[Subtotal radical resection and primary reconstruction in breast cancer].

The authors present information on 44 patients with local breast carcinoma (Stages II and III) who were treated by a new type of operation in the form of radical subtotal resection of the breast and one-stage reconstruction without the use of endoprostheses. The authors describe some scientific and technical problems of the performance of these complicated surgical interventions. In future these operations may become operations of choice for a certain category of patients because they solve both therapeutic and rehabilitation problems.

[Laser scalpel in organ-preserving surgeries for early stages of breast cancer].

The work describes the peculiarities of radical resections of the breast with high-energy CO2 laser in the treatment of patients with early stages of breast carcinoma. The advantages of the laser scalpel are pointed out. Twenty radical operations were conducted for breast carcinoma with the use of a CO2 laser. The operation did not last longer practically, blood loss diminished by half (from 350 ml to 150 ml), and the wounds healed in the usual periods. Reduced pain sensitivity in the region of the wound was noted in the postoperative period. The study is continued.

[The use of the carbon dioxide laser in the surgical treatment of breast cancer].

The paper discusses peculiarities of breast cancer surgery using high-energy CO2 laser. Advantages of laser scalpel are discussed. CO2 laser was employed in 120 cases of breast surgery including 70 operations for cancer (radical mastectomy and radical resection--35 cases each). Operative blood loss was reduced by half (from 350 to 140 ml). The duration of surgery and wound healing did not increase. Postoperative pain was less severe. The study is in progress.

[CO2 lasers in the treatment of breast cancer].

The paper is a review of the experience gained by foreign and Soviet (the Moscow Cancer Research Institute) investigators in using high-energy CO2-laser fo treatment of breast cancer. It is demonstrated that employment of laser scalpel improves operative results, does not induce complications, prolongs survival and recurrence-free period of breast cancer patients.

[Criteria of selection of patients with breast cancer for organ-sparing operations].

The analysis is given to a number of criteria guiding the selection of breast cancer patients for preserving surgery. The tumor size is one of the most essential criteria. Breast-sparing surgical treatment within radical resection of the breast according to the above selection proved not inferior by survival characteristics to radical mastectomy surpassing it by social and occupational results.

[alpha 1-proteinase inhibitor in breast cancer].

Level of alpha 1-proteinase inhibitor (alpha 1-Pi) and antitryptic activity in blood serum were assessed in 167 patients with breast cancer and 30 cases of benign lesions. An increase in blood serum-alpha 1-Pi level was shown to be associated with tumor advancement but could not be regarded as a marker of cancer. Concentration of the inhibitor exceeded 8 mg/ml in 83.4% of cases with metastatic breast cancer but was as a rule, equal to or below that value in metastasis--free patients. Those two groups were prognostically different. Morphological analysis established a correlation between blood serum-alpha 1-Pi level, on the one hand, and grade of malignancy, degree of pathologic changes in tumor stroma and extent of lymph node involvement, on the other.

[Errors in the diagnosis and treatment of thyroid cancer].

The analysis of the data on 744 patients with thyroid tumors showed inadequate examination to be the main cause of untimely diagnosis of cancer of the organ. Cytologic examination of tumor punctate proved most effective for diagnosis since it confirmed cancer in 92.3% of cases. Adequate examination and extrafascial surgical procedure with intraoperative contrasting of the thyroid assured five-year survival rate of 95.9% and a decrease in the risk of injury to n. recurrens by 23%.

[Errors in the diagnosis of malignant tumors during the prehospital stage].

A group of experts from The P. A. Herzen Research Institute of Oncology reviewed 6000 histories of cancer. The analysis showed errors by outpatient hospital staff to account for untimely diagnosis of cancer in the majority of cases of advanced disease. A classification of medical errors was developed. Objective and subjective factors leading to errors were analysed. Review of records of diagnostic data is suggested. It is also suggested that physicians should bear responsibility for errors in oncology which have grave consequences.

[Criteria for selecting patients with breast cancer for organ-saving operations].

The study discusses the results of treatment of 597 cases of stage I-IIa breast cancer who underwent either mastectomy after Halsted, Patey or Pirogov (434 patients), or organ-saving radical resection of the breast (163). The analysis of the data identified biologic, morphologic, psychologic and anthropometric criteria as well as laboratory tests which may serve as criteria for selecting candidates for organ-saving surgery and adjuvant radiotherapy. In a group of patients who met the criteria, sparing surgery yielded good results in terms of five-year survival rate (88.9%) and degree of socio-occupational rehabilitation.

[Stage IIIa (T3NOMO) cancer of the breast].

The article deals with information concerning the peculiarities of diagnosis, methods of treatment, and prognosis of stage IIIa breast cancer in 219 patients. It is shown that stage IIIa cancer is a form following a peculiar clinical course which is characterized by localized, often hardly noticeable or moderate rate of growth of the tumor and a relatively favourable prognosis. The tumors were multicentric in one fifth of the patients and histologically peculiar forms of breast cancer with a more favourable prognosis in one third. Combined treatment with the use of pre- or postoperative radiotherapy or chemotherapy produces satisfactory late-term results.
