[Short- and long-term immune status in people after radiation injury as a result of the accident at the Chernobyl Nuclear Power Station].

Humoral and cellular factors of the immune system of 133 subjects injured in the Chernobyl accident were studied during first 1.5-2 months after radiation, 5-17 months and 3 years after they suffered acute radiation sickness (I-IV degree). Significant disorders in the immune system correlating with the severity of the disease were recorded. In late terms certain shifts persisted in the immune status of subjects who had suffered acute radiation sickness, II and III degree.

[Prevention of infectious complications in patients with acute bone marrow depression].

Clinical efficacy of intravenous gentamicin in combination with oral use of gentamicin, ristomycin and nystatin was studied in 1977. In 1980-1984 two antiinfectious regimens were tested: intravenous administration of gentamicin in combination with total decontamination (oral use of gentamicin and nystatin) and selective decontamination (biseptol with nystatin of amphoglucamine). It was shown that the incidence of severe infections in the patients under the observation dropped against the controls. The incidence and spectrum of infectious complications in cases with acute myelodepressions were proved to depend on the intestinal autoflora inhibition.

[The issue of paid medical care].

The paper deals with the problem of improving the relationships between the state system of public health and medical cooperatives and cost-accounting organizations with a view of increasing the efficiency and quality of health care delivery. It has been shown that the paid system of health care delivery is assuming ever greater importance. On the basis of one's own investigations and practical measures introduced in the Samara region, recommendations are given to improve the cooperation between the Department of Health and medical cooperatives, an analysis is made of the reasons for insufficient development of paid system of health care in the country.

[Experimental colonic prosthesis using xenograft materials].

The authors by means of electrohydrodynamic modelling on a resistance paper substantiated the sizes of a graft necessary for colonic prosthetics. In the experiment on 16 mongrel dogs, a possibility to use xenograft materials for plasty of the colon is shown.

[Predicting the complications of hydronephrosis in children after surgical interventions on the pyelo-ureteral segment].

On the basis of mathematical analysis of the long-term results of operative treatment of 119 children with unilateral hydronephrosis caused by congenital obstruction of the pyelo-ureteral segment, the prognostically significant signs (risk factors) having unfavourable effect on the outcome of an operation were revealed. In retrospective analysis, the main types of the course of unilateral hydronephrosis in children were distinguished. In stable course, the conservative therapy is indicated, in non-stable--the intensive medicamentous therapy, or repeated corrective operation, in progressive--nephroureterectomy .

[Experience with socio-psychological evaluation of the activities of public health administrators].

The study deals with new approaches to the selection, improvement and training of leading and managerial staff in the system of public health. This task is of exceptional significance in the course of transition to the new economic mechanism in public health. The psychometric methods of studying the personality of health leaders permit us to optimize and objectify the entire process of work with management staff, to select ones which are most suitable for this role, to avoid unnecessary expenses, to recognize the potentials and aptitudes of each manager.

[Characteristics of clinical manifestations of the developmental defects of the ureters in children].

Analysis of the clinical manifestations of the obstruction of the pelviureteral segment, of the vesicoureteral reflux, reflux and obstructive megaureter, ureterocele and ectopia of the ostium ureteris in 423 children has demonstrated that the clinical symptomatology of the abnormalities depends to a great measure on the degree of urodynamic disorders. The developmental abnormalities that provoke the obstruction of the initial part of the ureter manifest themselves primarily by the painful syndrome. Provided the urodynamics is disturbed in the inferior parts of the ureter, urination disorders become first and foremost.

[Effect of polyvinylpyrrolidone on microbial cells].

The oral administration of polyvinylpyrrolidone to rats produces no effect on the intestinal microflora. At the same time, polyvinylpyrrolidone solutions decrease the toxicity of Escherichia coli and Proteus vulgaris, produce a bactericidal and agglutinating effect.

Normalizing effect of immunoglobulins in the treatment of endogenous infection and intestinal dysbacteriosis in irradiated mice.

It has been demonstrated in experiments on 2160 CBA mice and mice of mixed breed, irradiated by LD90/30, that therapeutical administration (subcutaneous or intraperitoneal) of immunoglobulins - homologous (polyglobulin, IgG, IgM) or heterologous - polyglobulin, IgG (from human, equine, canine blood) repeated three times, i.e., 2, 24 and 48 hours after irradiation, not only induces longer survival, but also shows a normalizing effect on the commonly developing dysbacteriosis and increased amount of intestinal microflora and, in addition, leads to suppression of postirradiation endogenous infection. Enterobacteria, enterococci, staphylococci, yeasts, disappear from the small intestine or their quantity decreases substantially in treated irradiated mice in contrast to untreated irradiated mice. In the large intestine, the amount of these organisms decreases considerably while the content of lactobacteria increases; no microbes can be found in the internal organs and in blood (or their content is small). Other conditions being equal, homologous immunoglobulins are more efficient in comparison with heterologous, this applying also to preparations containing normal antibodies to tissues.
