[Status of protective properties of the gastroduodenal mucosa after surgical treatment of duodenal ulcer].

The defense properties of the gastroduodenal mucosa (lymphoid system cells secreting immunoglobulins A, M, and G, as well as the secretory component IgA) were studied in 85 patients with duodenal ulcer. The acid-producing function of the stomach was tested at the same time according to the pH value and the basal and maximal acid production. Various types of vagotomy were conducted in 52 patients and resection of the stomach in the remaining patients. The results were followed-up before and after the operation. Peptic ulcer was characterized by secondary immunodeficiency. Selective proximal vagotomy restored the tissue immune defense of the mucosa, gastric resection preserved and even increased immunodeficiency.

[Selection of the method of reoperation after closure of perforated duodenal ulcer].

The work deals with the study of the fate of patients who underwent closure of a perforated opening in a duodenal ulcer. In 27-53% of such patients subsequent nonoperative treatment was ineffective and they were subjected to reoperation: resection of the stomach or vagotomy. After comparative evaluation of these two reoperations on the grounds of the clinical data and the secretory and evacuating function of the stomach which had been operated on, the authors give preference to selective proximal vagotomy as an operation that is more physiological and devoid of the short-comings of gastric resection.

[Aspiration puncture biopsy in the preoperative diagnosis of adrenal tumors].

The results of fine-needle aspiration puncture (AP) and cytologic examination in the preoperative period in 4 patients with various lesions of the adrenals are discussed. AP was conducted under local anesthesia and control by ultrasonic examination; no complications occurred. Analysis of the cytologic findings allowed preoperative morphological verification of the process, as a result of which the initial clinical diagnosis was changed in 2 of the 4 cases. With an exactly established preoperative cytologic diagnosis a rational therapeutic tactics can be chosen, and the necessity of using complicated examination methods, diagnostic laparotomy among others, obviated.

Adenoma

[Intraoperative pH-metry in the prevention of incomplete vagotomy and ischemia of the gastric wall in selective proximal vagotomy].

Experiments were conducted on 14 mongrel dogs to appraise the validity of the results of intraoperative pH-metry in the control of vagotomy completeness depending on the force of pressure exerted on the pH electrode with consideration for the type of a pH catheter used. The level of parietal pH increased with elevating pressure force due to diminished circulation in the gastric wall. The encountered diminution of circulation in the gastric wall after SPV increases still more the pH level and reduces the amplitude of its growth even in areas with maintained parasympathetic innervation with the use of histamine, which raises the probability of incomplete vagotomy. The authors conclude that pH-metry by slight application of the pH electrode to the gastric wall raises the validity of the values recorded and reduces the frequency of incomplete vagotomy in patients operated on in the clinic by 4.7 times. Increase of parietal pH to more than 6.7, irrespective of the force of the exerted pressure, is an indirect sign of ischemia of the gastric wall.

[Functional status of the operated-on stomach in patients with post-burn esophageal stenosis].

The article discusses the results of complex functional examination of the stomach in 41 patients with post-burn stenosis of the esophagus in different postoperative periods. Group I consisted of 21 patients with isolated chemical burns of the esophagus, group II was made up of 20 patients with combined chemical burns of the esophagus and stomach. Special new methods of examination (3-canal and endoscopic pH-metry, balloonography, radioisotope gastroscintigraphy, radioimmunological test for the hormone gastrin) were conducted to study the functional condition of the stomach which had been operated on. Objective evaluation is possible only by complex use of special methods of examination.

[Autodermal plasty in postoperative ventral hernias].

The method of autodermal plasty was used in 299 patients with postoperative ventral hernias. Among them great and giant hernias made up 62,8%. The autodermal method of plasty results in less amount of complications in the wound which can be explained by an increased concentrations of humoral non-specific defense factors in the operation field due to the effect of the skin flap. Remote results were followed in 84% of the patients during 2-6 years. Recurrences were detected in 3,2%.

[Association of stomach and duodenal ulcers and diaphragmatic hernia].

On the basis of complex examination of 83 patients with the ulcer disease including ionomanometry 25 of them were found to have hiatal hernia. Specific features of the clinical picture of this combination are described. The detection of hiatal hernia in patients with the ulcer disease allows the correction of the valve mechanism of the esophago-gastric transition to be simultaneously fulfilled during the operation.
