24-year preoperative evolution of a temporal astrocytoma.

We report the case of a patient suffering since 5 months of age from complex focal seizures that could not be controlled with medication and who developed severe psychomotor retardation. When she was 25 years old she was operated on for a left temporal lobe type II astrocytoma which had been detected but misinterpreted on CT scans performed at 12 and 18 years of age. After surgical and radiotherapy treatment the patient was seizure-free for 9 months, but then epilepsy reappeared and the patient died 19 months after surgery. Postsurgical malignant transformation is suspected but not histologically confirmed.

[Benign neonatal convulsions. Review of 23 cases].

Benign neonatal convulsions, though rare, are seen with increasing frequency and are characterized by seizures during the neonatal period with favorable prognosis. Two distinct entities can be identified, based on whether the syndrome is familial or non-familial, with epilepsy developing in 14% of patients with the familial form. We report a retrospective study of 23 patients, 13 with benign familial neonatal seizures and 10 with benign idiopathic neonatal seizures. All except one had normal neurologic development. We observed central temporal (rolandic) EEG foci in the follow-up of a few patients in both groups, with no clinical manifestations. We consider the possibility that these entities may share common genetic factors with benign rolandic epilepsy.

Optic gliomas in neurofibromatosis type 1 (NF-1). Presentation of 31 cases.

We present a series of 31 cases of optic glioma associated with neurofibromatosis type 1 in patients under 11 years of age. They represent 64% of all the optic gliomas seen in our service between September 1965 and September 1993. The optic nerves were affected in 25 cases (83%); 9 children (30%) had a isolated, unilateral tumor; 16 (53%) had involvement of the optic chiasm as well as of one or both optic nerves. In 6 cases (19%) only the chiasm was affected, with or without involvement of other intracerebral structures. A coincidental orbital plexiform neurofibroma was associated with a poor prognosis. Other complications included 8 cases (26%) presenting with slowly developing aqueductal stenosis requiring treatment with a shunt. High-resolution computed-tomographic scans provided well-defined images for the diagnosis of optic nerve glioma, but magnetic resonance imaging was the preferred procedure for the diagnosis of gliomas involving the chiasm and the optic tracts and radiations, particularly if there was no mass effect. In our patients, tumor growth was hardly noticeable during follow-up even up to 20 years, with no difference between patients who were treated with radiation and those who were not treated. However, endocrine disturbances developed in all cases subjected to radiotherapy, and were less frequent in untreated patients. Aqueductal stenosis was observed with similar frequency among both treated or untreated patients. Three patients died, two due to complications of hydrocephalus secondary to aqueductal stenosis and one because of respiratory problems due to compression of the hypothalamus and brainstem by the chiasm tumor. Two of the 3 patients who died had orbital plexiform neurofibroma.

Is cerebral arteritis the cause of the Landau-Kleffner syndrome? Four cases in childhood with angiographic study.

Four children with Landau-Kleffner syndrome were studied over a six year period. They presented with acquired aphasia, epilepsy, and focal or generalized EEG discharges which were exacerbated during sleep. In addition, cerebral angiography demonstrated isolated arteritis of some branches of the carotid arteries in all cases. Computed tomographic and magnetic resonance images were normal. Nicardipine in a dose of 1 to 2 mg/kg/day, added to conventional anticonvulsant drugs provided effective supplementary control of seizures, of paroxysmal EEG discharges, and of language and behavioural disturbances, even several years after the onset of the disorder and in patients whose response to other medications, including steroids, had been poor. Interruption of nicardipine administration was followed by relapse of the language disorder. Repeat angiography was performed in all four patients and showed recanalization of obstructed vessels in two cases. Focal cerebral vasculitis may be the pathogenesis of the Landau-Kleffner syndrome and calcium channel blockers such as nicardipine may be effective and specific therapy.

[The role of neonatal electroencephalography in the prognosis of the hypoxic-ischemic syndrome].

