New approach to human high-risk papillomavirus (HR-HPV) genotyping.

Human high-risk papillomaviruses (HR-HPVs) are involved in the induction of invasive cervical cancer. The aim of this study was to introduce a simple, semi-automated and reproducible approach suitable for HR-HPV detection in clinical practice. The procedure is based on DNA isolation, nested polymerase chain reaction, single strand conformational polymorphism and evaluation of HR-HPV genotypes with Gel-Pro software. The clinical performance of the new approach was assessed in two different patient materials: 1) cervical smears with cytological classification Pap2-3 or Pap3 lacking nuclear atypia (anisonucleosis and polychromasia) or koilocytotic atypia and without any previous therapy 2) formalin-fixed, paraffin-embedded cervical carcinoma and lymph node sections. Using the new approach we detected HR-HPV DNA in 64% patient samples cytologically classified as Pap2-3 or Pap3 respectively and in 80% formalin-fixed, paraffin-embedded lymph node sections histologically classified as lymph nodes without carcinoma cell infiltration. The combination of methods described in this study results in increased sensitivity of HR-HPV identification allowing detection of HPV DNA in a very small amount of target DNA so that it can be widely used in distinguishing the pre- malignant lesions and in determination of invading carcinoma cells to lymph nodes in patients with advanced cervical cancer. The new approach is useful in unambiguous HR-HPV genotyping even in double-HPV infection.

[Long-term treatment with desmopressin in children with primary nocturnal enuresis. An international multicenter study].

BACKGROUND: Desmopressin (an analogue of antidiuretic hormone) holds an important place in the treatment of primary nocturnal enuresis. According to some long-term trials its action is mainly symptomatic. The benefit of treatment and persistence of the effect in relation to the expected decline of enuresis in 15% children/1 year is discussed. METHODS AND RESULTS: The open multicentre trial lasted 42 months. In the first stage 265 patients (164 boys, 101 girls) aged 9.4 +/- 2.8 (5-18 years) were given desmopressin (nasal drops) to achieve a 4-week dry integral. Enuresis stopped in 207/265 (78.1%) children within six (median) weeks of treatment after an effective dose of 10.5 micrograms (median) based on titration. During the second stage 55/265 children (25 boys and 30 girls) proceeded with treatment for 2-30 (median 12) months, one boy did not complete the trial. An effective dose was administered for 3.5 months (median) and then the dose declined depending on the effect by 3.5 micrograms (1 drop) per months (median). In the titration stage enuresis receded in 89.1% (49/55) children. After the first year of the trial there were 72.7% responders (p < 0.001, as compared with the assumed decline), after two years 70.9% (p < 0.01) and after three years 61.1% children (p < 0.05). The trial was completed by 61.1% (33/54) children as respondents. 23 of them 17-38 months after termination of treatment. 29.6% (16/54) patients were relapsing responders on long-term treatment, 5.6% (3/54) patients completed the trial as partial responders and 3.7% (2/54) children as non-responders. Minor side-effects were recorded during the titration stage in 4.5% children, during long-term treatment 5.4% children. The osmolality of morning urine increased during treatment regardless of the final effect (p < 0.01). The authors did not find a significant relationship between age, sex, familial incidence of enuresis, period of treatment and the achieved effect. CONCLUSIONS: The authors provided evidence of a rapid onset of the effect of desmopressin and a high effectiveness throughout the trial. The osmolality of the morning urine was not a reliable predictive factor of the effect. In the authors opinion long-term treatment is important for development of regulation and regression of complaints. During a relapse the authors recommend return to maintenance treatment and gradual discontinuation after 6-12 months. Desmopressin treatment is in the authors' opinion safe, well tolerated and very useful.

Doppler flowmetry in the differentiation of biological characteristics of ovarian cancer.

On basis of ultrasonographic structural criteria and Doppler flowmetry in a group of 76 patients with ovarian cancer, benign lesions were discovered in 59 patients and malignant lesions in 17 patients (77.6% and 22.4%, respectively). Surgical intervention was indicated according to the clinical findings and following of the trend of organ-specific oncomarkers in 32 out of 76 patients (42.1%). The stratification involved 17 patients with sonomorphologically/Doppler-flowmetry - diagnosed malignancy as well as 15 patients with sonographically diagnosed benign ovarian tumor. The clinical findings were histologically verified in all 32 patients. Histology confirmed malignant tumor in 16 patients, 1 histologically borderline malignancy and 15 benign tumors. The Doppler-flowmetry showed 94.1% sensitivity, 93.3% specificity, 93.3% positive predictive value, and 93.75% accuracy.

Relevance of the Doppler flowmetry in the diagnosis of pathological alterations in the post-menopausal endometrium.

