[Cause of failure in the surgery of renovascular hypertension].

The experience of 193 operations on 172 patients with affected renal arteries is described. The causes of failure in the surgery are analyzed. Organ-preserving operations with reconstruction of 189 renal arteries were performed on 136 patients. Primary nephrectomies were performed on 36 patients. During recent 3 years nephrectomies were made in 8% of 70 operations. Postoperative mortality was about 7%, mainly due to atherosclerotic patients. The main causes of uneffective operations are as follows: technical errors in revascularization of the kidney, pyelonephritis and neuroangiosclerosis non-diagnosed preoperatively, progressing lesions of the vessels in atherosclerosis and panarteritis.

[Characteristics of clinical picture, diagnosis and surgical treatment of hypoplasia of the thoracoabdominal portion of the aorta].

The experience with diagnosis and surgical treatment of 10 patients with a rare disease--hypoplasia is set forth in the article. According to the author's data this disease is always complicated with aortoarteritis with the involvement of the renal and visceral arteries. It is suggested to subdivide hypoplasia into several types: infrarenal hypoplasia, interrenal and supradiaphragmatic ones; hypoplasia with preseved orthograde blood flow and without the latter, with unilateral and bilateral involvement of renal arteries. 8 patients were operated upon. Errors of surgical tactics made when operating the first 3 patients are discussed. The other 5 patients showed a good effect of the operation. There were no fatal outcomes.

[Results of the surgical treatment of hypertension due to a bilateral lesion of the renal arteries].

The paper analyzes experience of the surgical treatment of bilateral lesions of the renal arteries in 52 patients who were subjected to 64 operations. The authors have shown that lesions of the both renal arteries in patients with renovascular hypertension can be observed in 28,9% of cases. In most cases atherosclerosis and aorto-arteritis are responsible for such lesions. The authors support the two-step operations in bilateral lesions of the renal arteries. Late results of such operations are favourable in 82% of cases.