One hundred and thirty four newborn children of 37 weeks' gestation age or greater, with evidence of hypoxic-ischemic encephalopathy following perinatal asphyxia and surviving through the neonatal period were prospectively studied. The study involved evaluation of clinical outcome and the presence of epilepsy associated with the EEG pattern in the neonatal period. The results prove that the presence of a burst suppression EEG pattern and a hypoactive/flat EEG are negative prognostic criteria. In presence of a focal or multifocal pattern it is difficult to establish a clear prognostic criteria. Last, in newborn children with a normal EEG; with a low amplitude register; with frequencies slow or with bilateral sharp waves transients, a good prognosis can safely be established.

[Acquired aphasia syndrome with epilepsy (Landau-Kleffner syndrome) secondary to cerebral arteritis. 4 cases].

We report four cases of acquired aphasia with epilepsy syndrome or Landau-Kleffner syndrome in three boys and one girl who were previously in good health. Along with the clinical picture, we describe cerebral arteriographic findings, which consisted of obstructive arteriopathy of small and medium size vessels in delimited areas. A specific cause of the arteriopathy was not found in any case. We discuss the treatment (corticosteroids and calcium antagonists) and the course of the patients. In front of the possibility of being isolated cases, we discuss the interest of performing cerebral angiographic studies in patients with Landau-Kleffner syndrome in initial stage in order to confirm the consistence of these findings and the adequate treatment.

[Focal and multifocal electroencephalogram in the neonatal period. Prognostic correlations].

In a prospective study of 35 newborns, with acute neurological manifestations of diverse etiology and focal or multifocal neonatal EEG recordings, both clinical and EEG evolution wee evaluated at three and fourteen months of age. More than half of the patients who presented this type of recording at birth were found to have neurological sequelae and almost one fourth had seizures upon posterior evaluation. Authors compare this type of EEG recording with others observed in neonatal period and discuss their results in the light of etiologic, clinical and EEG findings in the literature.

[Etiology of mental deficiency. Study based on 3735 cases (author's transl)].

A study on the etiology of MD was done on 3735 children with mental retardation. Selection of patients was verified by precise alphabetical train in order to obtain the different factors of MD with objectivity. The highest percentage of MD was constituted by pathology concerning pre- and/or peripartum problems (53.78%), being on the contrary very low the percentage of MD of unknown etiology (8.219%). Authors think that it is possible to verified correct diagnosis of cases with MD if the clinical knowledge is large the adequate para-clinical studies (biochemistry, EEG, EMG, cytogenetic and neuroradiology) are practised. The unknown etiology of MD must be low. In their opinion MD accompanying CNS malformations and dysmorphic syndromes must not be classified as MD of unknown etiology. Emphasis is done on the necessity of having in consideration real percentages of etiological factors when programs for education of subnormal children are developed.

[Generalized ceroid-lipofuscinosis].

A histological verified case of late infantil type of generalized ceroid-lipofuscinosis of Jansky-Bielchowsky syndrome is reported. Paroxismal activity provoked by a low frequency fotostimulation or Pampiglione phenomenon is registrated at the E.E.G. An analysis about the clincial findings and a review of the Pampiglione phenomenon in the medical literature is made, being stimated this phenomenon of a high value for the diagnosis fo the type II or late infantil type of generalized ceroid-lipofuscinosis.

[Lodgement of the intraventricular catheter into cerebral parenchima in infantile hydrocephalus (author's transl)].

Ten cases of hydrocephalus of diverse ethiology, treated with ventriculo-peritoneal shunt are presented. In following controls it was observed that extreme of the catheter was located in cerebral parenchima. These features were followed of minimal clinics alterations and electroencephalographic focal manifestations. The pathogenesis of the complication due to the findings in the computerized axial tomography (CAT) and the real value in the diagnosis of this complication are analized.

[Sturge-Weber syndrome. Findings in 22 infantile cases (author's transl)].

