei-containing eminence, 2. marginal processes, 3. separate microvilli. The dependence of the microrelief on arrangement of endothelial cells in the valvular area or intervalvular fragments is noted. This is also concerned orientation of endotheliocytes; therefore, predominance of stretched endotheliocytes of the postvalvular fragment can be connected with demonstration of viscous friction force in the lymph stream, that produces deforming and orienting influence on the endotheliocytes.

[The microcirculatory bed of the human heart].

Using a complex approach in studying microcirculatory bed of the human heart, possibilities of scanning electron microscopy of corrosive preparations, those of silver nitrate impregnation after V. V. Kuprianov are demonstrated. The silver nitrate impregnation makes it possible to study the wall structure of the microcirculatory pathways, to analyse arrangement of nuclei in the endothelial and muscle cells of the microcirculatory links, to reveal together with the vessels the surrounding tissues. Scanograms of the corrosive preparations of the arterioles demonstrate "circulatory strips", that are absent in the venular part. The relief of the luminal casts of the microcirculatory bed vessels in the human heart is presented as impresses of nuclei of the endothelial and smooth muscle cells. Peculiarities in form and distribution of these nuclei in various links of the microbed are demonstrated.

[Principles of organization of tissue mosaics of simple squamous epithelia].

With the aim to study general principles on organization of tissue mosaics of various types of simple squamous epithelii, a quantitative topological analysis has been performed in the frog mesothelial mosaics, in the posterior epithelium of the rabbit cornea and in the rat aortal endothelium after Lewis method. To all the types of simple squamous epithelii studied the histological law of Lewis can be applied. As demonstrates the qualitative analysis of the mosaics, the mesothelium and the posterior epithelium of the cornea are typical isotropic mosaics, while the mosaic of the aortal endothelium is nearer to the polystichous one. The possibilities of qualitative and quantitative topological analysis of the biological mosaics after Lewis method for studying their organization, degree of regularity, histogenesis and proliferative properties are discussed.

[Lymphatic bed of the heart and the nucleic acid content of the cardiomyocytes in alcohol poisoning].

The lymphatic channel of the heart in alcohol poisoning and the content of nucleic acids in the cytoplasm and nuclei of cardiomyocytes were studied. Four forms of lymphatic capillaries, postcapillaries and lymphatic vessels were found: functioning, "dystrophized", sclerosing, and regenerating. Disorders of interfragment connections interfering with the lymph flow were observed. A decrease in the content of nucleic acids in the cytoplasm and nuclei of cardiomyocytes in alcohol poisoning was established.

[Effect of alcoholic intoxication on the lymphatic bed of the heart].

Cardiac lymphatic bed was studied in 120 male corpses at the age of 17--55 years, normal and of those died from alcohol intoxication and from cardiac ischemia. The methods of intratissue injection of coloured masses, impregnation after V. V. Kuprianov, histological and histochemical investigations were applied. Data in figures were obtained by means of a grid for cyto-, histo- and stereometric investigations and by a scanning integrant microspectrophotometer sim-2. Statistical data were obtained by a computer "Mir". It was demonstrated that the cardiac lymphatic system responds in a definite way to alcohol intoxication. Four forms of structural state in the lymphatic bed elements were revealed, specific density of its different components was calculated. A certain connection was noted to occur between morphological changes under the effect of toxic doses of alcohol; there was also a reaction of compensatory-adaptive elements of lymphodynamics. A number of pathological changes in cardiac tissues resulting in heart insufficiency were presented. Peculiarities in poisoning phases were demonstrated morphologically.
