[The pathogenic role of Helicobacter pylori].

The urease test and bacterioscopy of impression smears were used to detect Helicobacter pylori (HP) in biopsies from pyloric gastric mucosa of 77 (89.5%) of 86 chronic gastritis patients, of 27 (4%) from 32 duodenal ulcer patients, of 84 (84.0%) from 100 healthy male subjects aged 18-20. There was focal hyperemia in pyloric part of the stomach in 59 patients, leukocytic infiltration of mucous membrane was found histologically in 78 ones. Close correlation between complaints, endoscopic and histological shifts, HP incidence rate was not registered. Positive results in determination of HP antibodies by enzyme immunoassay (EIA) were obtained in 32 (45.7%) of healthy subjects. EIA findings and histological evidence on HP presence failed to coincide in 42 (92.9%) duodenal ulcer patients.

[Comparative clinical characteristics of rheumatic and reactive arthritis].

The difficulties encountered in differential diagnosis of rheumatic and reactive arthritides are caused by the similarity of the articular syndrome not infrequently seen in both diseases, especially in their acute phases, a high frequency of little manifest forms of rheumocarditis and the rarity of other main manifestations of rheumatic fever as well as by a possibility of myocardial derangement in reactive arthritides. At the same time of paramount diagnostic importance are a detailed study of the disease history, estimation of the epidemic situation, careful clinical assessment of the character and evolution of the articular syndrome, the signs of lesions of the heart, other organs and symptoms, recognition of the symptoms characteristic for diseases of the group of seronegative spondylarthropathies, particularly Reiter's syndrome.

[The typical features and clinical characteristics of Reiter's syndrome with a nonvenereal origin].

Analysis of the clinical picture in 37 patients with Reiter's syndrome unrelated to venereal urethritis showed that in the majority of cases, it was preceded by acute intestinal infection which occasionally ran an unmarked course. The difficulties in diagnosing Reiter's syndrome were caused in some patients by the fact that in the disease debut, conjunctivitis and urethritis were little manifest and of short duration or occurred at the later stages, especially where the triad was preceded by the provoking factors, such as supercooling and nasopharyngeal infection. Also, in the acute phase of the disease, the patients demonstrated catarrh of the upper respiratory tract, the signs of myocardial injury and some symptoms characteristic of other rheumatological diseases. In this connection attention is drawn to the necessity of inquiring after the patients' health and sensations in doubtful cases, including the elucidation of the epidemiological anamnesis, thorough examination of the patients on admission to hospital and during the treatment to specify both the main and additional signs of Reiter's syndrome, and analysis of the disease course.

[A comparative evaluation of the efficacy of sucralfate and H2-histamine blockaders in treating peptic ulcer].

Sucralfate (55 patients), cimetidine (25 patients), ranitidine (30 patients), antacids and cholinolytics (32 patients) were studied and compared in open clinical trials for the efficacy in the treatment of ulcer disease of the stomach and duodenum. During 5 weeks, ulcers completely healed in 100% of the patients given sucralfate (in 94.5% for 4 weeks), in 90% of the patients treated with ranitidine, in 84% on cimetidine, and in 78% of the patients undergoing treatment with antacids and cholinolytics.

[Protein kinase activity and phosphorylation of endogenous proteins in the rat colonic mucosa during chemical carcinogenesis].

Treatment with a single dose of 21 mg/kg, of 1,2-dimethylhydrazine was followed by a significant decrease in the level of cAMP-dependent protein kinase (histone kinase) in rats' large bowel mucosa after 72 hrs. However, its level returned to basal value after 2 weeks, with casein kinase level remaining unchanged. Both carcinogen--induced tumors of the large bowel and adjacent mucosa showed a drop in protein (mol. wt.-50 kdalton) was enhanced in tumor considerably.

[Change in the protein kinase activity of glandular stomach mucosa in rats during malignant transformation].

Changes in the activity of protein kinases in the glandular stomach mucosa of rat were studied in the case of carcinogenesis induced by N-methyl-N'-nitro-N'-nitrosoguanidine. No essential changes in the activity of cAMP-dependent protein kinase (histone kinase) were found in the mucosa as well as in tissues of the developed tumours of the rat glandular stomach. The activity of cAMP-independent protein kinase (casein kinase) increased significantly 3 months after the beginning of the carcinogen administration and at the late stages (after 12-15 months) it was decreased considerably both in the glandular stomach tumours and in the stomach mucosa without the characters of the malignant growth. It is supposed that changes in the activity of casein kinase in the stomach mucosa at the late carcinogenesis stages are associated with the neoplastic transformation and precede the appearance of morphological characters of the malignization.

[Indices of protein kinase activity and the problem of biochemical diagnosis of malignant growth].

Protein kinase, cAMP-dependent and casein kinase activities and the ratio of these activities were determined in normal and neoplastic tissues of the colonic mucosa. Substantial activation of casein kinase and an appropriate decrease in the activity ratio of the enzymes were revealed in the malignant tissue. Protein kinase activity changed in a similar fashion in the histologically intact mucosa located 25-30 cm from the center of the tumor. The data obtained are suggestive of the possibility of using the above-indicated findings for the biochemical diagnosis of malignant neoplasms.

[Reactive arthritis in young men in organized collectives].

The authors followed up 398 young males with reactive arthritis who had been admitted from organized collectives to hospital settings. Only one third of them could connect the development of reactive arthritis (ReA) with a survived infection: acute nasopharyngeal (26%), acute enterocolitis (7%), urethritis venerea (1%). Analysis of the clinical picture demonstrated that in case of indefinite ReA etiology together with the signs of myocardiac involvement one should not only exclude the presence of rheumatic attack but also to establish a possible relation of the disease with a prior survived poorly manifest intestinal infection. This fact should be taken into account if in the collectives, where from the patients were hospitalized, epidemic outbreaks of acute dysentery or cases of ReA and Reiter's disease etiologically connected with manifest intestinal infection were documented. Postenterocolitic origin of ReA could be suspected in patients with its continuous course and resistance to antiinflammatory therapy in whom arthralgia was preserved for a long time as was joint rigidity after a subsidence of joint changes due to the effect of acute inflammation. Besides, the detection of certain features of the articular syndrome and separate extraarticular signs characteristic of Reiter's disease and other conditions which comprised the group of seronegative HLA--B27-positive spondyloarthropathy are of high diagnostic value.
