[Use of less invasive techniques in surgical treatment of dumping syndrome].

Aiming to minimize the surgical trauma and interventional risks of treatment, as well as to reach the most precise visualization and safe separation of the n. vagus, in order to increase the efficacy of truncal vagotomy during correction of severe post-gastric-resection dumping syndrome we have developed the new method of thoracoscopic supradiafragmal vagotomy. The suggested method of bilateral thoracoscopic supradiafragmal truncal vagotomy allows to decrease the surgical intervention trauma, and the risk of post operative complications, as well as to better the immediate and remote results of treatment of the dumping syndrome.

[Resection with pyloric retention as a method for prophylaxis of dumping syndrome].

The aim of the study was the evaluation of efficacy of the resection with pyloric retention proposed by the authors as a method for prevention of post gastroresectional damping syndrome in patients with ulcers. Resection with pyloric retention is performed in six modifications of three basic methods of resections. We have the experience of using this approach in 712 clinical cases. Among them 237 were women and 475 were men 19-75 years of age. It was shown that resection with pyloric retention provides rhythmic evacuation and prevents development of damping syndrome and duodenal reflux. It was found that the main reasons of the development of postgastroresectional syndromes include wide-area mobilization of the stomach and resection of the small curvature, which induce insufficiency of cardia, atony, chronic disturbances of the duodenal passing. Due to the retention of pyloric function damping syndrome have been developed only in 3 patients. In all these cases the syndrome was mild. It may be concluded that gastric resection with pyloric retention represents the most "physiological" intervention and has many advantages in comparison with other methods.

[Intraoperative ultrasonic scanning of extrahepatic bile ducts].

The intraoperative ultrasonic investigation (IOUSI) of bile tracts was used during laparoscopic cholecystectomy for chronic (53 patients) and acute (25 patients) calculous cholecystitis and during 74 open operations in patients with mechanical jaundice. In half of the cases IOUSI was combined with intraoperative cholangiography (IOCG). A conclusion made on the basis of the clinical material suggests that IOUSI allows better assessment than IOCG of the state of the bile duct walls and pancreas head, the consistency of the duct content and can detect small (2-5 mm) concrements more exactly. However IOCG is more effective in determining the function of the bile tracts. So, the methods are thought to be not competing but mutually supplementing.

[Prophylactics and treatment of pyo-inflammatory complications after appendectomy].

The developed method of prophylactics and treatment of pyo-inflammatory complications in the wound after operation of appendectomy with the help of mid-frequency ultrasound with dioxidin as phonophoresis allows to make suppurations in the postoperative patients 5.15 times rarer and the number of pyo-inflammatory complications 6.68 times rarer. The time of healing the wounds was 1.2+/-0.3 days shorter and the duration of treatment of such patients in the hospital 1.8+/-0.8 days less.

[Treatment of patients with postgastrectomy syndromes].

An experience with treatment of 545 patients with organic, functional and combined postgastroresectional syndromes (PGRS) is analyzed. The patients are divided into groups depending on the character of the syndromes and their combinations. The most frequent causes of the development of PGRS are formulated. Special methods of preoperative examination and preparing are described which allow individual approach to choice of the method of reconstructive-restorative operations. Some technical details of the reconstructive interventions are described. In 95% of the patients operated upon long-term results of treatment were good and satisfactory.

[The individualized choice of the method of operation in the planned surgery of gastric and duodenal peptic ulcers].

A working classification of gastroduodenal ulcers was developed by the authors on the basis of surgical treatment of 1838 patients with ulcer disease of the stomach and duodenum. When used in combination with the complex examination of the patients it allows to individually choose the operative method. The indications and contraindications for different kinds of surgery are given. Specific features of the postoperative management are described.

[An analysis of the treatment indices of patients with acute surgical diseases of the abdominal organs in Saint-Petersburg over 50 years (1946-1996)].

The article analyzes results of treatment of 900 thousands patients with acute surgical diseases of organs of the abdominal cavity. Factors responsible for unfavorable results are shown. The authors consider that reduced lethality depends on strict observation of the main principle of the medical doctrine in emergency surgery and on wider introduction of endovideosurgical technologies into practice.

[A method of treating high ulcers of the posterior gastric wall].

The method of operative treatment of high ulcers of the posterior gastric wall has a number of advantages and is technically less difficult than other available methods. The use of the method includes preservation of blood supply of the gastric stump and pyloric sphincter, resection of the posterior gastric wall with ulcer, creation of the gastric tube from the anterior wall and putting a direct anastomosis with the preservation of the pyloric sphincter which allows avoidance of incompetence of the anastomosis sutures and such late disturbances as dumping syndrome, gastric stump gastritis and reflux-esophagitis. The method was used in operations on 6 patients and gave good results.

[Pathological states following vagotomy].

The work generalized the authors' experience with the treatment of 83 patients with pathological states after vagotomy. On 61 of them operations were performed, 22 patients were subjected to conservative treatment. Organic disorders were revealed in 48.4%, functional disturbances were found in 22.9%; 28.7% of the patients had associated disorders. The conservative treatment was not effective enough. Good long-term results after operative reconstructive-restorative operations and resections of the stomach were obtained in 96.7%.

[Reconstructive surgery in postgastrectomy syndromes].

An experience with the treatment of 429 patients with postgastroresection syndrome was generalized, in 231 out of them (53,8%) associated pathological conditions had been revealed. The selection of the method of treatment was dependent on clinical manifestations of the syndrome and its causes. The surgical treatment was used in 226 patients (52,7%). Resections occupied the first place (69,5%) among the reconstructive operations. Long-term results in 95,2% of the examined patients were excellent, good and satisfactory.
