[Three cases of vaginal adenosis after topical 5-fluorouracil therapy for vaginal HPV-associated lesions].

Three cases of vaginal adenosis after topical 5-fluorouracil therapy for vaginal HPV-associated lesions are reported. Three patients with colposcopic, histologic and viral evidences for subclinical papillomavirus infection (in combination with low-grade vaginal intraepithelial neoplasia in of them) are treated with 5-FU. In follow-up control examinations persistent ulcerations were found without regression after applied therapy. By colposcopic and histologic examinations vaginal adenosis was proved without histories of intrautering DES exposure. After the destructive therapy the reported lesions were regressed without appearance of new lesions in follow-up control examinations. The application of 5-fluorouracil has to be used only in cases of recurrent vaginal warts and in cases of high-grade vaginal intraepithelial neoplasia with strict folow-up cytological and colposcopic control examinations.

[Implementation of Reid's colposcopic index in teenagers].

UNLABELLED: The aim of the present investigation was to determine the possibilities of the Reid's colposcopic index to diffentiate the grade of the findings in sexually active teenagers. MATERIAL AND METHODS: The study was based on 83 girls without child-birth in history, aged between 14 an 19 (mean age 18.13 years)q with different grade cytological and/or colposcopic atypia. Colposcopic findings were joined, according to semiobjective Reid's index and the results achieved were compared with the histological findings from the target biopsies. RESULTS AND DISCUSSION: The results from the colposcopic investigation were divided into 4 groups: in group 0--without pathological findings were classified 12 cases, in gr. 1 (Reid)--42, in gr. 2 (Reid)--24 and in gr. 3 (Reid)--5 patients. Histological equivalents were summarized as follow: normal and papiloma epithelium--in 27, CIN I--in 36, CIN II--15q and CIN III--5 cases. CONCLUSIONS: The investigation performed, shows the good correlation (91.6%) between colposcopic impression and histological diagnosis, and this put the question about the place and role of the Reid's colposcopic index in predicting the severity of the pathological changes of the uterine cervix in sexually active adolescent girls.

[Immunohistochemical studies of the pancreas in newborns with diabetic fetopathy].

The number and the size of the Langerhans islets in the pancreata of most of investigated cases with diabetic foetopathy is increased (polynesia and macronesia). The B endocrine cells are immunoreactive for both insulin and IAPP, the reaction for IAPP being weaker in comparison with the controls. In one of the cases (the mother with long history of treated type 1 diabetes, who died during delivery) no presence of insulin-immunoreactive of PAF positive B cells is discovered. The reactivity for glucagon and somatostatin in the pancreata with diabetic foetopathy is found to be similar to the controls. In the vicinity of some islets lymphoid cell infiltrations are observed.

[A parallel comparison between cytological and cervicographic screening in early pregnancy].

The aim of this study is to compare the diagnostic possibilities of cytologic and cervicographic screening of the cervix uterus in patients with early pregnancy. Fifty-four patients between the ages of 20 and 37 were examined cytologically and cervicographically in a prospective screening study. The dependability of each of the diagnostic tests was measured histologically (with the patient's approval) using punch biopsy under colposcopic control. The histologic results showed the presence of CIN in 9 women (16.7%), 8 of them with CIN I and one with CIN III. The rest of examined patients (45) had normal of various inflammatory findings. The analysis of the obtained results confirm a statistically significant difference between the two diagnostic, screening methods. The cervicographic examination is considerably more sensitive in finding CIN-44.4% compared to 22.2% of the cytology. Whereas the cytological method has a higher specificity as a test in determining the healthy patients-91.1% versus 51.2% of the cervicography. 12 or 22.2% of the cervicograms were technically defective.

[The colposcopic, cytological and histological changes in the cervix uteri of women under 20 with a first pregnancy].

The study includes 42 pregnant women chosen at random under the age of 20, in their first trimester presented for voluntary termination of pregnancy. Colposcopic, cytologic and histological test was performed. Chronic, often acute, non-specific inflammatory changes were observed, probably connected with early sex life. The establishing of a histologically atypical results in 33.3% of the cases as well as the observation of 21.7% falsely negative results from the colposcopic study support the theory of pregnancy as a factor causing complication for diagnosis. The relatively low sensitivity of the cytology method and 16.7% false negative results none the less the existence of high specificity (100%) is due to coexistence of low-stage dysplasia (I and II stage) and chronic non-specific inflammatory processes and the early, pilot character of the research.

