[Vagotomy in the treatment of bleeding reflux-esophagitis and gastroduodenal bleedings].

Fifteen-year experience in the treatment of 588 patients with esophageal and gastroduodenal bleedings is analyzed. Surgery was performed in 286 patients, 302 patients were treated conservatively including 71 patients who underwent endoscopic procedures. Principles of differentiated treatment policy are developed. Selective proximal vagotomy in combination with fundoplication (if indicated, with surgeries draining the stomach) is surgery of choice in bleeding reflux-esophagitis and gastroduodenal ulcerous bleedings. These methods eliminate pathogenetic factors of bleeding gastro-esophageal reflux, lower acidity of gastric juice.

[The surgical correction of the disordered sphincteric function of the cardia].

For comparative assessment of some methods of surgical correction of the cardia the results of surgical treatment of 180 patients were analyzed. In 33 patients hiatal diaphragmatic hernia (HDH) was detected, in 84--duodenal ulcer was combined with HDH and in 63--duodenal ulcer was detected. In 15 patients operation by Hill was performed for correction of cardial function, in 9--operation by Dor was carried out, in 3--valvular gastroplication by N.N. Kanshin, in 23--esophagofundorrhaphy and in 130--modified by N. N. Kanshin and A. F. Chernousov fundoplication was performed. In 49 patients fundoplication by improved method was carried out with the use of intraoperative esophagomanometric control for adequacy of the procedure. The effectiveness of the treatment was evaluated according to the short-term and long-term results. It was thus established, that the best procedure of surgical correction of the cardia was fundoplication. Introduction of esophagomanometry during fundoplication valve formation is the most important factor contributing to adequate restoration of the cardia.

[Combined surgical treatment of duodenal ulcer and reflux esophagitis].

Under study were results of the associated surgical treatment of 349 patients with ulcer disease of the duodenum and reflux-esophagitis. The optimum operation procedure for this pathology at the present time is selective proximal vagotomy with fundoplication, while for stenosing reflux-esophagitis the preoperative bougienage of the esophagus should be used. If the bougienage is impossible, resection of the altered portion of the esophagus is necessary. In most patients such tactics gave good and excellent nearest and remote results.

[The possibilities for correcting lipid peroxidation in suppurative wounds in diabetic patients].

The authors have investigated the state of lipid peroxidation in 118 patients having purulent wounds of soft tissues against the background of diabetes mellitus. It was shown that the parenteral administration of vitamin E and the intracorporeal UVI of blood accelerated the process of stabilization of the parameters of lipid peroxidation and shortened the time of treatment. A combination of these two medical factors included in the complex of curative measures proved to be most effective.