The sonopathomorphology of the menopausal endometrium was investigated by measuring the endometrium thickness using the Doppler flowmetry technique. The endometrium thickness extending five millimeters has been considered critical, indicating possible neoplastic disorders. Such clinical status has been discovered in 21 out of 58 post-menopausal patients inspected (36.2%). Furthermore, the Doppler blood-flow analysis, based on the evaluation of the resistance indices (RI) in the ascendent branches of uterine arteries bilaterally, and - in case of the vascularization analysis - in the edometrial and perimetrial regions, has been performed. These investigations brought evidence indicating full capacity of the method in detecting blood flow through the uterine arteries, while significant differences in the endometrial flow-through parameters were detected only in 14 (66.7%) patients. In all 21 patients with endometrium thickness extending 5 millimeters, bioptic examination followed by histology confirmed endometrial cancer in 9 patients, epidermoid cancer in 3 patients, and cystic hyperplasia in 3 patients, respectively. The remaining 6 patients showed either secretory or quiet endometrium (5 cases), or necrotic endometrial regions (1 case). In all 12 cases of malignant endometrial cancer and in 2 out of 3 cases of histologically verified benign hyperplasia, intra- and periendometrial vascularization has been confirmed. It is to note that the mean RI values measured in the intra- and periendometrial vessels in case of endometrial cancer were significantly lower than in patients with benign cystic hyperplasia of the endometrium. Our results brought evidence indicating that the estimation of differences in the RI values in patients with detectable intra- and periendometrial vascularization has a significant relevance in the distinction of endometrial cancer from nonmalignant endometrial lesions, predominantly the benign atypical hyperplasia. The reliability of the test was 100%, however - in case of malignant disorders - a significant decrease in the RI values has been seen in the intratumoral vs. peritumoral vascularization.

Characterization of adhesion associated surface properties of uropathogenic Escherichia coli.

Escherichia coli was isolated from the urine of patients with pyelonephritis, with urinary tract infections other than pyelonephritis and with asymptomatic bacteriuria. Surface properties of the strains were analyzed by the salting-out aggregation test (SAT), hydrophobic interaction chromatography (HIC), Congo red binding (Crb), agglutination of erythrocytes (MRHA) and latex particles covered by digalactoside (PF) and by adherence to tissue culture cells. In addition, a DNA probe for the pap gene was used. The DNA probe detected the highest proportion of strains with pap gene in the group of patients with pyelonephritis, lower in the urinary tract infections other than pyelonephritis and the lowest in the group with asymptomatic bacteriuria. Tests for P-fimbriae (PF, MRHA) showed a similar distribution. Hydrophobicity measured by SAT and by HIC did not show differences among the tested groups of strains. The results suggest that factors other than the P-fimbriae and hydrophobicity may contribute to the persistence of E. coli in the urinary tract.

[Transitory hyperphosphatasemia in childhood].

Transient hyperphosphatasaemia was detected in 11 children hospitalized at the First and Second Paediatric Clinic of the Paediatric Faculty Hospital in Bratislava in 1985-1990. The authors analyzed retrospectively 6 children from this group where it was possible to evaluate accurately the trend of serum alkaline phosphatase. In all children the high alkaline phosphatase activity was detected incidentally during the initial examination. The reason for hospitalization were in four instances respiratory infections, in one instance coeliac disease and in one instance infectious mononucleosis with infection of the urinary pathways. The maximum increase of serum alkaline phosphatase was 5-15 fold higher than the upper borderline of the reference range for the given age group. The activity declined to normal spontaneously independently on the course of the basic disease and treatment, always in the course of 3-12 weeks. The isoenzyme pattern of alkaline phosphatase with a high ratio of the thermolabile isoenzyme was typical. Finally the authors emphasize that recognition of this obscure condition which does not endanger life can spare the children many unnecessary and expensive examinations.

Dynamic 99mTc-DTPA renal gammagraphy with glomerular filtration rate determination.

Using the method of 99mTc-DTPA (diethylenetriaminepentaacetic acid), sequential renal gammagraphy with subsequent glomerular filtration rate determination by means of a gamma-camera and computer, a total of 285 patients with suspected renal and urinary tract diseases, subjects after surgery on these organs or those serving as controls after drug therapy were examined. With the help of a modified computer user's program, a study was designed to perform quantitative evaluation of some functional parameters as well as determination of total and separate glomerular filtration rate values by 99mTc-DTPA according to Gates. The benefits of the method employed in our study are that it is non-invasive, rapid, atraumatic, with low exposure to radiation while offering a broad range of applications in routine diagnostic practice of nephrology, and especially so in paediatric patients.

[Determination of glomerular filtration in children using 99mTc-DTPA and the sequential gammagraphy method].

The method of sequence scintigraphy of the kidney with 99mTc-DTPA (diethylene triamine pentacetic acid) and subsequent determination of glomerular filtration rate by means of a gamma camera and computer were applied in examining 160 children with different diseases of the kidneys and urinary tract. By modifying the commercial computer program a study with quantitative evaluation of some functional parameters was designed, involving also the determination of the overall and of separate values of GF 99mTc-DTPA according to G. Gates. The main advantages of the applied method are its noninvasiveness, speed, safety, low radiation load, and its relatively broad use in diagnostic nephrological practice particularly in children.

Simple diagnosis of diabetes insipidus and antidiuretic hormone excess.

The value of information that may be obtained by measuring osmolality of body fluids is not generally appreciated by clinicians. Osmolality determination by freezing point depression is technically simple to perform and requires only 0.2 ml fluid. The simultaneous measurement of plasma and urine osmolality may yield useful information concerning alterations in water homeostasis (diabetes insipidus and antidiuretic hormone excess) and avoid uncomfortable and sometimes hazardous investigations of patients.
