Invasive meningococcal disease and latex agglutination test--is it still beneficial for diagnosis?

We showed current clinical usefulness of the latex agglutination (LA) test for confirmation of meningococcal etiology on 32 cerebrospinal fluid, 77 serum and 93 urine samples collected during the first week of hospitalization from 19 patients with laboratory confirmed invasive meningococcal disease. The positivity of the LA test in cerebrospinal fluid was 47%, in serum 42% and in urine 24%, while the PCR of cerebrospinal fluid and serum was positive in 95 and 47% cases, respectively. The latest positivity of the LA test was detected on day 2 in cerebrospinal fluid, on day 3 in serum and on day 4 in urine. In the group of patients who had received antibiotic therapy we found nonsignificant reduction of LA positivity and also statistically significant reduction of culture positivity in CSF (p = 0.04); the PCR positivity changed minimally. In blood samples, nonsignificant reduction of culture positivity and no difference in LA and PCR positivity was found. We did not find any statistically significant relationship between test results and clinical forms. The LA test can be therefore considered to be an auxiliary diagnostic method, rapid and easily practicable but less sensitive than PCR. It can be recommended especially for local laboratories where PCR is not available and the patient already received antibiotics before admission to the hospital.

PCR in lyme neuroborreliosis: a prospective study.

OBJECTIVES: DNA proof is the only widely available direct diagnostic tool in Lyme borreliosis. Sensitive PCR detecting of spirochetal DNA was prepared and a prospective study in neuroborreliosis was performed. MATERIALS AND METHODS: 57 hospitalised patients with active neuroborreliosis and proved CSF antibodies synthesis were examined. Nested-PCR (utilizing three targets) was used for the detection of specific DNA in plasma, CSF and urine. RESULTS: Before treatment 36 positive patients (63.1%) were found in all tested specimens in parallel, 28 patients (49.1%) were positive in urine, 20 in CSF (35.0%) and 16 in plasma 28.0%). Later only urine was tested and the following results were obtained: 17 positive patients (30.0%) immediately after treatment, 8 (14.0%) after 3 months and one patient persisted positivity after 6 months. CONCLUSIONS: The highest sensitivity of PCR was achieved in the acute period of neuroborreliosis - 63.1% in three body fluids comparing with CSF antibody synthesis.

[Evaluation of methods of trace analysis of various opiates including hydrocodone and hydromorphone in the blood and urine using gas chromatography-mass spectrometry].

A GC-MS method suitable to conduct practical trace toxicological analyses of various opiates including hydrocodone or hydromorphone is presented both in scan and selected ion monitoring modes. Silylation can be a convenient reaction to derivatize unknown analytes in toxicological samples. However, silylation of drugs with tautomeric keto/enol groups in molecular structure can cause analytical problems due to poor reproducibility of conversion degree. After using N-Methyl-N-trimethyl-silyltrifluoracetamide (MSTFA) acceptable results were achieved.

[Sensitivity of GC-MS in the detection of benzodiazepines in the urine in the form of trimethylsilyl derivatives].

A sensitive evidence of trace concentrations of benzodiazepines and their metabolites in urine can be enabled after special sample preparation including enzymatic hydrolysis, special solid phase extraction, silylation and following analysis by gas chromatography with mass spectrometry in electron impact mode. After optimalization of the procedure the extraction recovery values in the range 50-85% are achieved. The scale of retention times with basic mass spectral data are presented for the spectrum of silylated benzodiazepines which can overlap some gaps in the standard toxicological literature up to now available.

Fatal opiates overdose. Toxicological identification of various metabolites in a blood sample by GC-MS after silylation.

A fatal opiates overdose, where ethylmorphine, hydrocodone, dihydrocodeine and codeine were consumed concomitantly, is reported. This case report may contribute to data on fatal blood concentrations of drugs with rare incidence. The relative retention times in capillary gas chromatography and full mass spectra of various opiates in their silylated forms, detected together in one sample, may serve as a helpful analytical reference for clinical and forensic toxicologists.

[Detection of opiates in the urine using thin-layer chromatography].

The determination of opiates in urine by thin layer chromatography may be difficult for the low concentrations of opiates and their mixtures. The result of the analysis is influenced by the isolation recovery, sensitivity of detection and by the choice of the chromatographic conditions for the separation of opiate mixtures. The study of these factors resulted in several modifications of routinely used method, so that the results are reliable for the concentration of opiates in urine 1 mg/l. Nevertheless, for the analysis of opiates in drug abuse control the comparison of thin layer chromatography findings with other chromatographic methods or immunoassays is important for the improvement of the laboratory performance and for the interpretation of the results.

[Detection of the metabolite of delta-9-tetrahydrocannabinol in urine by means of GC/MS and interpretation of analytical findings].

Delta-9-tetrahydrocannabinol is one of many components of plants of Cannabis. In the human body it is susceptible to oxidation and its metabolites together with metabolites of other cannabinoids are excreted into urine and faeces. In this paper there is described our experience with the specific confirmation of the metabolite of the main psychoactive substance, 11-nor-delta-9-tetrahydrocannabinol-9-carboxylic acid, using gas chromatography with mass spectrometry after urine alkaline hydrolisis, solid phase extraction and silylation. The preliminary immunochemical screening of cannabinoids in urine is performed by EMIT method.

[A sensitive method for detection of sympathomimetic amines in pentafluorobenzamide form in biological samples using gas chromatography].

A toxicological method of sensitive and specific confirmation of methamphetamine and other primary and secondary amines in biological samples after extractive perfluorobenzoylation is described. The method is based on the principle of gas chromatography with parallel specific nitrogen detection and electron capture detection. The other alternative is the gas chromatography combined with mass spectrometry in chemical ionization mode. The method described allows the detection of methamphetamine and amphetamine in urine in concentrations below 10 ng/ml.

[Kaposi's sarcoma in a patient with developed AIDS].

The authors give an account on the first case of Kaposi sarcoma in a patient with developed AIDS in CSFR. They describe the case-history, clinical course and histopathological finding.

[A minor epidemic of pertussis cough in Brno].

In the course of six months the authors investigated 32 children aged 1-32 months with pertussoid cough. This number included one child where pertussis was confirmed in a 25-month-old properly vaccinated child and parapertussis in an 8-month-old incompletely vaccinated infant. In the remaining children by cultivation or serological examination a different aetiology was found (RS virus, adenovirus, parainfluenza 2, M. pneumoniae, H. influenzae, Branhamella catarrhalis).
