[Surgical treatment of patients with cholestatic hepatitis].

The experience with treatment of 86 patients with cholestatic form of hepatitis has been summarized. The operative intervention included drainage of the extrahepatic bile ducts, denervation of the hepatoduodenal ligament, marginal hepatic resection. In ineffective conservative therapy, the patients should be operated on at the early period. When establishing the indications for the operation, the functional state of hepatocytes should be considered.

[Managing the postoperative period after gastric resection in an experimental study].

In 60 mongrel dogs of both sexes, gastric resection was performed under endotracheal narcosis. Depending on peculiarities of conducting the postoperative period, the animals were subdivided into three groups: in group 1 animals, total parenteral nutrition was used, in group 2--total enteral nutrition, in group 3--combined therapy. Combined parenteral (dextrans) and enteral (energetic and plastic substrates) administration of drugs and nutrients is an optimal method of corrigative therapy after resection of the stomach.

[Puncture aspiration biopsy under ultrasonic control in the diagnosis of breast neoplasms].

The authors studied the results of sonography of the breast and zones of regional lymph drainage in 15,973 patients. In 154 of them puncture aspiration biopsy was conducted under ultrasonic control. Breast carcinoma was found in 23 patients, in 2 of them the tumor was not palpable. The examination was performed on the Aloka-SSD-630 apparatus. The sonographic picture was that of a structure with diminished echogenic properties, irregular notched contours, and non-uniform echo signals from the internal structures. Echography with puncture biopsy made it also possible to reveal the presence of nonpalpable axillary metastatic lymph nodes and to establish the precise differential diagnosis of benign and malignant intraductal tumors. The method of puncture aspiration biopsy under control of ultrasonic examination is highly effective in the differential diagnosis of breast carcinoma.

[Injuries of the heart].

The authors had 53 patients with heart injuries under observation; 52 patients were operated on for vital indications. The heart wound was sutured, the concomitant injuries to other organs were removed, and the pericardial and pleural cavities were drained. Air-tightness of the injured heart muscle was attained in all cases. Seven of the patients who were operated on died. One patient who did not undergo operation died suddenly in complete well-being from acute cardiac tamponade 2 days after injury to the chest inflicted with a pin-like object. The authors emphasize the importance of organizational measures promoting earliest performance of the operation.

[Gynecomastia in children].

The experience with treatment of 34 children with gynecomastia is summarized. The examination included roentgenography of the Turkish saddle, definition of bone age, investigation of buccal sex chromatin, excretion of 17-ketosteroids, excretory urography against the background of retropneumoperitoneum, ultrasound investigation of the mammary glands. In juvenile gynecomastia, the mammary glands were removed, the expectant tactic being considered unwarranted. The long-term results of the surgical treatment are good.

[Chronic gastritis in patients following cholecystectomy].

A study of 198 patients with chronic gastritis after cholecystectomy revealed that chronic gastritis occurred in 2.6-4.5% of patients with benign diseases of the biliary tract and postcholecystectomy syndrome that were not diagnosed before surgery. Gastric disorders should be considered in the complex of rehabilitation measures after cholecystectomy. Treatment in a gastroenterological department and at health resort will improve long-term results.

[Double kidney and vesico-ureteral reflux in children].

Our clinical material has shown that results of organ-preserving operations on patients with doubling of the kidneys and vesicoureteral reflux are dependent on a number of factors. The leading factors are: degree of pathomorphofunctional alterations of the calyceal-pelvic system and parenchyma of the kidneys, the presence of pyelonephritis. After operations all the patients should be followed-up at dispenseries.
