A case of Sertoli-Leydig cell tumor in a postmenopausal woman.

Sertoli-Leydig cell tumor belongs to the group of sex cord-stromal tumors of the ovary. These neoplasms account for less than 0.5% of all ovarian tumors and are more often encountered in young women between the ages of 20 and 30 years who usually become virilized. We described an unusual case of Sertoli-Leydig cell tumor in a postmenopausal women who presented with a solid right pelvic mass, a large amount of ascites, and laboratory tests revealing an elevated CA125, all suggesting a pelvic malignancy. Although five similar cases of postmenopausal women with Sertoli-Leydig cell tumor of ovary have been reported in the literature, we believe that this is an useful addition to the literature.

Neoadjuvant chemotherapy in cervical cancer: a 67 patients experience.

The purpose of this study was to evaluate the efficacy and toxicity of neoadjuvant chemotherapy in patients with locally advanced cervical cancer. Between 1992 and 2003, all consecutive women with locally advanced cervical cancer receiving neoadjuvant chemotherapy were identified. Sixty-seven patients received neoadjuvant chemotherapy: 34 had stage I disease, 28 had stage II disease, and 5 had stage III disease. Clinical response to neoadjuvant chemotherapy occurred in 61 patients, including six with complete and 55 with partial response; five women showed stable disease and one progressed. After neoadjuvant chemotherapy, 58 women underwent surgery, whereas the remaining nine received radiation. Hematologic toxicity was seen in 14 patients, with most of them consisting in severe anemia. The 5-year survival rate and median survival were 63% and 93 months. In univariate analysis, response to neoadjuvant chemotherapy, treatment after neoadjuvant chemotherapy, cervical stromal invasion >50%, and lymph node involvement were important prognostic factor responsible for survival. Neoadjuvant chemotherapy followed by surgery seems to be tolerated and active in the treatment of locally advanced cervical cancer and might be an alternative choice of therapy to chemoradiation. A prospective randomized trial with a larger number of cases is needed.

A rare case of choroidal metastasis presented after conservative management of endometrial cancer.

Hematogenous dissemination from endometrial cancer is quite rare. We report a 31-year-old woman who developed choroidal metastasis following conservative management of early-stage endometrial carcinoma. She had received kidney transplantation and was taking steroids and cyclosporine. Three years after hysterectomy for persistent endometrial carcinoma, she developed multiple metastatic disease (to both lungs and right pelvis), and while on treatment with paclitaxel and carboplatin, she complained of a rapid visual deterioration. Ophthalmologic evaluation revealed a metastatic choroidal tumor associated with multiple central nervous system metastases. The patient refused further treatment and died 1 month after diagnosis of choroidal involvement. In conclusion, this is the first reported case of choroidal metastasis from endometrial cancer and highlights the need to consider immunosuppressive treatment as an absolute contraindication to conservative fertility-sparing treatment in gynecological malignancies.

[Ultrasonic assessment of the state of the cervical canal at the term of pregnancy].

BACKGROUND: To assess if ultrasonic measurement of the cervical canal has carries, at term of pregnancy, a relationship with the onset of labour. To assess if cervical length identifies women at risk to deliver beyond 42 completed weeks of gestation (287 days). METHODS: Prospective case-control study, carried out between 37 and 40 weeks of gestation in uncomplicated, singleton, cephalic presentation, pregnant women. SETTING: outpatient ultrasound Department of a tertiary referral centre, serving > 1000 pregnancies. Weekly measurements of the cervical canal by transvaginal ultrasound probe. RESULTS: Inverse relationship between sonographic measurements and advancing gestational age. At 38 weeks of gestation, significant difference in the ultrasonic recordings between patients delivered before and beyond 41 completed weeks. CONCLUSIONS: It is possible to identify as early as 38 weeks of gestation those women that will deliver beyond 41 weeks. There is a subgroup of nulliparous patients that will probably be candidates to induction of labour.

[Clinical experience with uterine sarcoma].

BACKGROUND: To report and compare personal experience with international literature on uterine sarcomas. METHODS: 17 cases of uterine sarcomas diagnosed during seven years in our Institute, have been clinically histopathologically, prognostically and therapeutically analyzed. RESULTS: 10 cases of LMS, 6 of MMT, and 1 of ESS have been found. Women affected were mostly 42 to 84 years old. Most of the youngest patients were affected by LMS, while the oldest by MMT. Almost all of them were in menopause and pluriparae. The symptomatology consisted mainly of vaginal bleeding (58.9%). The diagnosis was preoperative for MMT and ESS; in 8 cases it was adopted for stage I, surgery and RT for stage II, surgery, RT and chemotherapy for a woman of stage III, and for the last two surgery and chemotherapy. The patients of stage III and IV survived respectively 8 and 10 months; three patients in stage I still have a 5 years survival and the other are still in follow-up. CONCLUSION: This work is a clinical contribution since the rarity of these neoplasms does not allow the standardization of a diagnostical and therapeutical iter and consequently the improvement of prognosis.
