[Large-fractional preoperative irradiation combined with organ-sparing operations in stage I and II cancer of the breast].

A method of combined organ-sparing treatment for first- or second-stage breast cancer has been developed. It includes large-fraction preoperative radiation (28.5 Gy in 5 daily fractions) and segmental mastectomy with axillary lymphadenectomy. The method has been used in 25 patients. Mean follow-up time is 23 months. No local or regional relapses have been recorded. Total relapse-free survival is 92%. The cosmetic and functional effect is satisfactory.

[Chemoradiation treatment of patients with aggressive non-Hodgkin's lymphomas in stages I to II of the disease].

Altogether 91 patients underwent treatment. Polychemotherapy (COP, ACOP) and radiotherapy were provided to 43 patients. Of these, 26 patients were irradiated locally and 17 were subjected to extensive radiation. Radiotherapy was given to 48 patients. Of these, 36 patients were subjected to irradiation of the local lesions whereas 12 underwent extensive radiation. Complete remissions were attained in 81 and 75% of the patients, respectively. The 5-year survival amounted to 65 and 34% (p less than 0.05), the 5-year survival without any relapses to 55 and 25% (p less than 0.01). In the IA stage, the 5-year survival was 89 and 44% (p less than 0.05) and the 5-year survival without relapses 63 and 13% (p less than 0.05). In the IIA stage, a statistically significant increase of the 5-year survival was also reached. No material differences could be seen in the results of the treatment depending on the scope of radiation. Drug + radiation treatment turned out most effective for the local stages of aggressive non-Hodgkin's lymphomas.

[Combined treatment of breast cancer using bremsstrahlung and electron radiation].

Combined therapy with accelerator high-energy radiations was applied to 116 patients with breast cancer, Stages II-III. At the beginning the patients received preoperative irradiation of the affected breast and ipsilateral axillary zone with braking irradiation at 25 MeV. The summary focal dose of 20 Gy was delivered with 5 daily fractions, 4 Gy each. This was followed by radical mastectomy. In the postoperative period irradiation of the supraclavicular and parasternal zones on the ipsilateral side with electron radiation at 10-22 McV was used when indicated. The summary focal dose of 44-50 Gy was provided in 22-25 fractions. Local recurrences developed in 2.6% of the cases. No regional recurrences and metastases to the supraclavicular lymph nodes were noted. No marked reactions and complications were revealed.

[Combined lung cancer and systemic scleroderma].

The article presents data of the literature and the author's personal two observations on combination of systemic scleroderma and cancer of the lungs. According to the data obtained, the most frequent forms of neoplasms in this combination were tumours of an adenomatous structure. A decisive role in the machanism of development of adenomatosis of the lungs was played by septo-alveolar sclerosis which led to rearrangement of the lung tissue. Potential malignancy of adenomatosis of the lungs in systemic sclerodermia was closely connected with participation of a "defective" collagen of the embryonic type in the formation of the basal membrane of the alveolar epithelium, as well as with the predominance among the tumour cells of granular pneumocytes of the B type characteristic of the embryonal period of mammals.

[The radiation and chemoradiation treatments of generalized low-grade non-Hodgkin's lymphomas. 1. The immediate results].

The remission rates were analysed during radiation and chemoradiation treatments and chemotherapy of 255 patients with low-grade generalized non-Hodgkin's lymphomas. Complete remission was achieved in 15.3% of patients, partial remission was in 82.4%. No effect was seen in 2.3% of patients. The rate of complete remissions was 4.2% for radiation treatment, 29.5% for chemoradiation, and 10.6% for chemotherapy. Chemoradiation treatment regimen was the most effective for this group of patients, in all morphological types and chiefly in Stage III. The intensified chemoradiation regimen improves the immediate outcome of treatment; a combination of series polychemotherapy with radiation applied to all diseased lymphatic collectors in a cumulative radiation dose of 30-40 Gy. The effect of the chemoradiation regimen is virtually completely revealed in the combination of three series of polychemotherapy and radiation therapy; the continued chemotherapy does not increase the rate of complete remissions.

[The radiation and chemoradiation treatments of generalized low-grade non-Hodgkin's lymphomas. 2. The late results].

Late outcomes of radiation and chemoradiation treatments and chemotherapy were analysed in 255 patients with low-grade generalized non-Hodgkin's lymphomas (NHL). In terms of total and relapse-free survival, chemoradiation treatment regimen yields the best results, as compared to chemotherapy. There is a general trend to improve the late outcomes when each of two components of the chemoradiation regimen is intensified. Chemoradiation treatment improves late outcomes not only in primary patients, but in patients with relapses after radiation or chemical therapy. Complete remissions are one of the most important predictors of total and relapse-free survival. There is a small group of patients with a diffuse lymphocytic type of NHL (about 10-15%). which is characterized by extremely slow disease progression and enlargement of individual lymph organs (tonsils, lymph nodes, spleen) in whom long-term total and relapse-free survival can be achieved by local radiation therapy.
