[Autonomic disorders in Charcot-Marie neural amyotrophy].

The paper is concerned with disorders of the autonomic nervous system (ANS) in Charcot-Marie amyotrophy. 32 patients were examined. Of these, 21 suffered from the demyelinizing type of the disease and 11 from the axonal one. The status of the peripheral part of the ANS was estimated in the cardiovascular and in the respiration systems as well as in the system of pupil innervation. The patients were distributed into groups with the above-mentioned disease types on the basis of the electroneuromyography data. The relationship was demonstrated between the intensity of vegetative disorders and the genetically determined type of nerve fiber injury--myelinopathy and axonopathy. In the axonal form of Charcot-Marie amyotrophy, the involvement of vegetative fibers into the pathological process was appreciably higher than in the demyelinizing type.

[Autonomic disorders in parkinsonism].

The authors analyze the clinical and paraclinical manifestations of vegetative disorders in parkinsonism. It is shown that vegetative disorders are relatively independent of motor parkinsonism manifestations and run their course by the type of concomitant sympathetic and parasympathetic insufficiency. Injury to the segmental and suprasegmental mechanisms of motor control underlies vegetative disorders. The role of segmental and suprasegmental disorders in degenerative diseases of the nervous system with marked vegetative manifestations is analyzed and compared.

[The clinical polymorphism and treatment of muscular dystrophy].

The paper provides a brief review of mainly foreign publications of recent years, concerned with the clinical pleomorphism, etiology, diagnosis, classification and treatment of diverse dystonic syndromes. Special emphasis is laid on the new or unusual clinical patterns of dystonia as well as on the latest advances in its treatment.

[The concept of "ganglionitis" in the modern science of the autonomic nervous system].

The authors review the concept of vegetative "ganglionitis" formed in the Soviet literature in the 40-60s. Consider the etiological, clinical and diagnostic aspects thereof. Provide the modern concepts of the clinical manifestations related to the lesions of the paravertebral sympathetic ganglia. Relate some hypotheses concerning the origin of painful phenomena within the framework of the syndrome of reflex sympathetic dystrophy. Indicate that verified lesions of the sympathetic ganglia are associated with well-defined clinical symptoms which absolutely differ in their appearance from the reported descriptions of "ganglionitis", which are unjustifiedly widely used in the Soviet literature over the recent decades. The conclusion is made about the necessity of reviewing the outdated concepts of the pathology of the vegetative sympathetic ganglia from the standpoint of the modern concept of peripheral vegetative insufficiency.

[Autonomic disorders in Parkinson disease].

The authors provide the clinical and laboratory findings regarding the vegetative nervous system in 60 patients with parkinsonism and in 11 healthy volunteers. It has been found that parkinsonism is necessarily associated with vegetative disorders which are pronounced to a greater degree in right-handed patients and bilateral clinical manifestations of the syndrome.

[Lateralized forms of parkinsonism (clinical, electroencephalographic and psychomotor aspects)].

As many as 101 parkinsonism patients were examined. The mean patients' age was 50 years. Of these, 46 patients (12 with left-handed clinical manifestations of parkinsonism, 17 with right-handed, 17 with bilateral ones) were subjected to comprehensive examinations -- clinical, electrophysiological and psychomotor. Ten healthy persons of the respective age and sex made up the control group. The examination made it possible to reveal the clinical and electrophysiological characteristics of the different patterns of parkinsonism. At the same time it allowed the conclusion about the involvement into the pathological process of both hemispheres whatever the primary lateralization of the clinical manifestations; about varying roles played by the right and left hemispheres in the pathogenesis of parkinsonism.

[Hysterical hyperkineses].

A clinico-laboratory study has involved 42 patients with hysterical hyperkineses. The data obtained have made it possible to identify four stages of the diagnosis which are important fo clinical identification and differential diagnosis. Correlation between the pathogenetic mechanisms of hysterical hyperkineses and the mechanisms of the psychogenesis of hysterical neurosis has been demonstrated. The results obtained have made it possible to come closer to the understanding of the complex neurophysiological and psychophysiological substrate participating in the symptom formation of hysterical hyperkineses.

