[Sotahexal pharmacodynamics in patients with ischemic heart disease].

Therapeutic and prophylactic antiarrhythmic efficacy of sotalol hydrochloride (Sotahexal, "Hexal", Germany) and its effects in intracardiac hemodynamics and ECG parameters were evaluated in 95 patients with ischemic heart disease (IHD). The highest response to the drug was observed in ventricular extrasystoles, arterial flutter and fibrillation. Acute episodes of arrhythmia are managed by bolus administration of Sotahexal [correction of Hexal]. In this case greater risk of side effects exists. It is desirable to decide on the drug dose, mode of administration on the individual basis with due consideration of the risks and dangers which could be avoided in case of adequate instrumental control.

[The pharmacodynamics of the new anti-arrhythmia preparation bonnecor based on the data from an acute drug test].

The pharmacodynamics of the new antiarrhythmic agent bonnecor was studied in 33 patients with coronary heart diseases and ventricular arrhythmias in the acute drug test. The parenteral formulation of the drug (0.04-0.8 mg/kg) and its tablets were given to 23 and 27 patients, respectively. Bonnecor was found to be effective in suppressing ventricular premature contractions. Its intravenous and oral efficacy is 65 and 70%), respectively. During intravenous injection, the drug showed its antiarrhythmic effect on an average of 14 minutes later, lasting about 68 minutes. During oral administration, the action of the drug started 0.5 hour later and lasted 7.2 hours. Adverse reactions were rare. An echocardiographic study revealed its negative inotropic effect which is likely to be insignificant since there was no development or increase of heart failure in any of the examinees (including 8 patients with signs of Stage I-IIA circulatory insufficiency).

[Effect of ethacizine on the heart conduction system and the indicators of central hemodynamics].

A trial of the antiarrhythmic drug ethacizine made in the Soviet Union which is an analogue of ethmozine was performed in 38 patients suffering from coronary heart disease. Automatism, conduction and excitation of the myocardium, hemodynamics and the antiarrhythmic effect were studied using electrophysiological methods, echo-cardiography and Holter monitoring. Ethacizine was found effective in arrhythmia. It reduces conduction in small changes of hemodynamics and myocardial contractility.

[Assessment of anti-arrhythmic activity of palpitin and cardio-hemodynamic effects of its administration].

Palpitin, an agent of the disopyramide series, was evaluated for its antiarrhythmic potency in the treatment of ventricular and supraventricular arrhythmias and for its effects of ECG and central hemodynamic parameters in 108 patients (91 had coronary heart disease, 8 presented with postmyocardial cardiosclerosis, 5 had hypertensive disease, and 4 suffered from neurocirculatory dystonia). 24-hour ECG monitoring and echocardiography were employed in the study. It was concluded that the drug possessed a high antiarrhythmic potency. No substantial changes were found in the mean values of echocardiographic parameters, as well as of atrioventricular and intraventricular conduction.

[Dry immersion in combined therapy of edema syndrome and its effects on hemodynamics].

Therapeutic 'dry' immersion (modified 'dry' immersion technique suggested earlier for simulation of the weightlessness state in cosmic research) was used in multiple-modality treatment of 15 patients with drug-resistant edematous syndrome of different origin. Therapeutic 'dry' immersion sessions lasting from 1.5 to 6 hrs potentiated the effects of diuretics and promoted compensation, thus improving central hemodynamics parameters.

[Complex ultrasonic examination in the diagnosis and evaluation of cardiac function in right-ventricular infarction].

Combined ultrasound investigation (two-dimensional, contrast, Doppler, Mechocardiography) was performed in 32 patients with acute myocardial infarction (AMI). Echo- and electrocardiographic evidence and clinical symptoms suggested AMI of both ventricles in 22 patients. Ten patients with AMI in the left ventricle only made up the control sample. Right cardiac compartments were probed to verify right-ventricular insufficiency. Right-ventricular involvement was confirmed by autopsy in 6 patients. Clinical signs of right-ventricular AMI were detected in 63.6% of second-group patients, and electrocardiographic signs of the same, in 54.6%. Asynergic areas were detected in right-ventricular wall of 90.5% of patients. In cases of right-ventricular AMI, right compartment section area was increased significantly, while right-ventricular evacuation fraction was as significantly reduced. Doppler echocardiography demonstrated a low right-ventricular rapid filling peak rate, as compared to the respective slow filling rate, an indirect indication of right-ventricular AMI. Combined ultrasound investigation is therefore an effective tool for the diagnosis of right-ventricular AMI and assessment of heart performance.

[Contrast echocardiography using hydrogen peroxide].

The practicability of using hydrogen peroxide solutions for contrast echocardiography was examined. The administration of 1-3 ml of 0.2% solution mixed with a drop of the patient's blood, into the cubital or catheterized subclavian vein produced intensive and lasting (up to 2 min) contrast of the right cavities of the heart. Cubital injection of the solution was associated with a more lasting effect, as compared to the subclavian administration. The causes of this difference are yet to be investigated. The contrasting effect was fairly persistent and marked even when concentrations of the hydrogen peroxide solutions were reduced to 0.15 or 0.1%. The rate of administration was not more than 1 ml/s and had no negative effect on the quality of the contrast. No side effects associated with repeated administration of 1-3 ml hydrogen peroxide solution in the above-mentioned concentrations were noted in either of the 20 patients thus examined.
