[Endoscopic occlusion of the bronchial stump in bronchial insufficiency].

The results of endoscopic occlusion of the stump of the main bronchus in its insufficiency were studied in follow-up periods of up to 3 years in 27 patients. The indications for the method were developed and the techniques of the occlusion described. Endoscopic occlusion is most expedient and sparing in early insufficiency of the bronchial stump under conditions of acute pyothorax and the presence of complications in a solitary lung. Its application is not opposed to the other commonly used methods of treating bronchial fistulas after pulmonectomy.

[Subtotal esophagectomy with simultaneous retromediastinal plastic surgery using gastric pedicled flap without thoracotomy].

Thirty-three operations for subtotal esophagectomy with one-stage plastics with a gastric pedicle without thoracotomy were carried out between 1985 and 1988. The indications for the operation were as follows: carcinoma of the esophagus (17) and of cardioesophageal localization (7), cicatricial stricture of the esophagus (6), IV degree cardiospasm (2), unspecific esophageal ulcer (1). The esophagus was resected through a laparotomo-transdiaphragmatic-cervical access, the graft formed from the greater curvature of the stomach was passed in the posterior mediastinum with the establishment of a cervical esophagogastroanastomosis. Postoperative complications occurred in 29 patients: incompetence of the anastomosis (26), mediastinitis and pyothorax, (4), peritonitis (2), pneumonia (4). Six patients died. With the performance of intrapleural esophagogastroplasty the mortality rate fell from 25 to 18.2%. The authors claim that subtotal esophagectomy with posteromediastinal gastroplasty without thoracotomy is a less traumatic and safer operative intervention.

[Interaction of the plasma membranes of the cells of tumors metastasizing to the lungs with the target organ].

Cells of lines of mice transplantable tumours metastasizing into the lung have been used: lung adenocarcinoma (AL), Lewis lung carcinoma (LL), melanoma B-16 (B-16), mammary tumour MMT-1 (MMT-1), malignant subline of L-cells (LS). Hybrid vesicles were obtained for each tumour. They contain fragments of cell plasmatic membranes (PM). It has been shown that AL-, LL-, LS- and B-16-liposomes were accumulated in lung, the trapping of AL- and LL-liposomes being higher than that of the other hybrid vesicles. Despite the similar dynamics and frequency of metastatic spreading into the lung for all tumours studied, no trapping of MMT-1-vesicles in the target-organ was observed. The role of specific interaction of tumour cells PM with the endothelium of the lung capillaries in the process of organotropic metastatic spreading is discussed.

[Ultrasonic diagnosis of benign focal lesions of the liver].

Proceeding from a study of an ultrasound picture of liver focal changes of benign nature in 60 patients a conclusion was made that nonparasitic cysts could be verified on the basis of a typical ultrasound picture and its stability over time. The presence of daughter cysts and septal membranes was indicative of unilocular hydatid disease. Additional laboratory investigations were necessary for the recognition of alveolar hydatid disease. It was desirable that the diagnosis of liver abscess and hematoma should be confirmed by fine needle spot puncture. By their ultrasound picture benign liver tumors did not differ significantly from malignant ones. To exclude the latter fine needle spot puncture was necessary. Hemangioma should be verified by angiography, the rest of benign tumors--by histology.

[Odontogenic mediastinitis].

An experience with the surgical treatment of 28 patients with odontogenous mediastinitis is generalized, 12 of them died. The clinical picture is described in detail. Active surgical tactics is given the decisive role in the improvement of results of the treatment.

[Thoracoscopy in nonspecific spontaneous and iatrogenic pneumothorax].

Thoracoscopy was used in 92 patients. The etiology of pneumothorax was established in 76 patients. In most of them the treatment was effective. Suture of the pleura and lung defect, pleurodesis, pleurectomy, coagulation of tissues were performed under the control of thoracoscope. Thoracoscopy allowed to avoid extensive thoracotomy, to shorten the period of treatment at the hospital.

[Surgical interventions in abdomino-mediastinal lipomas].

The leading role in the recognition of abdomino-mediastinal lipomas is given to roentgenological examinations. Great significance is attached to thoracoscopy. Operations are fulfilled by the transpleural or transrectal accesses. The latter is the method of choice and must be accompanied by active drainage of the bed of the removed tissues during a day.

[Results of a microscopic study of several remains of the internal organs of Shandrin's mammoth].

The survived portion of an organ which topographically can be considered as a part of the lung, as well as a section of muscular tissue, perhaps that of the heart, has been studied. Under weak systems, the portion resembles the lung, spaces resembling alveoles, alveolar ducts and alveolar saccules are seen. "Alveolar" walls are stained dark grey by different methods; "alveoles" contain yellow granules, ferric components are not detected in them. The intestinal wall and that of the large vessel in the mesenteric cavity preserves a fibrillar carcass, fibrillar ultrastructure corresponding to collagen.

[Experimental inflammation of placenta in sheep].

The sheep desmochorionic placenta has been studied at aseptic inflammation produced by suturing it together with the uterus. The inflammatory process is evident from a decreased intensity of leucocytic phase and trophoblast participation in macrophagal phase; cytotrophoblastic elements and diplocariocytes transform into symplastic structures which seem to accomplish dendritic lysis and resorption. The foreign body incapsulation is performed with the maternal and fetal connective tissue participation.

[Effect of sodium aurothiomalate in acute toxic hepatitis].

The effect of lysosomotropic agent--sodium aurothiomalate on liver damage and lysosomal changes during acute toxic hepatitis was studied. Combined administration of sodium aurothiomalete led to less extensive necrosis and to widening dystrophic areas in the hepatic parenchyma. Solubilization of the acid RNA-ase activity was decreased, and nonsedimentable activities of beta-galactosidase, cathepsin D were the same as in acute CCl4-hepatitis.

[Human placental morphology as an index of features of maternal-fetal relationships].

Principle possibilities of the complex evaluation of the mother-fetus exchange were studied by means of morphological investigations of the human placenta in normality, in vesicular mole, late toxicoses, hemolytic disease of newborns and malformations of the fetus. The investigation of the ratio of the villi of different size, deposition of fibrinoid on their surface, the change of the amount of cytotrophoblastic elements, cells of Kaschenko-Hofbauer and the polymerity of the basic substance are of diagnostic value. The authors propose the basal lamina of the placenta to be included into the scheme of the examination of placenta.
