The effect of trauma program registry on reported mortality rates.

BACKGROUND: This study assesses the relationship that the brand of trauma program registry (TPR) has on mortality rate (MR) in the reports prepared by the American College of Surgeons Committee on Trauma (ACSCOT) trauma center (TC) site surveyors. METHODS: Data from 242 ACSCOT adult TC survey reports (88 Level I, 115 Level II, and 39 Level III) were analyzed for annual trauma volume, injury severity score (ISS), MR, and TPR. Six TPR (A through F) were identified; group F was a composite of several infrequently used TPRs. This report focuses on the ISS range 16-24 because of the likelihood that the mean for each TC would be near 20 and MR is high enough so that a difference, if present, could be statistically documented. RESULTS: For the total group, MR showed no correlation with TC volume or TC level for ISS 16-24. MR was significantly different according to which TPR was used by the TCs. The MR is less (4.8%) for 14 high volume TCs (over 1200 admits) using TPR A compared with 33 low volume TCs (below 800 admits) using TPR A (6.34%). CONCLUSION: The MR for ISS 16-24 in ACSCOT-surveyed TCs differs within subgroups based on type of TPR utilized. This may reflect improper use of the software programs. Enhanced skill in the application of software programs designed to generate ISS scores is essential if meaningful studies on the effects of improved trauma care on MR are to be conducted. Hand scored ISS by trained personnel may circumvent this problem.

Pap smear brochures, misogyny and language: a discourse analysis and feminist critique.

Text from a public health brochure on pap smears was analysed with particular reference to ways in which the language used conveys particular messages about women and their bodies. In the text, 'humans' were found to be excluded as such. Both the pap smear service provider and the women who are the recipients of this service--and at whom such brochures are targeted--are objectified and their characteristics of human existence (i.e. ontological capacities) were restricted. The language of the pamphlet invokes an image for women associated with vaginal (penile penetrative) sex. The discourse also is found to be didactic, biomedical and written in the voice of the service provider. Further, the encounter of pap smear events is contextualized as procedural such that not only is the woman 'done to' in the process of having a cervical smear test but the woman's and provider's experiences of the encounter are silenced. It is concluded that the texts may be viewed as misogynist and that such texts do not take account of the complexity of women's decisions to 'submit to' or comply with cervical cancer screening.

Training strategy for nursing staff in using computers in hospitals--a Malaysian viewpoint.

The ability of hospitals to fulfil their roles--of information processing and dissemination, and of quality patient care provider--is influenced by the availability of supporting information systems. Using computers in wards, which is a change process, introduces new working practices accompanied by attitudinal and knowledge alterations in the users. This paper suggests that as a practical approach users need to be consulted and assessed prior to the introduction of computers in their work places. A questionnaire survey, the main purpose of which was to determine the potential users' responses and to measure their computer competencies, was sent to 183 nursing staff in several hospitals. Results show that the respondents have slightly positive attitudes towards computers even though 85% of them were computer illiterate. A training strategy is needed to increase competencies and to develop more favourable attitudes, which can be monitored using four training indicators.

Typists' influences on transcription: aspects of feminist nursing epistemic rigour.

This paper describes the process by which female typists became participants of sorts while transcribing audiotaped interviews. The primary data included sensitive and sexual subject matter about women's Pap smear (PS) experiences. Besides overcoming minor technical difficulties, the major discovery was that the typists had a tendency to interpolate and even normalize the data. A feminist post-structuralist perspective was extended to examine these interpolations as secondary data. This revealed that the typists were becoming unexpected participants as commentators, validators, analysts and normalizers of the data. From the subjective positions of both medical typists and recipients of PS, the typists provided data validation that included contradictions and transgressions from the cancer screening scheme propaganda. Furthermore, both typists were exposed to other women's PS stories and experienced vicarious consciousness raising. For one typist this included embodiment of other women's distress, raising questions about the occupational hazards associated with transcribing distressing data. Thus the social and relational aspects of those involved in the transcription process proved influential to both the data (and analysis), and to the practical aspects of data management. I argue that details of such influences need to be submitted for scrutiny as additional aspects of rigour.

