The antigenic properties of 15 strains of hepesvirus isolated from herpetic lesions of different localization were studied. The serotype of each strain was established by the neutralization tests with hyperimmune sera to the reference type 1 and 2 strains: the cross-neutralization tests revealed antigenic relationships between the strains of 1 and 2 serotypes. Most strains isolated from lesions of the genitalia belonged to type 2, although one strain isolated in herpes genitalis belonged to type 1, and 2 strains isolated from facial lesions also belonged to type 2. These results confirm the necessity of using inactivated polyvaccine prepared from herpesvirus strains of antigenic types 1 and 2 for relapse control therapy instead of using monovalent type 1 and type 2 vaccines as reported by foreign authors.

The use of the combination of 5-iodo-2-dioxyuridine (IUDR) and 6-azauridine drugs differing in their mechanisms of action in an experimental herpetic infection demonstrated its definite antiviral activity, not only not inferior to the effect of each of the drugs alone, but also remaining sufficiently high when the concentration of IUDR was decreased 2-fold. In clinical trials in relapsing genital herpes all the drugs under study showed statistically significant therapeutic activity. The combined use of the drugs exerted a certain synergistic effect.
Fifty-six patients with recurrent genital herpes (RGH) and 53 normal subjects were examined. No interferon was found in the blood sera of the patients either in the period of the disease relapse or during remission. The leukocyte capacity to produce interferon in vitro (leukocyte interferon reaction, LIR) in the patients was found to be 4-5 times lower than in normal subjects. Study of the normal killer activity of lymphocytes in patients with RGH as compared with that in normal subjects revealed its decrease in 37.5% of the patients and only in 5.1% of normal subjects. Interferon therapy with purified human leukocyte interferon given to 36 patients with RGH resulted in clinical improvement in 88.9% of the patients that was accompanied by increasing normal killer activity of lymphocytes. Vaccine therapy given to 35 patients resulted in a stable clinical effect in 91.4% of the patients, however, without activation of LIR in them. It is concluded that the therapeutic effects of interferon therapy and vaccine therapy have different mechanisms.
In patients with recurrent genital herpes in the active stage of herpetic infection, herpes simplex virus (HSV) is located both extra- and intracellularly in keratinocytes of the pricky and glandular layers of the epidermis as well as in macrophages and neutrophilic leukocytes. The observed incompleted phagocytosis of HSV by macrophages may be one of the causes of insufficient local resistance to the virus.
The therapeutic effectiveness of levamisole, megasine, and combination of these drugs was studied in 74 patients with genital herpes. Interferon production was first established in the patients treated with levamisole-megasine combination. A direct correlation was established between the therapeutic effectiveness of the drugs and interferon production. Most patients showed varying degrees of the positive effect in treatment with these drugs without any marked side effects. Recurrent genital herpes must be treated with levamisole and megasine for long periods of time.
