Isolated aortic valve replacement. A six-year follow-up.

A selected group of 259 patients who underwent aortic valve replacement was analyzed retrospectively for early and late mortality and late valve failures up to six years following operation. In this group, 100 patients received aortic valve homografts, 100 pulmonary valve autografts and 59 Starr-Edwards prosthetic valves. The two tissue valve groups, composed of younger, healthier individuals, had a lower early mortality rate than did the group receiving prostheses; mortality decreased even further with additional surgical experience. The homograft group also showed a lower long-term mortality and complication rate. The incidence and significance of the appearance of a diastolic murmur in the tissue valve group were also studied and seemed to play a major prognostic role for these patients.

Left atrial myxoma simulating a systemic disease with pleural-pericardial effusion. Detection by two-dimensional echocardiography.

Clinical symptoms of cardiac myxomas are often very confusing. We observed a 46-year-old woman with a left atrial myxoma simulating systemic disease with pleural-pericardial effusion. We think that like other systemic symptoms effusion (which to our knowledge has not been previously described in literature) can be attributed to the result of tumor secretion or necrosis. In fact, soon after surgical removal of the tumor, all pathological signs disappeared.

Negligible hemostatic toxicity of intermediate-dose Erwinase in adult patients with acute lymphoblastic leukemia: preliminary data.

The hemostatic toxicity of low dose L-asparaginase from Erwinia carotovora (Erwinase) has been reported to be negligible in adult patients with acute lymphoblastic leukemia (ALL); conversely, no consistent data have been obtained when Erwinase is administered at intermediate doses. We report preliminary clinical and laboratory hemostatic data from 10 adult patients with ALL treated during induction phase with intermediate doses of Erwinase (20,000 IU/m2s.c. every other day, for a total of six administrations). No thrombotic or hemorrhagic events were registered and the mean values of PT, aPTT, fibrinogen, antithrombin and D-dimer did not change during treatment. Only one patient showed a decrease of antithrombin (48% on day 8) requiring temporary suspension of Erwinase therapy. These data suggest that intermediate doses of Erwinase also have negligible hemostatic toxicity in adult patients with ALL.