Twenty two infants, 13 females and nine males, with Sturge-Weber syndrome are reviewed. Facial "nevus flammeus" is found to be located unilaterally in 13 cases and bilaterally in nine cases. Twenty infants presented seizures with generalized type being most frequent. E.E.G. alterations appeared in 18 cases. The I.Q. was normal in five infants although somewhat lower than the inferior limits of a normal I.Q. range. The major radiological alterations seen were an increased thickness of the skull cap; intracranial calcification (11 cases), unilateral in eight and bilateral in three; cerebral hemiatrophy (10 cases), arterial and venous hypoplasia and tortuosity in the entire extension of the carotid artery. One of the infants died with no apparent cause. This death cannot be attributed to the orignal syndrome. Computerized tomography offers early adequate data concerning the intracranial calcifications and cerebral hemiatrophy. Based on this study there does not seem to be sufficient motives to separate the Sturge-Weber and the Klippel-Trenaunay syndromes into two different entities.

Angiomatosis↗

[E.E.G. in hydrocephalia with shunt (author's transl].

Twenty five cases of infantile non-tumoral hydocephalia that have been submitted to shunt therapy are presented. These cases presented EEG alterations of a focal character which were persistant or transitory at some point in their evolution. These alterations are attributed to lesions provoked by the catheter after perforating cerebral cortex or to intraventricular extreme in the same ventriculum where catheter was implanted or in contralateral ventriculum after performation of the septum pellucidum. These lesions can possibly provoke epileptical seizures and are more frequent in those cases with a larger number of surgical interventions secondary to obstructions of the shunt due to major trauma to which they have been submitted. It should be noted that the above facts need to be taken into account in order to realize an adequate therapy.

[Cerebral tumours in infancy (author's transl)].

37 cases of cerebral hemispheric tumours are presented. These make up 23,1% of all the intracranial tumours observed over a period of 11 years in a Child Neurology Service. 19 cases were males and 18 females. Their ages were between 16 days and 7 1/2 years. Vomiting and headache were usually the first symptoms followed by seizures, frequently of the focal kind. Motor difficulties used to appear later. 69.6% of the cases presented a malfunctioning focus on the E.E.G. on the side of the tumor. The simple cranial X-Rays showed firstly widening of the fronto-parietal sutures, intracranial calcifications were seen in some of the cases with ependymonas and piloid astrocytoma in patients with Bourneville's disease. Pneumoencephalography as well as carotid angiography, radioisotope examination and computerized tomography gave us very positive results in the localization and determination of the size of the tumor. The ependymomas showed pathological vascularization regularly. The nature of the tumours corresponded to: 14 cases of ependymoma, 8 cases of astrocytoma I and II types, 1 case of astrocytoma of types III and IV, 3 cases of plexus papilloma, 2 cases of meningioma, 1 case of sarcoma of the basal ganglia, 1 case of teratoma, 3 cases of indifferentiated malignant tumours, 4 cases not proven. There was a 20% survival of patients five years later.

[The prevalence of headache in a population of patients with coeliac disease].

INTRODUCTION: Headache is the commonest clinical symptom during childhood and adolescence, from a neurologist s point of view. The pathophysiology of migraine and tension headache involve personality and biochemical factors, such as serotonin, which are also common in coeliac disease. OBJECTIVE: To establish the prevalence of headache in children and adolescents with coeliac disease, and any possible relation between these conditions. PATIENTS AND METHODS: We made a randomized selection of 86 patients with coeliac disease. All were on gluten-free diets and had no current gastroenterological symptoms. They had direct interviews and full physical and neurological examinations. The diagnosis of headache was based on criteria of the International Headache Society in 1988. RESULTS: The average age was 12.71 +/- 4.5 years (range 5-24). Headache occurred in 34 (39.5%) of the 86 patients studied. In 18 cases (20.9%) headache was of tension type and in 16 (18.6%) of migraine type. Of the latter, 10 cases had auras and 6 did not. There was no significant sex difference. CONCLUSIONS: An increased prevalence of both migraine and tension headaches was observed in the coeliac patients studied as compared with data published in the literature. In the former patients there was also a lower frequency in histories of migraine in first degree family members. This data is probably related to the personality of the patient or to his family or social circumstances in the case of tension headaches. In the case of migraine it may be due to biochemical factors such as a lowered plasma serotonin, seen both in coeliac disease and in migraine.
