[alpha-Interferon indices in systemic lupus erythematosus and the effect of interferon therapy].

The effect of interferon inducers of viral and nonviral nature as well as of exogenous leukocyte interferon on interferon response of leukocytes (IRL) was studied in patients with different rheumatic diseases, mostly in patients with systemic lupus erythematosus (SLE). Studies in vitro showed that synthetic polyribonucleotide poly(1) X poly(C) induced in SLE patients on the average 3 times as high IRL indices as the viral interferon inducer Newcastle disease virus. A two-week course of treatment of some SLE patients with exogenous leukocyte interferon in various dosages (10(6), 3 X 10(5), and 10(4) units) resulted in a 2--16-fold increase of IRL values in the majority of the patients.

[An immunosorbent for interferon purification].

Immunoaffinity chromatography was shown to be the method achieving the most complete elimination of antigenic admixtures from leukocyte interferon preparations without the loss of the preparation activity. An affinity sorbent has been developed on the basis of covalently linked polyvinyl alcohol (PVA). The immobilization of the antigen-specific rabbit globulin in the preparation of the sorbent is achieved by reaction of protein amino groups with the activated matrix. The proposed sorbent achieved the elimination of the antigenic admixtures from interferon preparations as effectively as those prepared on the basis of sepharose 4B, the productivity of the purification process being at least 5 times higher. The proposed sorbent is stable at the limit values of rH, is not destroyed by detergents, is sterilized in the process of preparation. Owing to the strong linkage of the immobilized immunoglobulin with the PVA-carrier, immunoaffinity chromatography on this sorbent does not involve contamination of the preparations with rabbit globulin allergenic for man. The combination of a large pore structure, wetting ability, stiffness, mechanical and chemical stability allows the proposed sorbent to be recommended for use in modern large-scale biotechnological production.

[The interferon reaction of the leukocytes and the natural killer activity in patients with acute viral hepatitis B undergoing interferon treatment].

The interferon response of leukocytes (IRL) alpha and gamma as well as the functional activity of natural killers (NK) were studied in patients with the verified diagnosis of acute viral hepatitis B (AVH-B) treated with leukinferon (a preparation of natural interferon-alpha and cytokines) and reaferon (recombinant alpha 2 interferon). The effect of therapy with LF and RF in patients with AVH-B manifested by an increase in the functional activity of NK up to complete normalization after termination of the course of therapy. In the group of patients treated with LF as a result of therapy IRL-alpha increased by the end of the treatment and in the convalescence period. The observed activation of NK and the IF-alpha system under the effect of IF preparations in patients with AVH-B correlated with enhanced elimination of HBsAg.

[The use of leukinferon in treating zoonotic cutaneous leishmaniasis. 1. Research on an experimental model in mice].

Leukinferon, a drug made in this country, represents a complex of cytokines of the immune response first phase with the predominating activities of interleukin-1, tumor necrosis factor, macrophage- and leukocyte-inhibiting factors and alpha-interferon; its effects on the murine immune system and on peritoneal macrophagal culture are diverse. Leukinferon activates phagocytosis and killing of L. major promastigotes with peritoneal macrophages. In vivo leukinferon in combination with monomycin had a marked effect on L. major infection in CBA mice, particularly at the tubercle stage. The cure is attained sooner than with monomycin therapy alone. Combined therapy of the ulcers was found less effective in BALB/c mice highly sensitive to the infection. In Swiss mice infected with L. major a prolongation of the incubation period and a more benign course of the infection were observed after a course of leukinferon injections.

[The use of a complex preparation of alpha-interferon and first-phase immune-response cytokines--leukinferon].

Cases of successful treatment of severe neurological complications, developing as a consequence of diphtheria intoxication, by injections of leukinferon, a new combined preparation of alpha-interferon and other cytokines of the first stage of immune response, are described. Toxico-allergic polyneuropathy with sensory, reflex, motor (tetraparesis) disturbances and the bulbar syndrome developed in males after nasopharyngeal diphtheria. For the first time effective and rapid therapy of the bulbar syndrome and other disturbances, developing in connection with the demyelinating toxico-allergic process, was shown to be possible due to the use of leukinferon in combination with other therapeutic measures. The preparation proved to be highly effective in the treatment given with a view to arrest the development of intercurrent infectious complications in severely ill, bedridden patients receiving prednisolone. Leukinferon produces a pronounced effect even in patients receiving massive doses of corticosteroids, thus making it possible to shortly abolish the use of these preparations.

[The use of leukinferon in treating zoonotic cutaneous leishmaniasis. 2. Patient treatment].

Zoonotic cutaneous leishmaniasis was treated by a combination of monomycin and an immunotherapeutic agent, leukinferon, representing a complex of cytokines of the first phase of in vitro immune response of human leukocytes with the predominance of the activities of interleukin-1, tumor necrosis factor, macrophage- and leukocyte-inhibiting factors, and alpha-interferon. A total of 182 patients were treated in endemic foci of zoonotic cutaneous leishmaniasis in Uzbekistan, 115 patients were followed up. Ointment applications (1000 U) or i.m. injections of leukinferon every 3-4 days were used in the treatment of 50 patients. Monomycin was administered only locally--as an ointment or collagen sponge ("leishmacol"). The best results were attained with combined therapy including i.m. leukinferon and local monomycin. The treatment duration was 3 weeks in grave complicated cases and 2 weeks in cases with a benign course of the disease. Local therapy with any of the drugs alone or combined was less effective, and lasted for about 1.5 months. The combined method is convenient for outpatient therapy, for it helps do without numerous daily injections that are necessary in parenteral monomycin therapy. Another advantage of combined therapy of zoonotic cutaneous leishmaniasis is an essential reduction of the total dose of monomycin, a highly toxic drug.

