[Fungal sinusitis. Report of four cases].

Fungal sinusitis has a nonspecific clinical presentation, is refractory to standard medical treatment and may produce expansion and erosion of the sinus wall. Various factors have been implicated in the development of fungal sinusitis: anatomical factors in the osteomeatal complex, tissular hypoxia, traumatic factors, massive exposure to fungal spores, allergy and immunosuppression. Our study of four cases diagnosed in the department included the clinical, pathological, and radiological features of fungal sinusitis. Five forms are described. Bacterial infection often occurs. The diagnosis of fungal sinusitis requires a high level of clinical and pathological suspicion. Before surgery, fungal sinusitis is suggested by the clinical and radiological features, and is confirmed by histological study. Culture of the tissue sometimes leads to identification of the fungus. Surgical removal of the infection is the main treatment for every form of fungal sinusitis.

[Lymphoepithelioma of the larynx].

Non-nasopharyngeal Lymphoepithelioma is a tumor of undifferentiated non-keratinizing cells enclosed in an lymphocytic stroma. The lymphoid tissue does not participate in the tumor process. Its tendency is to precociously spreading in the lymph nodes and viscera, but shows a good radiotherapeutic response. Unlike the nasopharyngeal lymphoepithelioma there is no etiological relationship with the Epstein-Barr virus, in spite of the histological similitudes, therefore the pathogenesis must be different in each. Discrimination between lymphoepithelioma and other neoplasms of the epithelial lineage is important in order to indicate the treatment and also to provide prognostic information.

[Review of ORL manifestations in AIDS. Case series at the Arnau de Vilanova Hospital, Lleida].

The majority of patients suffering AIDS-syndrome present with ENT aspects, being the most prevalent lateral adenopathy of the neck firstly, followed by thrush, pharyngo-esophageal candidiasis and herpes simplex. Differences regarding the distribution in percentage in risk groups are signaled by the AA. between their own statistics and those of the americans writers.

[Cervical cystic hygroma].

Hygroma being a rare overgrowth the AA. of the paper consider interesting the report of a case sitting in the neck, diagnosed and treated at Arnau de Vilanova Hospital. Memento concerning this entity.

[Recurrent polychondritis].

The interest of the relapsing polychondritis is specially due to its otolaryngological implications. The case reported evolved favourably and responded fairish to the corticoidtherapy. Nevertheless an assidous follow-up will be wise in order to stablish the pronostic in view of the outbreacks of the illness. The AA. compare the own case with others referred to in the literature.

[Tuberculous otitis media].

Report of a case of middle ear tuberculosis in an elderly woman with a chronic discharging ear withstanding the common treatment. The diagnosis could be made through examination of the pieces taken at surgery (tympanoplasty). The A. advice is to arouse suspicion when findings compatible with tubercular otitis are encountered, in order to treat wisely the patient and avoid complications.

[Otosclerosis surgery with CO2 laser].

Since Shea described the stapedectorny, many physicians have developed modifications of the technique, prosthesis or instrumentation. We have adopted one of the most recent advances to the treatment of otosclerosis, CO2 laser. Interventions were carried out in 20 consecutive ears using CO2 laser. Otosclerosis with type I, II and III foci were diagnosed and stapedectomies were reviewed. Stapedectomy was performed with a Causse prosthesis. We describe the technique and the laser power and application time used. The preoperative and auditory results at 1 month are reported. The advantages of opening otosclerosis with CO2 laser as the first or review procedure are analyzed.

[Inverted papillomas of the nasal sinuses].

After reviewing the Arnau de Vilanova Hospital's records, a total of ten cases of inverted papillomas were found. Nasal obstruction was present in 100% of the cases. External surgery was performed in 70% of these patients, finding that in 50% of them, the real size of the tumor was smaller than that show in the radiological study. After surgical treatment, only one of the patients developed a malignant tumor. Incidence of papilloma recurrence was 30%.

[Peritonsillar abscess: treatment guidelines].

A prospective study was carried out on 25 patients showing symptoms suggestive of peritonsillar abscess. Twelve patients were treated with i.v. antibiotics and three months later underwent tonsillectomy. Thirteen patients underwent acute tonsillectomy within 24-48 hours. In order to evaluate the benefits of acute tonsillectomy we compared three parameters; operation length, intraoperative bleeding and overall length of hospitalization.
