[Intraoperative cholangioscopy].

Intraoperative fibrocholangioscopy (FCS) was used in 446 patients with cholelithic disease and acute cholecystitis. 399 (89.5%) of the patients were hospitalized urgently. The use of FCS in patients with choledocholithiasis, obturative jaundice, cholangitis and pancreatitis is analysed. The safety and diagnostic accuracy of the method are proved. The use of FCS has provided significant improvement of the immediate and long-term results of the biliary tract surgery.

[Preoperative instrumental diagnosis of postoperative choledocholithiasis].

Experience in examination of 221 patients with postoperative cholangiolithiasis is discussed. Analysis of the results of instrumental examination ERPCG to surpass all the existing methods of diagnosis of stones in the bile ducts in informativeness and safety. A diagnostic algorithm of examination is suggested by means of which the necessary tactical problems connected with the forthcoming operation were solved in 98.2% of cases.

[The continuity between the polyclinic and the surgical hospital in rendering specialized medical care to patients with chronic calculous cholecystitis].

A thorough clinical analysis of pains in the right hypochondrium was carried out in 880 patients. There were 80 men (the mean age 45 years) and 800 women (the mean age 40 years). Three groups of the symptom complexes were distinguished, pointing to: (a) dyskinesia of the gallbladder by the hypotonic type; (b) dyskinesia of the gallbladder by the hypertonic type; (c) chronic cholecystitis. Ultrasonography of the abdominal organs in 55 persons, primarily in the third group patients revealed calculous cholecystitis which accounted for 6. 25% of the total number of the examinees. All the patients suffering from chronic calculous cholecystitis received cholecystectomy on an elective basis. It should be mentioned that preoperative examination and treatment of concomitant diseases were done on an outpatient basis, which permitted the patients' stay at the hospital to be reduced more than two-fold (to 15 days). The data obtained allowed a conclusion about the necessity of a wider use of ultrasonography of the gallbladder as a screening method to examine outpatients with pains in the right hypochondrium and of carrying out an all-round examination and treatment of concomitant diseases in patients suffering from chronic calculous cholecystitis at the prehospital stage.

[Recurrent and residual stones in the bile ducts].

Data of 205 patients with recurrent (48) and residual (157) choledocholithiases are presented. In most observations postoperative cholelithiasis proceeded by the type of chronic intermittent obturation of bile ducts. Highly effective method of diagnosing recurrent and residual calculi is thought to be the endoscopic retrograde pancreatocholangiography, best results of the treatment being obtained after the endoscopic papillosphincterotomy which should be used as the operation of choice.

[Evaluation of late results of surgical treatment of choledocho- lithiasis].

An experience with repeated examinations of 656 patients operated upon for bile calculi is presented. It was shown that the estimation of long-term results of surgical treatment of choledocholithiasis should be performed on the basis of common criteria three years after operation by complex use of clinical, laboratory, X-ray and endoscopic methods of examination. Debatable questions associated with the interpretation of special examinations and their influence on the clinical estimation of long-term results are elucidated.

[Late results of the surgical treatment of patients with biliary calculi].

The authors analyse the late results of surgical treatment of 463 patients who underwent operation for stones in the bile ducts. The late results were found to be determined by the character of the affection of the bile excreting system and the rational choice of the operative method. The indications and contraindications for various methods of treating cholelithiasis were determined more precisely from study of the late results.

[Acute cholecystitis and its complications].

The results of operative treatment of 4,011 patients with acute complications of cholecystitis are analysed. Problems of diagnosis and tactics and the results of treatment are discussed. It is pointed out that the management of patients with acute complicated cholecystitis is a great and urgent problem in abdominal surgery in view of the diffuse character of the process.

[Endoscopic papillosphincterotomy in the treatment of choledocholithiasis and its complications].

Endoscopic papillosphincterotomy was performed in 306 patients with hepatocholedochal calculi. Mechanical jaundice was found in 271 patients (88.5%), 183 patients (59.8%) had acute cholangitis, 41 patients (13.4%) had acute pancreatitis. Complete sanitation of bile ducts was successfully fulfilled in 264 patients (86.3%). Three patients had bleedings from the area of incision, 7 patients had transient diastasuria, 3 patients had edematic form of acute pancreatitis, 24 patients died (7.84%).

[Endoscopic retrograde cholangiopancreatography in the diagnosis of diseases of the organs of the hepato-pancreato-duodenal group].

Experience with using endoscopic retrograde pancreatocholangiography (ERPCG) in 278 patients has shown the method to be the most valuable one in preoperative diagnosis of diseases of bile ducts. It facilitates establishing the presence of calculi in bile ducts, diagnosis of stenosis of the major duodenal papilla and cholangitis. ERPCG allows establishing the localization of a tumor process in bile ducts. ERPCG should be performed first of all in patients with jaundice and in cases when other diagnostic methods have failed to make the correct diagnosis.

[Late results with a deep suture of the common bile duct].

Analysis of long-term results has shown that the primary suture of the common bile duct is quite a reliable method for completing choledochotomy provided it is used in case of good patency of bile eliminating ducts having no concrements.

[Diagnostic value of fibrocholedochoscopy and its role in the selection of a method of choledochotomy closing].

The authors have analyzed the experience with using fibrocholedochoscopy in 136 patients mainly with complicated forms of cholelithiasis. The endoscopic examination was found to be more effective in the revealing and better differentiation of various forms of cholangitis. It facilitated the detection of small concrements in the intrahepatic ducts and the terminal portion of the common bile duct which had not been found by other methods and helped to diagnose cicatricial strictures of the Vater's papilla. Fibrocholedochoscopy helped to choose the optimal method of finishing choledochotomy, gave considerably wider indications for tight suture of the common bile duct.
