Five patients with a recently described novel leukoencephalopathy with brainstem and spinal cord involvement and elevated lactate.

Recently, a novel leukoencephalopathy syndrome was described in eight patients with a distinct pattern of MRI abnormalities. Here we describe the clinical, laboratory, and MRI findings in five new, unrelated patients. The clinical picture was homogeneous with onset in childhood, a slowly progressive course, variable mental deficits, signs of pyramidal and cerebellar dysfunction and sometimes dorsal column dysfunction. In two patients, a minor head trauma was followed by neurological deterioration and fever. No underlying metabolic defect was found. In two patients serum lactate was elevated, but no evidence of a mitochondrial defect was found. MRI showed variably extensive, diffuse, or spotty cerebral white matter abnormalities and a selective involvement of particular brainstem tracts. The tracts involved included the pyramidal tracts, sensory tracts, superior and inferior cerebellar peduncles, and intraparenchymal trajectories of the trigeminal nerve. In four patients spinal MRI was performed and revealed involvement of tracts over the entire length depicted. Single voxel proton MRS in three patients revealed increased lactate within the abnormal white matter. The uniform and highly characteristic MRI findings, in combination with the similarities in clinical and MRS findings, provide evidence for a distinct nosological entity.

[Three cases of non-differentiated leukodystrophy with involvement of cerebral hemisphere's white matter and cerebellum and specific lesions of cerebral pathways. Neuroimaging and clinico-biochemical characteristics].

We presented three cases of the non-differentiated White Matter's disease. There are clinical resemblance and the similar MRI patterns including MR spectroscopy in all patients. They are outpatients: girl (K), 7 years old, boy (N), 13 yrs and young men (N), 24 yrs without any family relationship, and were investigated in Burdenko Neurosurgical Institute, in 2002 year. The cerebral MRI examinations all of that patients have shown extensive abnormalities in the hemispheric's white matter of cerebrum and cerebellum. Meanwhile there are some specific lesions of the brain stem's tracts (the posterior limb of the internal capsule, the splenium of the corpus callosum, the medial lemniscus and the mesencephalic trigeminal tracts). Step by step progression of suffer ness with unknown time of beginning, mild mnestic reduction and hemyparesis and moderate cerebellar disorder without loss of sensitivity were characteristic for all patients. The results of the biochemical markers for leucodistrophy testing are negative. A proton spectrum of the abnormal white matter showed elevated lactate in one case (girl, 7 yrs). In our opinion clinical diagnosis in all three cases is the new pathology that some body named as "WMDL" (White matter disease with lactate). It should be differentiated with adrenoleucodistrophy, Refsum disease and NARP syndrome (neuropathy, ataxia, pigmentive retinitis).

A double-blind, prospective, randomized, multicenter group comparison study of iopromide 240 vs iohexol 240 in myelography.

The aim of this study was to evaluate the safety and efficacy of iopromide 240 mgI/ml in comparison with iohexol 240 mgI/ml in myelography. A total of 421 patients in seven centers and four countries received an average of 11.9 ml of either iopromide 240 (278 patients) or iohexol 240 (143 patients) for X-ray and/or CT myelography in a randomized (2:1), prospective, double-blind study. All patients were followed up 3-4 h after the procedure, and 327 patients remained hospitalized for 24 h. In 82 patients an EEG was recorded prior to as well as 3-4 h and 24 h after myelography. Physical examinations, including measurement of vital signs, were performed in all patients at these time points. The results were subject to statistical analysis with the primary variable being the incidence of adverse events. Both contrast media (CM) were equally effective in terms of opacification. The rating for opacity was "good" or "excellent" in 88 % for both CM. Four patients (iopromide group: n = 3; iohexol group: n = 1) had transient EEG changes but did not show clinical symptomatology. The overall rate of patients experiencing any adverse event (AE) was 16.9 % for iopromide 240 and 14.0 % for iohexol 240. Equivalence testing was inconclusive; however, the results indicated equivalence. The rate for AEs considered as study-drug related was slightly lower with iopromide 240 than with iohexol 240 (7.2 vs 7.7 %, respectively). Neither unknown nor unexpected AEs known for myelographic X-ray CM nor serious adverse events were observed. Iopromide 240 and iohexol 240 are equally safe and effective and can be recommended for myelography.

The use of hyperventilation in contrast-enhanced MR of brain tumors.

Angiographic studies have demonstrated improved visibility of glial tumors after hyperventilation. The present study was undertaken to determine whether hyperventilation would change the MR enhancement characteristics of various glial tumors. Eighteen patients were studied twice: once with standard contrast-enhanced MR imaging and again with standard imaging plus hyperventilation. After hyperventilation, six low-grade astrocytomas showed no change and three showed a small decrease in relative enhancement (<10%). The ependymomas showed a 10% to 13% increase in the degree of enhancement, but no change in the area of enhancement. All the anaplastic astrocytomas showed an increase in the degree of enhancement (mean, 38%). Three of the anaplastic astrocytomas showed new foci of enhancement that were not seen on the nonhyperventilation study. Hyperventilation appears to be an inexpensive and safe method for increasing the conspicuity of abnormal areas of the blood-brain barrier.

