[Effect of combined psychocorrection on dynamics of pain syndrome and psychological status of patients with spinal osteochondrosis].

AIM: To study effectiveness of a new psychocorrection method (combined use of neuroleptic trifluoperasine and psychorelaxation therapy using biological feedback) in rehabilitation of patients with long-term pain syndrome provoked by spinal osteochondrosis (SOC) to determine predictors of response to this method of psychocorrection. MATERIAL AND METHODS: 132 patients with SOC and associated pain syndrome were examined using pain assessing methods and psychological tests. RESULTS: Psychocorrection including neuroleptic significantly raises the efficacy of SOC treatment and relieves pain, hypochondric fixations and depressive disorders. The reduction of pain syndrome intensity in the course of psychocorrection correlates with attenuation of hypochondric disorders and inclination to fixation. As to predictors of the treatment effect, the strongest relief of pain in psychocorrection was achieved in patients with hypochondric and anxiodepressive disorders. CONCLUSION: Combined psychocorrection comprising minimal doses of neuroleptic trifluoperasine and psychorelaxation is effective in the treatment of SOC patients with long-term pain syndrome.

[Comparative effectiveness of psychotropic drugs in patients with chronic ischemic heart disease].

As many as 348 courses of psychotropic and antianginal chemotherapy were performed for coronary disease confirmed angiographically. Hypochondriac patients were resistant to the drugs, depressive states and anxiety showed the best response. Tranquilizers and neuroleptic agents proved effective in anxiety, neuroleptics in cardiophobia and hypochondria, antidepressants in depression and asthenia. The addition of psychotropic drugs to antianginal therapy produced a positive effect on cardiac pain with antidepressants benefit ranking first, neuroleptics second and tranquilizers third. Being moderate and transient, treatment-related side effects did not cause the treatment discontinuation.

[Predictors of the efficacy of psychorelaxation therapy in hypertension].

Results of clinical, psychological and psychophysiological examinations of 136 patients with mild essential hypertension indicate that psychorelaxation therapy is more effective in patients with a higher level of BP response to emotional stress and a shorter period of BP recovery after stress. Those patients had a moderate level of anxiety and interpersonal conflicts. Predictors of the efficacy of autogenic training, breathing relaxation training and biofeedback differed. The antihypertensive effect of those methods was associated with various personality traits.

[The psychological predictors of the participation of hypertension patients in low-sodium diet therapy].

According to the Multifactorial rating scale, the Russian version of Mini-Mult, the Luscher color test and the "Quality of Life" test data the noncompliance patients were characterized by tension, anxiety, irritability, fatigue, low mood, interpersonal relations problems accompanied by the lowering of psychosocial adaptation and quality of life. On the basis of an objective psychological assessment the mathematic formulas for prediction of noncompliance as well as compliance towards hyposodium dietetic therapy have been elaborated.

[The efficacy of the hypnosuggestive psychotherapy of myocardial infarct patients].

The efficacy of the short-term psychotherapy was studied in 90 patients with myocardial infarction using the rating scale and the psychological Mini-Mult test. The hypnosuggestive therapy improved the mental status of the patients by reducing affective disorders. It promoted the formation of adequate psychological reaction to the illness and successful psychological adaptation, exerted a favourable effect on angina pectoris, cardialgia and moderate arterial hypertension.

[The comparative effectiveness of different psychological methods for treating patients with a labile course of hypertension (based on international research data)].

Examination of 110 patients with essential hypertension (EH) carried out in the USSR and GDR revealed, by the end of the observation period (one year), a significant lowering and normalization of arterial pressure at rest and a certain reduction of the intensity of the hypertensive response to psychoemotional stress influences in the presence of psychological relaxation therapy (PRT). The survey made it possible to try the designed methods for PRT in both countries and demonstrated that the intensity of the hypotensive action of the methods appeared comparable in different populations of patients suffering from essential hypertension. The intensity of the hypotensive action of PRT correlated with the level of arterial pressure before treatment, with the age of EH patients, and with their psychological characteristics.

[Diagnostic possibilities of the BIPI test in hypertension].

To gain an insight into the potentialities of the psychologic test Brief International Personality Inventory (BIPI), clinical and psychologic studies were undertaken to examine 390 hypertensive patients. The study provided evidence for wide psychodiagnostic potentialities of the BIPI in hypertensive disease. This test reflects the severity of mental changes, specific features of a psychopathological profile of patients with hypertensive disease and the time course of these parameters during follow-up and management. It may be useful both in the evaluation of hypertensive ' personality traits and in the differential diagnosis of psychopathological syndromes. It was shown that it was possible to use the BIPI in the prediction of effective therapy for arterial hypertension and length of temporary disability in patients with hypertensive disease.

[Mechanisms of the hypotensive effect of psychorelaxation therapy in hypertension].

An evaluation of the effect of mental relaxation treatment on endogenous opioid activity and vascular reactivity in 20 patients with labile essential hypertension has demonstrated that mental relaxation treatment results in a significantly greater drop in arterial BP, as compared to pharmacologic placebo, and is associated with the improvement of the patients' psychological status, lesser psychophysiologic and vascular reactivity, and a smaller beta-endorphin increment under emotional stress.

[Quality of life of patients with hypertension].

Diagnostic feasibilities of the methods for assessing the quality of life were studied in 260 patients with essential hypertension and 94 patients with a history of myocardial infarction. The reasons for lowering the quality of life were shown to be different in these groups in question. The life quality was found to be associated with the data of the psychological examination and indicators of the working capacity of patients with essential hypertension.

[Experience using nootropil in patients with chronic ischemic heart disease].

Course nootropil treatment was conducted in 34 IHD patients with neurotic and neurotic-like diseases. Dynamic changes in the psychic state were assessed by the clinical scale and the MMPI psychological test. Significant improvement or normalization of the psychic state was achieved in 58.8 per cent of the patients treated with nootropil. The best results were obtained in the treatment of asthenic states. Administration of nootropil had no influence on the incidence of angina pectoris attacks and produced no side effects.

[A method of rapid psychological diagnosis of patients with a history of myocardial infarction].

The aim of the study was to develop a compact questionnaire for analysis of the psychological status of patients who had survived a myocardial infarction (MI) meant for physicians with no special psychology or psychiatry training. For this purpose of 185 questions selected by expert method on the data of 236 cases of MI using the program of stepped discriminant analysis, 37 most informative questions were chosen. These questions allowed determination of the general response to MI (adequate or neurotic) as well as of the degree of overestimation or negation of the disease and the degree of anxious-depressive syndrome. On the basis of the obtained data a method of psychologic express diagnosis (PED) for assessment of the named variables was developed. PED is aimed at wide practical use for psychological screening of patients who had survived a MI.

[Use of tricyclic antidepressants in patients with chronic ischemic heart disease].

The results of a course of therapy in 187 men with proved ischemic heart disease (IHD) showed that tricyclic antidepressants in minimal and small doses may be effectively used in patients with IHD for treatment of psychic alterations as well as in chest pain syndrome. Amitriptyline was more effective than azafen or melipramine but significantly more often induced side effects. The data obtained demonstrated close relationship between the severity of psychic alterations and various clinical manifestations of angina pectoris as well as dependence of reduction of the anginal attacks frequency on the efficacy of psychic alterations therapy.