[Phenomenon of inverted rotation in spastic torticollis].

The article describes a rare clinical case where a patient with right-side spastic torticollis developed stable left-side spastic torticollis following discontinuation of nacom. On the basis of clinico-physiological analysis of the given phenomenon and having compared their findings with the literature data the authors substantiate the involvement of the nigrostriatal systems of both hemispheres in the pathogenesis of torticollis and in the mechanisms of rotation inversion reported in the given disease for the first time.

[Facial paraspasm].

Thirty-two patients with facial paraspasm were subjected to comprehensive examination with the use of psychometric and electropolygraphic methods, with the brain awake and asleep. On the basis of the results obtained the author considers clinical differential diagnostic criteria, as well a possible neurodynamic substrate of the above syndrome. Many levels of the nervous system, particularly nonspecific formations of pontomesencephalic portions of the brain stem closely linked with subcortical and rhinencephalic structures, are involved in ensuring this substrate.

[Facial hemispasm].

Using methods of clinical, psychometric and electropolygraphic examination 36 patients with facial hemispasm (FH) were studied in the sleep-awakening cycle. The authors describe neurodynamic shifts at both peripheral and supranuclear levels of facial innervation. These shifts were correlated with the severity and side of damage. Different levels of the nervous system, including the brain stem and cerebral non-specific formations, were involved in the pathogenesis of FH.

[Clinico-physiologic analysis of psychogenic tics in children].

The results obtained in a study of 100 patients formed the basis for major differential diagnostic criteria of psychogenic tics in children. Polygraphic examination of the sleep-wakefulness cycle revealed dysfunctions in the nonspecific cerebral systems in the form of hyperfunction of the activating and insufficiency of the inhibitory apparatuses which were expressed in all functional states in the EEG, motor and vegetative spheres. These changes are considered to be pathogenetically important in view of their connection with the most significant clinical characteristics of the disease. On the basis of electrophysiological indicators, the data were obtained which substantiate therapeutic efficacy of psychostimulators in patients with the syndrome of motor hyperactivity.

[Functional state of non-specific brain systems in dystonia musculorum deformans].

The paper deals with the results of an electropolygraphical study of night-sleep structure, as well as a clinical and experimental-psychological study of 27 patients with deforming muscular dystony. The quantitative and qualitative characteristics of functional shifts in different links of the nonspecific system in the known clinical forms and at different stages of the disease are given. The data were obtained with the aid of correlational statistical methods and were used for a clinico-physiological analysis of the syndrome of deforming muscular dystony. It is demonstrated that changes of a functional state of the nonspecific brain systems correlate with motor, emotional-personal vegetative and electroencephalographical manifestations of deforming muscular dystony.

[Dreams under normal conditions and in pathology].

Experiments on wakening from "rapid" and "slow" sleep, as well as clinical and epidemiological studies revealed differences in the reports of dreams. These differences were in correlation with the character of cerebral or somatical pathology and with the representativeness of phasic components of "rapid" sleep.

[The role of disorders of "non-specific systems" of the brain in the pathogenesis of the Parkinsonian syndrome].

The authors conducted a clinico-physiological analysis of the syndrome of parkinsonism in different functional states (intense vigilance, flaccid vigilance, various phases and stages of night sleep, a hypnotic state) using the method of a polygraphic registration and an experimental-psychological study. The functional state of so-called nonspecific brain systems in parkinsonism and their disturbances in the pathogenesis of the disease was studied. A complicated character of shifts in different chains of the non-specific systems with various clinical forms and at different stages of the diseases were demonstrated. The authors proposed a method of quantitative evaluation of these shifts. It was demonstrated that motor, emotional-vegetative, somnic and electroencephalographic manifestations in parkinsonism correlated with the character and degree of the pathological changes in the non-specific, mainly activating brain systems.
