Humoral immune responses in osteomyelitis.

Fifty-three patients with osteomyelitis were grouped according to the duration of the disease at presentation, the bone and the part involved, the aetiology, activity, severity and stage of disease. Sera from these patients was estimated for IgA, IgM and IgG levels and in 23 matched controls. The results were compared with the normal range, and correlative studies were carried out between serum immunoglobulin levels and the different clinical parameters. Serum levels of the 3 immunoglobulins were found, in general, to fail to rise above the normal in the presence of infection. Furthermore, the levels did not show significant variation with the clinical parameters. These findings suggest that there is an intrinsically subdued immune response to osteomyelitis in most of the patients, partly confirming the suspicion of an immunodeficient state.

Adult respiratory distress syndrome following non-thoracic skeletal trauma.

Thirty adult patients with severe non-thoracic skeletal trauma were monitored over the first five days for evidence of adult respiratory distress syndrome (ARDS). Three of the patients developed progressive respiratory distress, refractory hypoxaemia and pulmonary infiltrates suggestive of ARDS. Two patients died while one recovered completely. Close monitoring is suggested for early recognition of the disease and institution of therapy.

Role of allogenic vesical mucosa in inducing neo-osteogenesis in guineapigs.

Fresh allogenic mucosa lining the urinary bladder was transplanted into a 1 cm gap of right ulna bone in 24 guineapigs. The 1 cm, bone gap in the left ulna served as control. At regular intervals, the implanted and control sites were studied grossly with naked eye, radiologically and histopathologically to ascertain the role of allogenic vesical mucosa in inducing neo-osteogenesis. The bone gaps were uniformly bridged by bone in 10-12 wk at the implanted sites and fibrous tissue at control sites. There was no evidence of graft rejection. The study thus suggests the potential of the use of stored allogenic vesical mucosal grafts for neo-osteogenesis in clinical practice.

Management of chronic osteomyelitis using an irrigation suction technique.

Fifty cases of chronic osteomyelitis were treated by closed, continuous, suction and irrigation and followed up for between six months and eight years. In group A, 25 cases were treated by irrigation with antibiotics, and in group B, 25 cases were treated by irrigation with Bitadine solution, irrespective of the culture and sensitivity reports. Appropriate antibiotics were given systemically. There was no notable difference in the results in the two groups, with an overall success rate of about 80%.

Radiological score in the assessment of clubfoot.

The clinical appearance and the radiographs of 31 patients with 47 clubfeet taken before the start of treatment have been studied. In spite of making a careful clinical evaluation, 14 feet (29.8%) remained "borderline" and could not be definitely classified into the easy or resistant groups. The radiological score of the foot, which is an objective representation of the severity of the deformities at all important levels and in all planes, has been found to be an accurate and sensitive index which enables such a distinction to be made.

The floating knee. 40 cases of ipsilateral fractures of the femur and the tibia.

"Floating knee" is the term used to describe simultaneous fractures of the femur and tibia in the same limb. Thirty nine patients, with 40 such injuries, are presented with a follow up interval of six months to five years. The final functional result was poor if the femoral fracture was situated in the condylar flare and the results were comparatively better in those cases treated by cast bracing or when the fracture of the femur was stabilised internally. In all cases the fracture of the tibia was treated conservatively.
