[Reconstructive surgery in injuries of major arteries and veins].

Retrospective analysis of surgical treatment for injuries of 408 major arteries and veins in 352 patients in the department of vascular surgery of N.V. Sklifossovsky Research Institute of Emergency Care from 1989 to 1996 is presented. Of 126 cases of prosthetic reconstructions in 72.2% autoveins were used, in 18.3%--synthetic prosthesis, and in 9.5% homologous arterial biologic prosthesis. Good results were obtained in 13.1% of patients, amputations were made in 4.8%. Lethality was 4.0%. Principles of treatment of combined injuries, prophylaxis of syndrome of inclusion and other complications are autlined.

[Treatment of pyogenic complications after reconstructive surgery on blood vessels with use of synthetic prosthesis].

Factors influencing result of surgical treatment of patients with festered postoperative wounds have been studied. Algorythm of determination of tactics for examination and treatment of this category of patients was established. The results obtained are based on the analysis of personal surgical treatment autcomes in 1526 patients with decompensated ischemia of extremities and 57 patients with clinical symptoms of deep suppuration. Division of purulent complications into primary and secondary is suggested. Effectiveness of various modes of prophylaxis was studied--systemic and indirect endolymphatic route for introduction antibiotics, intraoperative application of pellicle ESBA, usage of antimicrobial vascular prosthesis "SISAN". The best results were obtained in the two latter methods. Various methods of surgical treatment in infected vascular prosthesis are reviewed.

Anti-Bacterial Agents↗

[Surgical tactics in prosthesis related vascular graft infections].

The outcomes of treatment of patients with purulent wound complications and infection of vascular grafts after plastic surgery on terminal part of the aorta and major arteries of the lower extremities are analyzed depending on the tactics and timing of the surgery. Minimal lethality was registered in re-do surgery in the infected grafts performed in 2 or 3 stages, with the use of antibacterial prosthesis SISAN. This provides an opportunity to eliminate recurrence of infective complications.

[Ways of lowering the level of amputation in decompensated ischemia of the lower extremities].

From analysis of the results of surgical treatment of 127 patients with decompensation of circulation and gangrene of the lower limbs the authors substantiate the prospective character of revascularizing operations which lower the level of the amputation. Revascularization by means of reconstructive operations on the main arteries of the limbs makes it possible to not only lower the level of the amputation, but reduce considerably the lethality, the incidence of postoperative complications, and the degree of invalidation.

[Reconstructive operations in suppurative and septic complications of vascular surgery].

Results of the preventive surgical treatment in infection of vascular prostheses were much better than those obtained after operations performed against the background of arrosive hemorrhage. The division of the operation into two steps--the extrafocal shunt at the first step and removal of the purulent prosthesis at the second step--results not only in better immediate results of the treatment of these complications, but also in reduced lethality in this group of patients. The traction mode of removal of the prosthesis is recommended to patients with a pronounced scarry process around the prosthesis.