[The characteristics of the immune response in acute viral hepatitis B].

The clinical and pathogenetic importance of a number of features characterizing cell-mediated immunity and nonspecific protective factors in acute virus hepatitis B. 124 patients with hepatitis B virus (HBV) infection were placed under observation. Of these, 115 patients had acute virus hepatitis B, 6 patients had acute virus hepatitis of mixed etiology (B + delta) and 3 patients had chronic virus hepatitis B. The study included, besides the detection of virus hepatitis markers and the biochemical analysis of blood, the determination of subpopulations of peripheral blood lymphocytes (CD3, CD4, CD8, CD57), the functional activity of natural killers, characteristics of the interferon status, serum neopterin and beta 2-microglobulin in blood serum. Considerable changes in cell-mediated immunity and the interferon system were found to occur and the optimum immune response in acute virus hepatitis B was characterized.

[The clinico-immunological effects of leukinferon in the treatment of patients with acute suppurative pyelonephritis].

A many as 20 patients with acute purulent pyelonephritis were studied for clinical and immunological effects of leukinferone. The above patients were convalescent earlier than control subjects. There were instances where clinical status was not accompanied by shifts in immunity. However, on an average in the group, such parameters as level of lymphocytes, amounts of B-cells, Igs G, A, T-helpers, and Tx/Tc ratio were found to be increased as was activity of EK-cells. Stimulating effects of leukinferone were particularly pronounced in those patients with decreased values for immunological status. A conclusion is reached to the effect that patients with acute pyelonephritis may benefit from those treatments involving the use of the above drug preparation.

[Human bone marrow interferon].

Bone marrow cells from suddenly dying people can produce interferon in response to incoulation of an inducer. The interferon obtained in bone marrow cells did not differ by its properties from the reference preparation. the lots of bone marrow interferon which had been prepared were not inferior in the antiviral activity to leukocyte interferon or were even superior to it. It is suggested that bone marrow cells be used for preparation of human interferon.

[The clinical efficacy in using leukinferon in adults with diseases caused by the varicella-zoster virus].

To achieve more effective treatment of varicella (chickenpox) and herpes zoster in adults, a wide-spectrum immunocorrective agent containing, together with alpha-interferon, a number of other cytokines of the first phase of immune response was used. In patients with the different severity of disease leukinferon induced a rapid decrease in the severity of the disease, arrested the development of new elements on the skin and the buccal mucosa, and reduced the duration of the fever period. When used in such forms as intramuscular injections in combination with the irrigation of the buccal mucosa and ointment, leukinferon proved to be a highly effective preparation for the treatment of diseases caused by varicella-zoster virus.

[Therapeutic antiradiation properties of leukinferon].

In experiments with three species of animals (mice, rabbits, monkeys) irradiated with sublethal and lethal doses of gamma-irradiation, it was observed, that leukinferon used in 4 hours or 10 days after provided antiradiation therapeutical effect. When lethal dose was used (mice) the survival of animals was significantly increased. The preparation decreased depression of the functional activity of leukocytes as it was observed with IFH-alpha and gamma-production. Pestoring this function was accelerated. Phenomena pancytopenia and thrombocytopenia induced by radiation were decreased. Therapeutical effect was explained with capacity of the preparation to defence the hemopoietic organs from the radiation and to stimulate hematopoiesis. So, spleen weights and quantity of cells into bone marrow were higher in experimental group in comparison with control at any observation. CFUs were also higher (with optimal dose of preparation). Stimulation of the restoring processes was confirmed also by more intensive increasing of the reticulocyte quantities in the circulation of experimental monkeys within 20 days. Then their level became more close in both groups. The preparation stimulated also differentiation of immune effectors: immature neutrophils were absent in the circulation of experimental monkeys. Production of IFN-gamma (which is function of T-lymphocytes) was restored more fast. As a result, immunoreactivity of monkeys in experimental group was more complete and restored faster: the growth of the automicroflora on the skin surface became to increase 7 days later and was restrained. Leukinferon hindered the development of acute radiation sickness symptoms.

[Protective action of leukinferin during radiotherapy of oncologic patients].

In clinical observation on 248 patients with breast cancer IIb-III-b stages given radiotherapy on breast gland, parasternal and axillary-subclavian regions (total doses 40 Gr for each) before mastectomy was shown, that use LF im 2-3 times a week through the course of therapy decreases statistically significant pancytopenia, induced by radiation. Quantity leukocytes was even increase in a process of therapy. Frequency of anemia and lyphopenia was decreased almost 3 times. E-RFC were depressed also more slight, then in the group without immunocorrection and after mastectomy practically, restored. IFN-gamma production by T-lymphocytes was not changed and essentially more high then in the comparative group. The presented data are shown, that LF provides a defensive action on hematological indices and immune effectors function in oncological patients during radiotherapy.