Removal of a cranio-orbital foreign body by a supraorbital-pterion approach.

Foreign body removal in blind, penetrating craniocerebral injuries, especially those involving craniobasal localization, could entail additional brain trauma risk. Under such conditions one can use surgical approaches for tumor removal or clipping of intracranial aneurysms. In this report, we present a case of blind, penetrating craniofacial injury in which a supraorbital-pterion approach for foreign body removal was used. Craniography, carotid angiography, and computed tomography data, including three-dimensional imaging, were taken into consideration while planning the operation.

Late diagnosis and removal of a large wooden foreign body in the cranio-orbital region.

The rare case of a large wooden foreign body impaled in the cranio-orbital region, and its late diagnosis and successful removal, is presented. A 26-year-old man was admitted to a regional hospital after suffering a severe penetrating craniocerebral injury from a motorcycle accident. Two months after the accident, computed tomographic examination at Burdenko Neurosurgical Institute revealed a large foreign body located in the cranio-orbital region and penetrating the right temporal lobe, with surrounding abscess development. Radiological examination, including three-dimensional computed tomography, enables one to choose the optimal surgical approach and to remove the foreign body, thereby avoiding purulent, inflammatory complications.

[Clinical aspects, diagnosis and treatment of various forms of viral encephalitis in children].

The authors provide the clinical and laboratory data on the etiological structure of acute viral encephalitides in children. Special attention is devoted to the description of the clinical picture of the gravest form of encephalitis caused by type I herpes simplex virus. Present the laboratory data on the long-term preservation of type I herpes simplex virus in the CNS, which correlates to the clinical and morphological findings of the subacute or recurrent course of the disease. Discuss the results of the treatment with the antiviral drug acyclovir (zovirax).

[Magnetic resonance tomography in evaluating the efficacy of the radiation therapy of tumors in the upper respiratory tract].

Analysis of the diagnostic potentialities of MR-tomography in the assessment of radiotherapeutic efficacy was performed in 11 patients (malignant tumors of the upper respiratory tract in 10 and juvenile nasopharyngeal angiofibroma in one). Repeated investigation was performed after abatement of local radiation reactions (usually after 4-8 weeks). MR-tomography permitted the assessment of the state of the accessory sinuses of nose, the detection of a residual tumor, and the differentiation between tumors and inflammatory changes of the mucosa. In one case radiotherapy resulted in the regeneration of posterolateral wall of the maxillary sinus.

[Computerized-tomographic examination after surgical treatment of chronic subdural hematoma].

On the basis of an analysis of changes in 43 patients with chronic subdural hematomas before and after surgical treatment the times have been established of complete disappearance of chronic subdural hematoma using closed external drainage bearing in mind the patient's age, hematoma standing, the clinical phase of disease and craniocerebral correlations. It has been proved that the use of sparing treatment methods enables one to attain reexpansion of long compressed chronic hematoma of the brain even in senile and middle-aged persons.

[Selective catheterization and embolization of the branches of the external carotid artery in neurosurgical clinical practice].

A method for selective catheterization and angiography of the branches of the external carotid artery and their subsequent embolization is described. Selective catheterization of arteries for diagnostic and therapeutic purposes was carried out in 148 patients (183 vessels). Embolization of the branches was performed in 79 patients, a total of 111 arteries were occluded. The method was successfully used in the treatment of arteriosinusal anastomoses and arteriovenous aneurysms supplied with blood from the branches of the external carotid artery, severe nasal hemorrhages, and as a first stage before operation for richly vascularized meningiomas, angiomas of the maxillofacial region, and tumors in the region of the neck.

[Clinical picture of cranio-cerebral trauma combined with trauma to the cervical region of the spine].

On the basis of 60 observations the peculiarites of clinical manifestations of skull-brain injuries combined with trauma of the cervical vertebrae are described. It is demonstrated that in such cases the syndrome of the cerebral lesion often plays the leading part, masking the symptoms of the spinal cord injury. The disorders in circulation in the vertebral-basilar system due to the compression of the vertebral artery canal by the dislodged fragments, its thrombosis, and irritation of the periarterial sympathetic plexus are of importance in the genesis of the developing syndrome in the rombencephalic portion of the brain. In the mechanism of vestibular disorders a certain place belongs, apart from the direct traumatization of the rombencephalic portion and circulation disorders in the vertebral-basilar system, to injuries of the vestibulospinal pathways. The autonomous-visceral pathology observed in cases of cervical injuries can be attributed to the direct effect of the trauma upon the segmental innervation appratus of the heart, diaphragm, thorax.