[Pregnancy in a rudimentary horn of the uterus].

The cases of pregnancy in the rudimentary horn are probably the rarest Mullerian anomalies. More often the pregnancy ends with an abortion in the first or the end of the second trimester. Extremely rarely (1% of the cases) end with a live birth. The authors examine and describe a case which successfully ended with a birth of a live fetus in 32 gestation week after a rupture of cornual pregnancy. Special attention is paid to the diagnostical difficulties, which are characteristic for these cases. It is reasonable in the cases of extrauterine pregnancy to think of a pregnancy in the rudimentary horn as well.

[The demonstration of HPV DNA in cervical intraepithelial neoplasms and in invasive carcinomas of the cervix uteri by DNA-DNA in situ hybridization].

Authors were applied a method of DNA-DNA in situ hybridization for demonstration of HPV 6, 11, 16 and 18 in cervical neoplasia and in invasion carcinoma of uterine neck. At 22 probes from CYV III and invasion carcinoma the results from hybridization with virus DNA were shown the presence of HPV-DNA at 15 patients. Some phases of method were optimized. Sensitiveness of method was controlled and compared with the results from the same probes, underlied to investigation with Southern-blot hybridization.

Detection of human papillomavirus DNA in gynaecological swabs by filter in situ hybridization.

Gynaecological smears from the endo- and ectocervix of women with and without cytological and colposcopic abnormalities of the epithelium were investigated for human papillomavirus (HPV) types 6, 11, 16, and 18 by filter in situ hybridization (FISH). The data were compared with cytological, colposcopic, and histological findings. Of the 266 gynaecological smears, HPV DNA was detected in 84 (32%); of 101 cytologically and colposcopically HPV negative cases, HPV DNA was found in 10%. Of 56 women, cytologically and colposcopically positive for HPV infection, HPV DNA was detected in 68%. The sensitivity of the method was controlled by comparing the results of FISH with those of Southern-blot analysis of five cervical tumour biopsies. The data presented demonstrate the necessity of FISH for identification of the HPV type that might be of prognostic value in cervical pathology. Cytological and colposcopic positivity is a reliable sign in about 70% of the cases where HPV infection was proved by FISH.

[A comparison of cytological and histological studies on women with a colposcopically atypical cervix uteri].

The study was made on 62 at random chosen women between 16 and 71 years of age with atypical colposcopic findings, on whom cytological and histological studies were performed. Histological atypical epithelium was found in 36 patients. 3 out of 36 women had invasive carcinoma, 8--carcinoma in situ, but dysplasia of I to III degree was established in 25 women. The comparison of cytological and histological findings show a high degree of correlation.

[The diagnosis of human papillomaviruses in cervical smears by deoxyribonucleic acid hybridization].

The method of filter in situ hybridization is applied for diagnostics of human papilloma viruses of the types 11, 16 and 18 in cervical smears [correction of frictions]. The results of the hybridization of 30 cell materials with clonal virus DNA probes proved the existence of human papilloma viruses in 15 patients. The precision of the filter in situ hybridization is demonstrated by comparing the results of the cytological study to the existence of HPV DNA in the same materials proved by in situ DNA hybridization.

[The demonstration of human papillomavirus DNA in genital lesions with and without colposcopic atypism].

34 cellular suspensions from the vaginal part of women were examined for presence of HPV DNA of the types: 6, 11, 16 and 18--from a lesion and a healthy area--by filter in situ hybridization. The women were examined by colposcopy because of cervical lesions established macroscopically as well as for other various complaints. Cells were also investigated from women after undergone ablation treatment: diathermy coagulation, conization and hysterectomy on account of CIN. The data were compared with cytological, colposcopic and histological findings. The established presence of HPV DNA of high risk type 16 and 18 in some of the patients after operative treatment required continuous control of these women as well as examination of lower genital tract because of the discovered multicentricity of HPV infection.

[DNA in koilocytotic dysplasia of the cervix uteri, cytophotometric studies].

In koilocytotic dysplasia of the uterine cervix the DNA content in squamous cells was quantitated by cytophotometry in histological preparations stained according to Feulgen. Three patterns of DNA distribution in the squamous cells were found. In type one (21.4%) the cells had DNA content in the diploid and paradiploid zone of the histogram. In type two (35.7%) cells with triploid and tetraploid DNA values were found, but with a conspicuous modal class of cells. In type three no modal class cells were found (42.9%). The quantitative DNA changes in squamous cells show that some of the HPV induced alterations may be regarded as precancerous.
