[Experience with the use of standardized inference in Doppler echocardiographic study].

The authors propose a scheme for the inference on the results of ultrasonic cardiovascular investigation in cardiological patients. The scheme for putting into the computer has been developed in the A. N. Bakulev Institute for Cardiovascular Surgery. Its introduction improved the diagnosis, allowed monitoring of the follow-up examinations with comparison of the results in different patients as well as statistical processing of the findings and making diverse calculations. The programs designed imply the work of an operator whom should be a specially trained physician in the department of ultrasonic diagnosis.

[The use of personal computers in the angiocardiometric evaluation of the pulmonary artery in complex congenital heart defects].

Application of personal computers in angiocardiometric assessment of the pulmonary artery in such congenital heart diseases as Fallot's tetralogy and total transposition of great vessels made it possible to set up a convenient data bank allowing one to store numerical and textual information, conventionalized drawings of pulmonary arteries showing the degree and location of stenosing lesions and color-marked data on surgically significant constrictions. The system has a rich visual presentation of data and allows for convenient and quick search and processing of information (including the use of standard statistic, graphic, and other present-day packages of analysis and presentation of medical information).

[Risk factors in Fontan's operation].

The results of Fontan's procedure performed in 51 patients with various congenital heart defects were analysed. The procedure was found to have the following risk factors: the total lung resistance is above 4 mu/m2, the ejection fraction of the arterial ventricle, under 55%, serious concomitant heart defects, and additional surgical procedures that increase the scope of surgical interventions. The best results were obtained after creating a valveless atriopulmonary anastomosis; the poorest results were associated with a right atrium-pulmonary trunk anastomosis possessing its own valve.

[Modern computer data base "necropsy protocol": potential and prospects].

Basic principles in developing computer variant of necropsy protocol with wide illustrative possibilities are presented. Perspectives in the use of this information base for research and practice and for teaching the staff of the research and clinical institutions are demonstrated.

[Methodology of supplying the work place of medical workers with computers].

The new approach to the construction of the problem-orientated automated working places of the main-modular structure, suggested by the authors, solves not only the concrete problems of the clinical subdivision in collecting, storing and processing information but can also be the basis of the information system of a health care facility as a whole (e.g. diagnostic centers of various purpose, specialized clinics and outpatient polyclinics).

[Experience with the creation of microcomputer-based automated case history system].

The article sums up the experience in using an automated case history handling system at the Bakulev Institute of Cardiovascular Surgery for improving treatment and raising research efficiency. The system is designed for collecting, storing, processing and giving out information about the results of diagnosis and treatment of patients in the form convenient for the physician. It is based on the dialogue principle in respect of setting, searching and processing medical data and on the principle of independent work of the personnel with the system terminals installed in different clinical departments.

[The epidemiology of occlusive lesions of the branches of the aortic arch].

The most significant risk factors of the development of this pathology are: arterial hypertension, IHD, smoking. A group of patients younger than 50 was three times less that a group of patients 51-60 years old. Based on data of morbidity the authors counted the spread of occlusive lesions of the aorta arc branches in the population of men studied, which amounts 4.15%.

[Digital subtraction angiography in the diagnosis of Fallot 's tetralogy].

The authors analyze the efficacy of digital subtraction angiography (DSA) in the diagnosis of Fallot's tetralogy (FT); this method helps simplify and cut down the scope of investigations but does not deteriorate their informative value. DSA findings in 120 patients with TF are analyzed. The study was confined to right ventriculography in the majority of cases, which permitted clear-cut imaging of the anatomical details of the defect due to computer processing of the image. Morphometrical parameters were used to make assessment of involvement of the pulmonary artery system more objective: the ratio of the width of various portions of the pulmonary artery system to the width of descending aorta and the pulmonary arterial index characterizing the area of section of both pulmonary arteries in relation to body surface. The quality of imaging the stenoses and hypoplasia in the pulmonary artery system was high, meeting the requirements of present-day surgery. Moreover, DSA helps diagnose other anatomical particulars of TF and the concomitant defects and abnormalities, left ventricular hypoplasia and the type of right aortic arch among other things. The advantages of DSA in the diagnosis of TF are a lesser volume of catheterization, reduced number of sessions and of the dose of contrast agent infused at each session, better imaging of the anatomical details of the defect and of concomitant defects and abnormalities. All this permit examinations on an outpatient basis.
