[The cell immunity parameters in patients with acute coronary syndrome].

The subjects of the study were 75 patients with instable angina (IA) and 16 patients with chronic coronary heart disease (CHD) following transcutaneous transluminal balloon angioplasty (TTBA). Their examination included the evaluation of the changes of some cell immunity parameters, such as the levels of interleukins IL-1, IL-6, and IL-10, tumor necrosis factor alpha (TNFalpha), and serum neopterin (NEOP), the measurement of C-reactive protein level, the detection of certain viral agents in blood (herpes simplex virus (HSV), cytomegalovirus, Chlamydia pneuoniae) using polymerase chain reaction (PCR), The study revealed such signs of cell immunity disbalance as the pronounced activation of pro-inflammatory factors (IL-6, TNFalpha, and serum NEOP). Along with no changes of the activity of anti-inflammatory factors (IL-10), the research revealed the similar direction of immune shifts in IA patients and CHD patients who had no clinical signs of exacerbation and had undergone TTBA. Most patients displayed signs of persistent viral infection, which was HSV in the majority of cases.

[A case of primary amyloidosis].

Diagnostics of primary (AL) amyloidosis is difficult enough; treatment of this disease in not less difficult or more adequate. Because of similarity of the pathogenesis of AL-amyloidosis and that of multiple myeloma, similar therapeutic regimens, directed towards depression of plasma cell dyscrasia, are used in both cases: administration of melphalan in various doses together with prednisolone, administration of vincristine, adriablastine and dexamethasone, as well as high-dose chemotherapy with melphalan and autologic stem cell transplantation. This therapeutic approach makes it possible to reach clinico-laboratory remission and prolong the life of patients with AL-amyloidosis. The article contains a case description of a patient with AL-amyloidosis, who underwent a successful high-dose melphalan therapy with subsequent autologic stem cell transplantation.

[The main reasons for late diagnosis and errors in the early period of treatment of intrahospital myocardial infarction in a general hospital].

The purpose of the study was to investigate the reasons for late diagnosis and late transfer of hospitalized patients with myocardial infarction (MI) to a specialized cardiological department. The study shows that the main reasons are an atypical clinical picture of MI and a low "infarct alert" of doctors of different specialties. The authors analyzed the fullness of urgent medical aid and the beginning of treatment in cases of intrahospital MI in patients being treated in different hospital departments. The analysis revealed the main objective and subjective factors that cause a low frequency of administration of thrombolytics, anticoagulants, and disaggregants.

[The predictors of ischemic stroke in cerebral atherosclerosis and ischemic heart disease].

The complex medical examination was performed in 145 patients admitted to the neurological department with initial diagnosis: "ischemic stroke". We compared anamnesis, clinical symptoms, laboratory and instrumental findings in the patients with verified ischemic stroke and in those without stroke. The significant clinical predictors of ischemic stroke were the following: arterial hypertension (> 165/ 95 mm Hg) age 64 years and older, circulatory insufficiency, diabetes, atrial fibrillation, lower extremity obliterating atherosclerosis, pneumosclerosis and pulmonary emphizema.

[A revised classification of hypoxic conditions].

The precised classification of circulatory hypoxia described in the paper is principally different from those published earlier by that it discriminates between hypoxia preconditioned by impaired cardiac-muscle constriction, on the one hand, and hypoxia due to malfunctions of smooth muscles (SM), on the other hand. The genesis of SM malfunctions is predetermined by impaired interactions of serotonin with SM serotonin receptors. The clinical use of serotonin adipinat reduces the local organic and total-tissue hypoxia in different pathologies and provides for better treatment results. The mentioned precised classification can be used to gain more data on the tissue-hypoxia pathogenesis and to schedule the clinical-and-experimental research on the purpose-oriented basis.

[Prediction of brain infarction in hypertensive patients].

A 7-year prospective study of 379 hypertensive patients was conducted with primary comprehensive clinical and device examination of the patients in hospital followed by annual control in hospital or outpatient setting. Two groups were formed: with uncomplicated essential hypertension (EH)(n = 263, group 1); with EH complicated by brain infarction (BI) (n = 116, group 2). Overall prognostic correctness was 85.9%. BI was predicted by age, smoking, high intake of salt, absence of regular antihypertensive therapy, non-dipper pattern, low output, atherosclerotic changes of major head arteries, frequent paroxysms of cardiac fibrillation, ventricular extrasystole, abnormal blood rheology. Integral index of the patient condition calculated on the basis of the decisive prognosis rule (data of the primary examination + MI within 7-year follow-up) enables targeted prevention of cerebral complications.

[Peculiarities of nervous system lesions in patients with multiple myeloma].

Multiple myeloma (MM) is B-cellular lymphoproliferative disease. We studied 174 patients aged 33-84 with established diagnosis of MM. Among the persons included into analyzed cohort the data about the presence of polyneuropathy at the moment of MM verification were available in 72 (41.4%) persons. Spinal cord involvement due to the various pathogenetic reasons was revealed in 24 (13.8 +/- 2.6%) patients. Compressional alteration was observed in 19 cases and in 5 patients the spinal cord involvement had uncompressional genesis. The connection between the peripheral polyneuropathy and the unfavourable prognosis in MM patients was statistically insignificant. In the patients with spinal cord prelum the probability of lethal outcome within 1 month was 0.26. It is 10 times more than in patients suffering only from peripheral polyneuropathy. Thus polyneuropathy in MM patients occurs in 41.4 +/- 3.7% cases and more often during the III stage of disease. There is a distinct interrelation between the nervous trunk involvement and disease severity. In the structure of polyneuropathy the sensorimotor variant (76.4 +/- 5.0%) prevails. According to our data the spinal cord involvement occurs in 13.8 +/- 2.6% cases.