Portable ultrasonography in critical care.

A 2-year retrospective analysis of portable ultrasound examinations in the intensive care unit was conducted to evaluate the indications and efficacy of portable sonography. A total of 86 examinations were performed on 79 patients. Of these, 22 per cent of the examinations supported the suspected diagnosis and contributed to treatment. Seventy per cent of the examinations excluded the suspected diagnosis. There was a 9 per cent incidence that yielded unsuspected information that contributed to treatment. Only 8 per cent of the examinations were misleading or proven false. The authors found that with an overall sensitivity of 79 per cent and specificity of 97 per cent, portable ultrasonography is a valuable, relatively inexpensive test in the critical care setting.

Superficial bladder tumors (stage pTa, grades 1 and 2): the importance of recurrence pattern following initial resection.

We treated 414 new patients with stage pTa, grades 1 and 2 bladder tumors by transurethral resection between 1970 and 1982. All of the patients with grade 3 or previous upper tract tumors, or who had been treated at some stage with intravesical chemotherapy were excluded. Followup for 5 or more years was available in 188 of the patients. There was a low increase in T stage (6 per cent). Of the patients followed for 5 or more years 46 per cent remained free of tumor. Only 16 per cent of the patients had multiple tumors at presentation and 20 per cent had tumors of 10 gm. or more. These factors were associated with a worse prognosis. Patients free of tumor at 3 months had an 80 per cent chance of having no further recurrences and this rate remained the same up to 2 years from the start of the disease. Patients with a recurrence at 3 months were much less likely to remain free of tumor, and had a higher chance of recurrence at every future visit.

Bladder neck contracture following prostatectomy.

In an 11-year period, 49 patients developed bladder neck contracture after prostatectomy--an incidence of 0.86%. This complication was found to be more common after resection of small fibrous hyperplastic prostates. The best results for treatment of the contracture were obtained after bladder neck incision, which gave a 9% incidence of recurrence; after transurethral resection of the bladder neck contracture the recurrence rate was 46%, and after bladder neck dilatation it rose to 100%.

Tuberculous stricture of ureter.

Tuberculous stricture of the ureter is not uncommon in centers which treat large numbers of patients with urinary tuberculosis. Ninety-seven such strictures were seen in 92 patients over a twenty-five-year period. All patients were treated with appropriate antituberculous chemotherapy. Transurethral dilatation of the ureteral stricture was done on 80 ureters. Dilatation was successful in 51 ureters (64%) and failed to relieve the strictures in 29. Technical difficulties prevented dilatation of 17 ureters. We believe that transurethral dilatation should be attempted on all tuberculous ureteral strictures and that other procedures should be employed only if dilatation fails.

Congenital patent urachus associated with complete (hypospadiac( duplication of the urethra and solitary crossed renal ectopia.

The literature is reviewed on the incidence of patent urachus. Three cases are reported and 1 is described in detail. In that case the patent urachus was associated with complete duplication of the urethra and solitary crossed renal ectopia. The rarity of these associated lesions is stressed, especially the fact that the second urethra was situated in the hypospadiac position. Two years after excision of the patent urachus urine retention developed, which was treated by dividing the septum between the 2 urethras in the region of the prostate.

Epodyl in management of noninvasive vesical neoplasms.

A prospective study to assess the effects of triethylene glycol diglyceridyl ether (Epodyl) in the management of superficial noninvasive vesical neoplasm (T1) is presented. Apart from a few local side effects, the response of the tumor to Epodyl was very good as assessed up to three years. Further long-term studies are being done.

Prognosis in carcinoma of the urachus.

The literature regarding the prognosis of carcinoma of the urachus is reviewed. Factors affecting the prognosis are discussed. En bloc partial cystectomy, if feasible, is the form of treatment which gives the best long-term survival rate. A series of 5 cases is presented with a 13-year survival rate of 40%.
